방사선 조사가 조직에 손상을 주는 기전은 미세 혈관 폐쇄, 광범위한 섬유화 및 염색체

변이에 의한 직접적인 세포 손상등을 들수 있다. 그러나 방사선 조사에 의한 조직의 손

상 정도는 시기별로 다른것으로 알려져 있다. 즉 급성기에는 모세 혈관의 투과성이 증가

하나 만성기로 갈수록 혈관의 수도 감소하고 내벽의 비후와 함께 내경이 좁아진다. 또한

조직의 섬유 성분도 증가하는것으로 알려져 있다.

한편 임의형으로 거상한 피판의 근위부는 기저부로부터 오는 혈관경의 영향을 받으나

원위부로 갈수록 혈관경으로 부터의 영양 공급 보다는 수혜부와의 혈관 재결합 밋 수혜부

로 부터의 혈관 증식이 생존에 중요하다.

따라서 방사선 조사후 거상한 임의형 피판은 방사선 조사에 의한 조직손상 정도가 심해

지기 전에 시행하는것이 생존에 유리할것으로 생각이 되며 반대로 시간이 경과할수록 피

판의 생존율은 감소할것으로 기대된다.

저자는 국소 방사선 조사를 받은 백서에서 수술 시기에 변화를 주어 임의령 피판을 작

도한 결과 6주 이후부터 피판 생존율이 감소하는것을 관찰하였고(p<0.05) 9주 이후 부터

는 심한 감소를 나타내며(p<0.001) 그 이후에는 생존율이 심하게 감소된 상태로 유지함을

관찰하였다(12주 P<0.001, 16주 P<0.01).

이상의 결과로 국소 방사선 조사가 백서의 배부에 임의형으로 거상한 피판의 생존에 미

치는 영향은 6주부터 나타나기 시작하여 9주 이후 부터는 생존율의 심한 감소상태를 유지

하는것을 관찰할수 있었다.

The effect of radiation on the tissue is closely related to the time after

irradiation. In the acute period, the capillaries are known to dilate and

endothelial permeability is known to increase. In the chronic period, the tissue

becomes fibrous and the number of blood vessels decrease.

In the random pattern skin flap, the main blood supply to the proximal site of

flap comes from its pedicle and to a lesser extent from its bed, but the distal

part acts like a composite skin graft and its take depends mainly on the recipient

bed. Therefore the changes in the vascular bed and fibrosis in the chronic period

pose the greatest challenge to the survival of local flap after irradiation.

Random pattern skin flaps were made on irradiated sites at 1, 3, 6, 9, 12 and 16

weeks after irradiation. The flap showed different survival area depending on the

time of operation. There is a mild decrease in survival rate at about 6

weeks(p<0.05) and a striking reduction in the survival rate at about 9 weeks after

irradiaition(p<0.001). After 9 weeks the surviving rate remains reasonably constant

as low(12week: p<0.001, 16 week: p<0.01).
