많은 사람들이 담석의 구성성분에 관해 연구해왔으며 이 결과로 담석은 Cholesterol, C

alcium Salt of Bilirubin, Calcium Car-bonate 및 Tricalcium Diphosphate 등 여러 물질

들로 구성되어 있다고 알려졌다. 그러나 화학적분석방법에 의한 담석의 성분분석은 많은

노력이 요구되며 여러 가지 어려움이 뒤따른다. Imfrared Spectrophotometric Analysis는

물질들의 정성분석에 비교적 광범위하게 이용된다.

본 논문은 Infrared Spectrophotometer를 이용하여 한국인 담석을 분석하였다. 담석자

료는 지난 7년 간 순천향의대병원 외과교실에서 채집된 약 300예의 담석 중에서 72예를

무작위 선택하였다. 담석의 분석은 Perkin-Elmer Model 221, Model 700 및 Beckman Model

18A를 사용하였다. 결과는 다음과 같다.

1. 72예 중 담낭에만 담석이 존재하는 경우는 43예(59.7%), 담낭 및 담도에 22예(30.6%

), 총수담관 및 간내담도에 7예(9.7%)이었다.

2. Infrared Spectrophometric analys에 의해 4가지 유형으로 분류할 수 있었다. Type

Ⅰ 은 Cholesterol 담석으로써 5예(6.9%), Type Ⅱ는 Bilirubin담석으로써 7예(9.7%), Ty

pe Ⅲ는 Billirubin 및 Cholesterol이 혼합된 담석으로써 21예(29.2%), Type Ⅳ는 Choles

terol, Bilirubin, Calcium Carbonate 등 여러 성분이 혼합된 형으로써 39예(59.1%)였다.

3. 담낭담성의 구성 성분 중 순수한 Cholesterol에 의한 것은 예상외로 적었고 담석의

위치에 상관없이 혼합형(Type Ⅲ 및 Type Ⅳ)이 83.5%로 대다수이었다.

4. Infrared Spectrophotometer에 의한 담석성분분석을 보면 담석을 육안적 소견에 의

해 분류한다는 것은 매우 부정확한 것으로 사료되었다.

Many workers studied the chemical composition of gallstones and the stones were

found to contain cholesterol, calcium salt of bilirubin, calcium carbonate, and

tricalcium diphosphate, as well as other substances.

Analysis of the stone by chemical means required a great deal of effort and was

attended with difficulty. Infrared spectrophotometric analysis of stones is now

being widely used for qualitative analysis and structural determination.

This investigation was performed to study a composition of the Korean gallstones

by means of infrared spectrophotometer. Random selection of gallstones were

collected from 72 patients among 300 cases for the past 7 years.

Perkin-Elmer Model 221, Model 700 or Beckman Model 18A infrared spectrophotometer

was used to analyse the gallstones.

The verified results are as follows.

1) Of the 72 cases, 43 cases (59.7%) were gallstones from gallbladder and 22

cases (30.6%) were from both gallbladder, CBD and intrahepatic duct.

2) Four different types of spectrophotometric patterns are observed and the type

Ⅰ is most similar to the standard spectrophotometric pattern of cholesterol (5

cases, 6.9%), the type Ⅱ is similar to bilirubin pattern (7 cases, 9.7%), the type

Ⅲ pattern showed both pattern of cholesterol and bilirubin (21 cases. 29.2%) and

the final type Ⅳ pattern showed mixed patterns(39 cases, 54.1%).

3) through the results of these infrared spectrophotometric analysis, most stones

are type Ⅲ and Ⅳ (83.5%) regardless of the location of gallstones.

4) These infrared spectrophotometric analysis have shown that judgement of

gallstone components from their appearances alone gave very erroneous idea.
