기계 환기보조를 받는 급성 호흡부전증 환자 예후의 조기 예측법

급성 호흡부전증은 부적절한 산소화나 환기 장애 또는 양자 모두가 초래되는 상태로써,

모든 연령층에서 발생하며 상하기도나 폐포같은 호흡기계뿐만 아니라 중추신경계, 심혈

관계 등 광범위한 신체 부위의 이상에 의해 유발될 수 있기 때문에 급성 호흡부전증의 원

인 질환은 매우 다양하다. 대부분의 호흡부전증은 기계환기보조를 포함한 집중 치료를 요

구하게 되며 사망을 또한 아직까지도 20-60%에 이르고 있다. 따라서 이러한 환자들이 기

왕의 치료에 대해 보일 반응, 그리고 생존이나 사망등의 궁극적인 결과를 조기에 예측하

는 것은, 적절한 시기에 이미 시행되고 있는 치료를 보완하거나 치료의 방향을 수정하는

데에 큰 도움을 줄 수 있다.

이에 저자는 1991년 5월부터 1992년 9월까지 연세대학교 의과대학 세브란스병원 중환자

실에서 급성 호흡부전증으로 최소한 7일 이상 기계환기보조중인 환자 140명을 대상으로

조사하였으며 기계환기보조를 시작하기 전의 상태가 심장내 단락이 없으면서 대기 호흡중

의 동맥혈 가스검사상 PaO^^2 가 50mmHg 미만인 경우만을 포함시켰다. 환자들이 중환자실

에서 기계환기보조를 시작한 날부터 처음 7일동안 매일 완충염기 편차(측정치-예측치)와

분당 환기량(분당 호흡수×상시 호흡량), 동적 탄성 [폐기량/(최고압-호기말양압)], 정적

탄성[폐기량/(고평부압-호기말양압)], 폐부전지수, 그리고 폐포-동맥산소분압차/흡입산

소분율(D(A-a)O^^2 /FiO^^2 ), 동맥산소분압/흡입산소분율(PaO^^2 /FiO^^2 ), 폐포산소분

압/흡입산소분율(PAO^^2 /FiO^^2), 동맥산소분압/폐포산소분압(PaO^^2 /PAO^^2 )등을 측

정하거나 계산하여 매일의 값을 각각 생존군과 사망군간에 비교하여 다음과 같은 결과를

얻었다.

1) 생존군과 사망군의 평균 연령은 50.9세와 53.4세로 양군간에 유의한 차이는 없었고

중환자실에 입원하였던 기간은 생존군이 사망군에 비하여 유의하게 길었다.

2) 완충염기의 편차와 분당환기량의 경우 양군간에 대체로 유의한 차이는 없었다.

3) 동적 탄성은 전반적으로 생존군에서 사망군에 비하여 높은 값들을 보였으나 역시 양

군간에 유의한 차이는 없었다.

4) 정적 탄성은 생존군이 사망군보다 전반적으로 높은 값을 보이면서 사망군에서는 3일

째부터 감소하는 양상을 보인 반면 생존군은 증가하는 양상을 보였으며 4일째부터 양군간

에 유의한 차이를 보였다.

5) 폐부전지수와 D(A-a)O^^2 /FiO^^2 , PAO^^2 /FiO^^2 등은 중환자실에 입원한 첫 7

일 동안 사망군에서 유의하게 높으면서 생존군에서는 점진적으로 감소하는 반면 사망군에

서는 조금씩 증가하거나 거의 변화가 없었다.

6) PaO^^2 /FiO^^2 , PaO^^2 /PAO^^2 는 첫 7일동안 생존군에서 유의하게 높으면서 지

속적으로 증가하는 양상을 보였던 반면 사망군에서는 거의 변화가 없거나 약간 감소하는

추세를 보였다.

이상의 결과에서 정적 탄성과 폐부전지수, 그리고 D(A-a)O^^2 /FiO^^2 , PAO^^2 /FiO^^

2 , PaO^^2 /PAO^^2 등의 척도들은 생존군과 사망군간에 유의한 차이를 보였음은 물론 생

존군이나 사망군에 따라 증가, 감소 혹은 거의 변화가 없는 등의 특징적인 변화 양상도

관찰되었다. 따라서 이들 척도들은 급성 호흡부전증으로 기계환기보조를 받는 환자들에서

예후를 조기에 예측하는 데에 도움을 줄 것으로 생각된다.

Prediction of Prognosis in Patients Mechanically Ventilated due to Acute

Respiratory Failure

Acute respiratory failure is a condition in which an illness or impairment of the

respiratory system results in inadequate oxygenation or ventilation, or both. It

occurs in all age-groups. The causes for acute respiratory failure are various, and

even though patients are managed intensively including mechanical ventilation, the

mortality rate has been 20-60% even until today. Therefore, early prediction of

outcome in such critically ill patients should be considered a high priority so

that corrective measures or alternative support methods may be rapidly instituted.

The patients who were treated with a mechanical ventilator longer than 7 days due

to acute respiratory failure in intensive care unit were listed, and only patients

whose initial PaO^^2 was less than 50 mmHg during breathing the ambient air in the

absence of an intracardiac shunt were included. From the daily collected data, the

following variables were calculated: buffer base deviation (measured predicted),

minute ventilation(tidal volume×respiratory rate), dynamic complance [tidal

volume/(peak pressure-PEEP)], static compliance [tidal volume/(plateau

pressure-PEEP)], pulmonary insufficiency index, indices for physiologic shunt or

gas wxchange such as PaO^^2 /FiO^^2 , PaO^^2 /PAO^^2 , D(A-a)O^^2 /FiO^^2 and

PAO^^2 /FiO^^2 . These variables between the survivors and nonsurvivors were

compared daily, The following result were obtained:

1) There was no significant differences in age and ICU care was longer in the

survivors than the nonsurvivors.

2) There was no significant difference in buffer base deviation and minute

ventilation between the two groups.

3) There was no significant difference in dynamic compliance between the two

groups.

4) Static compliance showed higher values in the survivors and from the 4th day

after mechanical ventilation, there was a significant difference between the two

groups.

5) Pulmonary insufficiency index, the ratio of D(A-a)O^^2 /FiO^^2 and PAO^^2

/FiO^^2 showed significantly higher values in the nonsurvivors throughout the first

seven days. There was a tendency of gradual decrease in the survivors but almost no

change in the nonsurvivors.

6) The ratio of PaO^^2 /FiO^^2 and PaO^^2 /PAO^^2 showed significantly higher

values in the survivors throughout the first seven days. There was a tendency of

gradual increase in the survivors. but almost no change in the nonsurvivors.

Static compliance, pulmonary insufficiency index, D(A-a)O^^2 /FiO^^2 , PAO^^2

/FiO^^2 ,PaO^^2 /FiO^^2 and PaO^^2 /PAO^^2 showed a significant difference between

the survivors and nonsurvivors, and each value also showed characteristic changing

patterns throughout the first 7 days after mechanical ventilation. With careful

observation of each value and the changing patterns during the first 7 days after

mechanical ventilation, these parameters were thought to be helpful in the early

prediction of outcome in mechanically ventilated patients due to acute respiratory

failure.
