한국인의 무지장외전근과 무지단신전근종지의 변이에 대한 연구

연세대학교 의과대학 해부학교실에서 수집한 46예의 수근(carpus ,wrist)에서 무지장외

전근(m. abductor pollicis longus)및 무지단신전근(m. extensor pollicis brevis)종지의

변이에 대하여 연구하였다. 무지장외전근은 보조건의 유무와 그 보조건이 종지하는 해부

학적 위치의 변이에 관하여, 무지단신전근은 종지하는 해부학적 위치, 주행 및 무지장신

전근과의 관계의 변이에 대하여 다음과 같은 결과를 얻었다.

1. 무지장외전근이 하나의 건으로 제1중수골 기저부에 종지하는 것은 1예(2.2%)뿐이었

으며, 무지장외전근 종지부위에서 하나이상의 보조건이 있는 경우는 45예(97.7%)였다.

2. 무지장외전근에서 하나이상의 보조건이 있는 모든 예중에서 능형골로 종지하는 경우

가 33예(71.7%), 무지구근 및 근막으로 종지하는 경우가 38예(82.6%)였다.

3. 무지단신전근은 기절골(proximal phalanx)에만 종지하는 것이 30예(65%)였고 무지장

신전근과 함께 모두 말절골(distal phalanx)에만 종지하는 경우가 8예(18%)였으며, 양측

에 종지하는 경우가 8예(18%)였다.

4. 무지장외전근의 보조건의 수 및 해부학적위치에 있어서, 동일개체에서 좌우대칭인

경우는 16구(88%), 비대칭인 경우는 6구(12%)였다. 무지단신전근은 종지하는 해부학적위

치와 주행 및 무지장신전근과의 관계에서 좌우대칭인 경우는 21구(95%), 비대칭인 경우는

1구(5%)였다.

Study on the Variations of Insertions of Abductor Pollicis Longus and Extensor

Pollicis Brevis In Korean

The author studied 46 cases of the wrists which were collected at the Department

of Anatomy of Yonsei University, College of Medicine concerning with the variations

of insertions of abductor pollicis longus and extensor pollicis brevis muscles. The

author studied the abductor pollicis longus on the relationship between the

presence of accessory tendon and the anatomical location of insertion and also

studied the insertion of extensor pollicis brevis on the variation of anatomical

location, course.

The results were analyzed and evaluated as followings.

1. Only one case (2.2%) of 46 cases of abductor pollicis longus inserted entirely

to the base of first metacarpal bone on its anterolateral surface as a single

tendon and one or more accessory tendons were present in 45 cases (97.7%).

2. 33 cases (71.7%) of abductor pollicis longus tendons inserted into the

trapezium, and 38 (82.6%) inserted into the thenar muscles.

3. 30 cases (65%) of 46 extensor pollcis brevis tendons inserted into the

proximal phalanx only and 8 (18%) inserted entirely on the distal phalanx with

extensor pollicis longus, and 8(18%) had both insertions.

4. Abductor pollicis longue showed main difference between right and left in 6

cadavers (12%) and extensor pollicis brevis showed it in 1 cadaver (4.5%).
