Alcohol consumption and risk of death from all causes, coronary heart disease, stroke and cancer in patients with diabetes mellitus

Abstract

Background and Purpose The aim of this study was to examine the association between alcohol consumption, especially heavy drinking, and risks of mortality due to all causes, coronary heart disease, stroke and cancer in patients with diabetes mellitus in a prospective cohort study.Methods This study followed a cohort of 1,329,525 Korean( 56,374 with diagnosed diabetes mellitus), 30 to 95 years of age, who received health insurance from the National Health Insurance Corporation and had a medical evaluation from 1992 to 1995. They were free of cardiovascular disease, cancer, or liver disease at baseline. Results During the 14 years of follow-up, 11,927 deaths due to all causes were documented among men with diabetes. The relative risks(95% CI) of mortality due to all causes estimated for those reporting 0, 1-24.9g, 25-49.9g, 50-99.9g and 100g or more of daily alcohol intake were 1.43(1.10-1.19), 1.00 (referent), 0.99(0.92-1.05), 1.09(1.00-1.18), and 1.34(1.19-1.52) after adjustment for age, smoking status, exercise status, total cholesterol, body mass index, systolic blood pressure and fasting glucose. Heavy drinkers of men of 100g or more of daily alcohol intake had increased risks of mortality from Coronary heart disease (adjusted hazard ratio(HR)=1.09; 95% CI: 0.56-1.43), total stroke (HR=1.77; 95% CI: 1.19-2.64), hemorrhagic stroke (HR=2.32; 95% CI:1.34-4.03) and cancer(HR=1.30; 95% CI: 1.00-1.70). Female drinkers showed an reduced risk of mortality from all-causes as compared with female nondrinkers, but the outcome was not statistically significant.Conclusion In Korea, heavy alcohol consumption was associated with increased risk of mortality from all causes, total stroke, in particular, hemorrhagic stroke, and cancer among men with diabetes.

연구배경 및 목적

여러 역학적 연구들을 통해 일반인구 뿐만아니라 당뇨환자에서도 소량의

음주는 모든 원인의 사망위험을 감소시키는 것으로 알려져 있다. 그러나

기존 연구들의 제한점으로 인하여 전향적인 자료를 통해 당뇨병 환자에서

알콜섭취량, 특히 고위험음주와 모든 원인 및 관상동맥질환, 뇌졸중, 암의

사망위험과의 관계를 알아보고자 한다.

연구 방법

1992년부터 1995년까지 국민건강보험공단에서 시행하는 검진을 받은 30

세에서 95세의 수진자 1,329,525명중에서 심혈관질환, 암, 간질환을 없었던

대상자중 이전에 당뇨를 진단받거나 시행한 검진상 공복혈당이 126mg/dl

이상으로 당뇨를 진단할수 있는 56,374명을 대상으로 하였다. 검진시 시행

한 설문지를 기초로 알콜섭취량을 조사하였고, 통계청 자료를 바탕으로

사망수를 측정하였으며 사망시 등록하는 진단명을 기준으로 사망원인으로

관상동맥질환, 뇌졸중, 암을 측정하였다.

연구결과

14년동안 추적연구한 결과, 총 16,687명의 사망이 보고되었으며, 그중

11927명이 남자, 4760명이 여자였다. 남자인 당뇨병환자에서 모든 원인에

의한 사망위험도( 95% 신뢰구간)는 나이, 흡연상태, 운동여부, 혈중 총콜레

스테롤, 체질량지수, 수축기혈압, 공복혈당을 보정하였을 때, 평균 하루 0,

1-24.9g, 25-49.9g, 50-99.9g 100g 이상 으로 알콜섭취량을 분류시 각각

1.43(1.10-1.19), 1.00 (참고치), 0.99(0.92-1.05), 1.09(1.00-1.18),

1.34(1.19-1.52) 였다. 평균 하루 100g 이상 알콜섭취량을 가진 고위험음주

군에서는 모든 뇌졸중(사망위험도 1.77 , 95%신뢰구간, 1.19-2.64), 출혈

성 뇌졸중( 2.32, 1.34-4.03), 암(1.30I, 1.00- 1.70)에 의한 사망이 모두 증가

하였다. 여성 음주자에서는 여성 비음주자와 비교할 때, 모든 원인에 의한

사망위험도는 감소하였으나, 통계적으로 유의미하지는 않았다.

결론

당뇨병을 가진 남성에서 고위험음주는 모든 원인 및 뇌졸중 특히 출혈성

뇌졸중, 암에 의한 사망위험도를 높힌다.
