소아암 환아 가족의 회복력 모형 검증 연구

dc.description.abstract[한글] 본 연구의 목적은 McCubbin의 회복력 모형 중 회복력 요인과 적응과의 관계를 검증하여 소아암 환아 가족의 적응에 영향을 미치는 회복력 요인을 규명하고 이들 요인간의 인과적 관계 및 효과를 파악함으로써 가족의 적응을 증진하기 위한 간호중재의 이론적 기틀로서 회복력 모형의 적용가능성을 확인하고자 함이다. 가설적 모형에서 가족의 적응을 설명하는 예측변수는 사회적지지, 가족관, 가족강인성, 가족기능, 문제해결대처, 문제해결의사소통으로 이 중 사회적지지를 외생변수로, 나머지를 내생변수로 하여 18개의 가설적 경로를 구성하였다. 가설적 모형을 실증적으로 검증하기 위해서 2003년 3월 14일부터 5월 31일까지 서울과 경기 지역에 소재한 4개 대학병원에서 자료수집을 실시하였고, 자료수집 방법은 설문지 면접조사를 통해 총 232개의 자료가 분석되었다. 연구도구는 사회적지지 척도, 가족관 척도, 가족강인성 척도, 가족APGAR 질문지, 가족문제해결대처 척도, 가족문제해결의사소통 척도, 가족원의 안녕척도를 사용하였다. 대상자의 일반적 특성과 연구변수에 대한 기술적 통계 및 변수간의 상관관계 분석은 SAS Program을 이용하였고 모형의 부합도 검증과 모수추정을 통한 가설검증은 PC-LISREL Version 8.12 프로그램으로 분석하였다. 본 연구에서 나타난 결과는 다음과 같다. 1. 모형의 부합도 검증/가설적 모형은 전반적 부합지수에서 대체로 잘 부합되는 것으로 나타났으나 표준화 잔차가 기준치를 초과하여(-2.89~3.94) 모형의 수정이 필요함을 보여주었다. 모형의 부합도를 높이기 위해 통계적 유의성과 이론적 타당성을 고려하여 1개의 경로를 제거한 수정모형을 확정하였다. 수정모형의 부합도는 χ2통계량/자유도(df)=2.4, 기초부합치(GFI)=.94, 원소간 평균차이(RMR)=.06, 표준부합치(NFI)=.93, 비표준부합치(NNFI)=.94, 표준화잔차 -2.38~3.30로서 양호한 것으로 나타났다. 2. 가설검증 결과 1) 소아암 환아 가족의 적응은 가족강인성 (β63=.97, T=2.30)과 문제해결대 ( β64=.38, T=2.95)에 의해 유의하게 직접적인 영향을 받았다. 2) 사회적지지는 적응에 직접적인 영향은 없었으나(γ61=.05, T=.40) 간접효과 (효과계수=.20, T=1.89)의 영향으로 총효과 (효과계수=.25, T=3.35)가 유의하였다. 3) 사회적지지는 간접효과와 직접효과를 포함한 총효과면에서 가족관, 가족강인성, 가족기능, 문제해결대처, 문제해결의사소통, 가족적응의 모든 내생변수에 유의한 영향을 미치는 변수로 나타났다. 4) 가족관은 가족강인성 (β31=.14, T=3/31)과 문제해결대처 (β41=.39, T=6.22)에 유의한 직접적인 영향을 주는 변수로 나타났다. 5) 가족강인성은 가족기능(β23=.65, T=2.62), 문제해결대처 (β43=.45, T=2.72), 가족적응(β63=.97, T=2.30)에 가장 큰 영향을 미치는 변수로 나타났다. 6) 문제해결의사소통은 가족강인성(β35=.36, T=2.97)에 직접적인 영향, 가족기능(효과계수=.27, T=2.48)과 문제해결대처(효과계수=.20, T=2.53)에는 간접적인 영향을 주는 변수로 나타났다. 결론적으로 회복력 요인 중 가족적응에 가장 큰 효과를 갖는 변수는 강인성과 사회적지지, 문제해결대처였다. 이 중 가장 큰 직접적인 영향을 주는 변수는 가족강인성이있고 그 다음이 문제해결대처였다. 사회적지지는 직접적 경로없이 간접적으로만 가족적응에 영향을 미치는 것으로 나타났다. 가족관과 문제해결의사소통은 가족적응에 가장 큰 영향을 미치는 가족강인성과 문제해결대처를 예측하는 변수로서 가족적응을 촉진하는 프로그램 개발 시 선행변인으로 중요하게 다루어야 할 변수임이 확인되었다. 따라서 소아암 환아 가족의 적응을 증진시키기 위해서는 이런 회복력 요인을 조기 사정하여 회복력 요인에 대한 간호중재를 개발하고 적극적인 실천방안이 강구되어야 할 것이다. [영문]The purpose of this study was to identify resiliency factor, to test causal relationship and effect of resiliency factors that influence on adatation of families of children with cancer, and to identify applicability of McCubbin''s resiliency model as a theoretical framework of nursing intervention. The hypothetical Model was constructed that resiliency variables would have a direct effect and mediate the effect on adaptation of family of children with cancer. Predictive variables included in this model are social support, family schema, family hardiness, family functioning, problem-solving coping, problem-solving communication, family adaptation. Empirical data for testing the hypothetical model were collected using self-report questionnaire at 4 general hospital in Seoul and Kyungi Province between March and May, 2003 from 232 families of children with cancer. The collected data were analyzed by SAS program for descriptive and correlational analyses and LISREL program for covariance structural analysis. The results were summarized as follows. 1. In the test of hypothetical model to the empirical data, the overall fitness indices were shown to be good. But when we look at other type of measure like Normalized Residual(-2.89 ~ 3.94) and Stem-leaf Plot, the hypothetical model needed modification. Modification model was constructed by deleting one path in accordance with the statistical and theoretical criteria. Compared to the hypothetical model, the revised one had better fit to the data(chi-square df=2.4, GFI=.92, RMR=.06, NFI=.93). 2. The results of testing hypothesis were followed : 1) Family hardiness(β63=.97, T=2.30) and problem-solving coping(β64=.38, T=2.95) had significant direct effects on family adaptation of children with cancer. 2) Social support had significant indirect effect(effect coefficient=.20, T=1.89) and total effect(effect coefficient=.25, T=3.35) on adaptation in families of children with cancer. Social support had significant effect on all endogenous variables : family schema, family hardiness, family functioning, problem-solving coping, problem-solving communication including direct, indirect and total effect. 3) Family Schema had significant direct effect on family hardiness(β31=.14, T=3.31) and on problem-solving coping(β41=.39, T=6.22) 4) Family hardiness had significant direct effect on family functioning(β23=.65, T=2.62), problem-solving coping(β43=.45, T=2.72) and adaptation in families of children with cancer(β63=.97, T=2.30). 5) Problem-solving communication had significant direct effect on family hardiness(β35=.36, T=2.97) and significant indirect effect on family functioning(effect coefficient=.27, T=2.48) and on problem-solving coping(effect coefficient=.20 T=2.53). Among the resiliency factors, family hardiness had the greatest effect on adaptation of family of children with cancer. In conclusion, this study supports the Resiliency Model by McCubbin and resiliency factors were identified. The finding of this study suggest that nursing intervention to enhance family hardiness, problem-solving coping and social support would result in an increase in adaptation of families of children with cancer. The multi-dimensional integrated intervention strategy and program that emphasize and promote these resiliency factors should be developed and established for families of children with cancer.-
