간경변증 환자에 있어서 문맥압 항진증은 정맥류 출혈과 같은 중요 합병증과 사망의 주원인이므로 이의 치료가 필요하다. 따라서 문맥압의 측정을 통해 항진증 정도를 평가하는 것은 문맥압 항진증 치료의 기본적 요소이다. 현재 문맥압 항진증을 측정하는데 가장 널리 이용되는 방법은 간정맥 압력차(hepatic venous pressure gradient, HVPG)의 측정이나, 침습적 방법으로 실제 임상에서 이용되는데는 많은 제한이 있다. 최근 비침습적인 도플러 초음파를 이용하여 여러 혈역학적 인자들을 측정함으로써 문맥압을 평가하고자 하는 연구들이 있어 왔으나, 아직까지 그 유용성이 입증되진 못했다. 도플러 초음파 검사에서의 간정맥 파형은 심장의 박동 주기에 따라 정상적으로 삼위상(triphasic) 파형을 보인다. 그러나 명확히 밝혀진 바는 없지만, 간경변증 환자들에 있어서는 그 파형이 이위상(biphasic) 또는 단위상(monophasic) 파형으로 변화됨이 관찰된다. 이에 본 연구는 간경변증 환자들에서 도플러 초음파 검사의 간정맥 파형과 문맥압을 반영하는 간정맥 압력차간의 상관 관계를 알아봄으로써 비침습적인 방법인 도플러 간정맥 파형 검사가 문맥압 항진증의 심화도를 반영할 수 있는지 알아보고자 하였다. 또한 문맥압 강하제인 terlipressin 투여 전후의 간정맥 파형과 간정맥 압력차의 변화를 살펴봄으로써, 간정맥 파형 측정이 문맥압 항진증을 개선하는 약물의 치료 반응을 평가하는데 유용한 지 알아보고자 하였다. 대상은 간경변증에 의한 문맥압 항진증으로 식도 정맥류 출혈을 보인 72명의 환자로 하였다. 도플러 초음파 검사를 이용하여 관찰한 간정맥의 파형은 다음과 같이 5가지로 분류하였는데 I형, 삼위상 파형; II형, 역류 없는 이위상 파형; III형, 진폭이 감소된 이위상 파형; IV형, 조동이 있는 단위상 파형; V형, 완전 단위상 파형이다. 그리고 간정맥 파형을 임상적으로 적용할 때 그 편리성을 위해 삼위상(I형), 이위상(II-III형), 단위상(IV-V형)의 3단계로도 분류하였다. 모든 환자에서 직접 간정맥 압력차를 측정하여 경증(HVPG≤15mmHg) 및 중증(HVPG>15mmHg) 문맥압 항진증으로 분류하였다. 이러한 분류를 통해 도플러 간정맥 파형과 간정맥 압력차 간의 상관관계를 분석하였다. 또한, 대상 환자 중 21명의 환자에 대해서는 문맥압 강하제인 terlipressin을 투여하여, 투여 전, 후에 간정맥 파형과 간정맥 압력차 각각의 변화를 관찰하고, 이들간의 상관관계를 알아보았다. 그 결과, 간정맥 파형과 간정맥 압력차 간에 유의한 상관성이 있어(p<0.01), IV형과 V형의 단위상 파형들은 정상적 삼위상 파형(I형)에 비해 중증의 문맥압 항진증의 위험도가 각각 22배와 56배 높게 나타났고, 단위상 파형 전체로 보면 28배 높게 나타났다.(odds ratio: IV형=22.369, V형=55.885, IV-V형=27.835) 도플러 초음파로 간정맥 파형이 단위상 파형일 경우에 중증의 문맥압 항진증일 민감도는 63.9%, 특이도는 91.7% 였다. 이위상 파형은 경증 문맥압 항진증에 대해 77.8%의 민감도와 66.7%의 특이도를 나타냈다. Terlipressin 투여에 따라 간정맥 압력차의 감소와 함께 간정맥 파형 또한 변화되는 소견을 나타내어, 간정맥 파형이 문맥압의 변화 또한 반영하였다. 결론적으로, 문맥압 항진증 환자에 있어서 도플러 초음파를 이용한 간정맥 파형의 관찰은 비침습적으로 간편하게 문맥압 항진증을 평가할 수 있는 검사법이며, 또한 문맥압 항진증 치료에 대한 반응을 평가에 있어서도 유용한 방법이 될 수 있을 것으로 판단된다.



[영문]Portal hypertension is most important cause of complications and mortality in liver cirrhosis and its control is main target of treatment. So, assessment of portal hypertension is important but, hepatic venous pressure gradient(HVPG), which has been accepted as the gold standard for assessing the severity of portal hypertension, is limited in its clinical application because of its invasiveness. In liver cirrhosis, the Doppler hepatic vein waveform changes to bi or monophasic waveform but, the causes of this are not clear yet. This prospective study aimed to determine the usefulness and possibility of non-invasive Doppler hepatic vein waveform in assessing severity and therapeutic response of portal hypertension by evaluation of relationship with hepatic venous pressure gradient(HVPG) and their response to terlipressin, which reduces the portal pressure in liver cirrhosis. The HVPG and the Doppler ultrasonographic hepatic vein waveform type were measured in 72 patients with liver cirrhosis and the relationship between them were analyzed. The Doppler hepatic vein waveform were classified into five types; type I, triphasic waveform; type II, biphasic waveform without reversed flow; type III, decreased amplitude of phasic oscillations; type IV, flat waveform with fluttering;V, completely flat waveform with fluttering and also classified into one of three types: type I, triphasic waveform; type II-III, biphasic waveform; type IV-V, monophasic waveform, because the five class classification has risk of relative subjectiveness and is troublesome in clinical use. The HVPG results were divided into two classes according to the available evidence of differences in severity.(less severe portal hypertension: HVPG≤15mmHg; severe portal hypertension: HVPG>15mmHg). Also, the changes in HVPG and hepatic vein waveform type after administering terlipressin and its relationship were evaluated in 21 of the 72 patients. With an increase in HVPG, Doppler hepatic vein waveforms tend to have monophasic waveform (type IV-V) significantly(P<0.01) Monophasic waveform was significantly correlated with severe portal hypertension compared to normal triphasic waveform(type I)(odds ratio=27.84). The sensitivity and specificity of monophasic waveform for severe portal hypertension was 63.9% and 91.7%, respectively. Also, the sensitivity which biphasic waveform indicates less severe portal hypertension(HVPG≤15mmHg) was 77.8% and its specificity was 66.7% in ROC curve analysis. After administering of terlipressin, both HVPG and Doppler hepatic vein waveform improved in all patients who evaluated. In conclusion, the Doppler hepatic vein waveform is non-invasive and useful in assessing the severity and therapeutic response of portal hypertension in liver cirrhosis patients.
