한국인 허리신경얼기의 구성과 그 가지의 국소해부학

허리신경얼기는 대부분 교과서에 L1-L4의 앞가지가 이루며, T12도 일부가 참여한다고 하였다. 많은 연구에서는 허리신경얼기는 T12-L5 사이에서 다양하게 형성되고, 그 가지를 이루는 척수신경의 구성도 다양하게 보고되었다. 그리고 허리신경얼기에서 일어나는 신경을 이루는 척수신경의 구성에 대한 형태계측에 관한 연구는 찾아 볼 수가 없었다. 허리신경얼기와 큰허리근 그리고 샅고랑인대 부위에서 각 신경의 국소해부학적 관계는 부위마취 때 임상적으로 중요하다. 이 연구는 허리신경얼기와 그 가지를 이루는 척수신경의 형성변이를 조사하고, 각 신경을 이루는 척수신경의 구성에 대한 형태를 계측하고, 부위마취시 도움이 되는 자료를 얻기 위해 시도하였다.

재료로는 한국성인시신 86구(172쪽)를 사용하였다. 그 중 15구(30쪽)에서는 허리신경얼기와 그 가지를 한 덩어리로 떼어내어 0.04M guanidine 용액에 담근 후, 초음파세척기로 처리하여 결합조직을 부드럽게 한 후, 수술현미경 아래에서 신경다발을 분리하였다. 긴지름과 짧은지름을 계측하여 그 평균값을 사용하였다.

허리신경얼기의 뿌리는 T12-L4가 33.7%, L1-L4가 56.6%, L1-L5가 9.6%였다. 엉덩아랫배신경은 T12에서 25.3%, L1에서 67.5%가 일어났다. 엉덩샅굴신경은 L1에서 83.3%, L2에서 7.1%가 일어났다. 음부넙다리신경은 7가지 유형이 있었는데, 그 중 L1에서 14.1%, L1-L2에서 28.2%, L2에서 42.3%가 일어났다. 가쪽넙다리피부신경은 8가지 유형이 있었고, L2에서 35.9%, L2-L3에서 29.3%가 일어났다. 넙다리신경은 7가지 유형이었는데, L1-L4에서 24.2%, L2-L4에서 65.7%가 일어났다. 폐쇄신경은 8가지 유형이었고, L2-L4에서 40.0%, L3-L4에서 40.0%가 일어났다. 엉덩아랫배신경을 이루는 척수신경의 굵기는 T12가 이루는 경우 2.38mm, L1이 이루는 경우 1.18mm였다. 엉덩샅굴신경은 L1이 이루는 경우 0.61mm, L2가 이루는 경우 0.40mm였다. 음부넙다리신경은 L1이 이루는 경우 0.56mm, L1-L2가 이루는 경우 L1은 0.57mm, L2는 1.02mm였고, L2가 이루는 경우 0.60mm, L2-L3가 이루는 경우 L2는 0.38mm, L3는 0.28mm, 그리고 L3가 이루는 경우 0.98mm였다. 가쪽넙다리피부신경은 L2가 이루는 경우 0.73mm, L2-L3가 이루는 경우 L2는 0.77mm, L3는 0.73mm였다. 넙다리신경은 L2-L4가 이루는 경우 L2는 1.48mm, L3는 2.91mm, L4는 2.19mm, L2-L5가 이루는 경우 L2는 0.48mm, L3는 2.47mm, L4는 2.87mm, L5는 0.51mm였고, L3-L5가 이루는 경우 L3는 2.11mm, L4는 3.30mm, L5는 1.00mm였다. 폐쇄신경은 L2-L3가 이루는 경우 L2는 0.56mm, L3는 1.79mm였고, L2-L4가 이루는 경우 L2는 0.53mm, L3는 1.45mm, L4는 0.64mm였으며, L3-L4가 이루는 경우 L3는 1.31mm, L4는 1.09mm, 그리고 L3-L5가 이루는 경우는 L3는 0.91mm, L4는 1.38mm, L5는 0.30mm였다. 허리신경얼기의 각 가지 중 음부넙다리신경과 폐쇄신경을 제외한 모든 신경들은 큰허리근의 가쪽모서리를 빠져나왔고, 폐쇄신경은 큰허리근의 안쪽모서리를 빠져나와 다리로 내려갔다. 음부넙다리신경은 1-3개의 가지가 큰허리근을 뚫고 나왔다. 샅고랑인대의 길이는 119.9mm였다. 위앞엉덩뼈가시에서 가쪽넙다리피부신경과 넙다리신경까지의 거리는 각각 17.6mm, 54.0mm, 두덩뼈결절에서 음부넙다리신경 중 음부가지와 넙다리가지까지의 거리는 각각 66.9mm, 68.6mm, 두덩뼈결절에서 폐쇄신경까지의 거리는 27.6mm였다.

이상의 연구 결과는 허리신경얼기와 이것에서 일어난 신경들의 형태변이를 관찰하여 한국인의 특징을 밝혔고, 그 결과를 다른 연구자들의 결과와 비교하였으며, 각 신경을 이루는 척수신경을 계측하여 그 구성비율을 밝혔다. 그리고 샅고랑인대 부위에서 각 신경이 지나가는 위치를 계측하여 부위마취와 관련된 임상적 의의에 대하여 고찰하였다.

[영문]The lumbar plexus and its nerves have been described to be formed diversely by the spinal nerves from T12 to L5. However the detailed anatomy of the nerves and the topographic relationship with the surrounding structures have not been clarified. Accordingly, this study was performed to clarify the diverse constituent of the lumbar plexus, the detailed anatomy of the nerves from the lumbar plexus, and its topographic relationship with the medial and lateral border of the psoas major muscle, the anterior superior iliac spine, and the pubic tubercle.

One hundred-seventy two sides of 86 Korean adult cadavers were used in this study. The lumbar plexus and its nerves were dissected under the surgical microscope, after being separated from the body.

The lumbar plexus was formed by the spinal nerves of the T12-L4 in 33.7%, the L1-L4 in 56.6%, and the L1-L5 in 9.6%. The iliohypogastric nerve mostly arised from T12(25.3%, thickness 2.38mm) or L1(67.5%, thickness 1.18mm). The ilioinguinal nerve mainly arised from L1(83.3%, thickness 0.61mm) or L2(7.1%, thickness 0.40mm). The formation of the genitofemoral nerve could be classified into 7 types, and the type in which L2 comprised the nerve, was the most common(42.3%, thickness 0.60mm). The lateral femoral cutaneous nerve could be classified into 8 types, and the most common type was L2(35.9%, thickness 0.73mm). The femoral nerve could be classified into 7 types, and the type in which L2-L4 comprised the nerve, was the most common(65.7%, thickness L2 1.48mm, L3 2.91mm, L4 2.19mm). The obturator nerve could be classifed into 8 types, and the common type was L2-L4(40.0%, thickness L2 0.53mm, L3 1.45mm, L4 0.64mm) or L3-L4(40.0%, thickness L3 1.31mm, L4 1.09mm). While the obturator nerve and the other nerves emerged from the medial and lateral border of the psoas major muscle, respectively, the genitofemoral nerve pierced the muscle by 1-3 branches. The mean distance from the anterior superior iliac spine to the lateral femoral cutaneous and the femoral nerve was 17.6mm, and 54.0mm, respectively. The mean distance from the pubic tubercle to the genital and femoral branch of the genitofemoral nerve was 66.9mm, and 68.6mm, respectively. The mean distance from the pubic tubercle to the obturator nerve was 27.6mm.
