피질골에 인위적 조작을 가함으로써 치아 이동을 빠르게 하여 교정 기간을 줄이고자 하는 방법 중에 피질골 활성화 (cortical activation) 술식이 치아 이동과 주위 골 조직에 미치는 영향에 대해 알아보고자, 건강한 성견 (beagle dog)을 이용하여, 먼저 상하악 좌우측의 제 2 소구치를 각각 발치 한 후, 치아이동에 대한 골 치유 과정과 발치에 의한 영향을 최소화 하고자, 4주의 발치와 치유기간이 경과 하도록 기다린 후, 상 하악 우측을 실험군으로, 좌측을 대조군으로 하여, 실험군의 협 설측에 점막 골막 피판을 거상 하고, #2 round bur 를 이용하여 Wilcko 가 추천한대로 3mm 간격으로 hole 에 의한 피질골 활성화를 피질골을 관통시키지 않을 정도로 시행하고, 실험군과 대조군 모두 상악에는 NiTi coil spring 에 의한 150g 정도의 교정력을 가해 제 3 소구치를 전방 견인 하였고, 하악에는 017 X 025 S.S. wire 를 guiding wire로 하여, 이동 하고자 하는 제 3 소구치에 0.022” slot bracket 을 장착 한 후, 역시 NiTi coil spring 을 이용 하여 150g 의 교정력을 가하여 제 3 소구치를 전방 견인 하여, 1주일 단위로 vernier caliper 를 이용하여 각각 치아의 이동 량을 8주간 측정 하여, 피질골 활성화를 시행하고 치아 이동을 시도한 부위와 단순히 치아 이동만을 시도한 부위를 비교하였다.

피질골 활성화 후 나타나는 치아주위 조직의 조직학적 미세 구조 관찰을 위하여, 제 2소구치 발치 만 시행한 경우와, 피질골 활성화를 시행한 후 각각 2주, 4주 동안 교정력을 가해 치아이동을 시킨 경우, 그리고 8주간 교정력을 가해 치아이동을 시키고 6개월 경과 후 다시 교정력을 가한 경우를, LM semi thin section과 EM을 통하여 비교 관찰한 결과 다음과 같은 결론을 얻었다.



1. 피질골 활성화를 시행 후 교정력을 가한 실험군에서 대조군에 비해 교정력을 가한 8주 기간 동안 빠른 치아 이동이 일어났다.



2. 피질골 활성화를 시행 후 각각 2주와 4주 동안 교정력을 가한 경우에, 발치만 시행 한 경우보다 증가된 세포 활성도가 관찰 되었다.



3. 피질골 활성화를 시행 후 교정력을 가하고 6개월이 경과 한 후 다시 교정력을 가한 경우, 발치만 시행 한 경우보다 오히려 낮은 세포 활성도를 보였다.



본 연구 결과를 종합하면, 피질골 활성화 후 교정력을 적용하면 초기에 빠른 치아 이동을 보이지만, 피질골 활성화를 시행하고 일정 시간이 경과 한 후 다시 교정력을 가해 치아 이동을 시도할 때, 오히려 주위 골 조직의 세포 활성도가, 발치만 시행 한 경우 보다 떨어지는 점이 관찰 되었다.

빠른 치아 이동을 위해 피질골에 어떠한 형태의 술식을 가할 경우, 초기 변화 뿐 아니라, 일정 시간이 경과 한 후 주위 골 조직의 변화와 세포 반응, 그리고 이러한 변화를 최소화 하기 위한 더 많은 연구가 필요하다고 사료된다.

[영문]

Of the procedures to fasten tooth movement by intentional manipulation in cortical bone, cortical activation procedure, used in this study assessed the effects of cortical activation on orthodontic tooth movement and surrounding bony tissue. In adult beagle dogs, we first extracted both upper and lower four second bicuspid, and waited another 4 weeks to minimize the influence of extraction and bone healing on tooth movement. On the right side of upper and lower jaw as in the experimental group, we performed cortical activation procedure with #2 round bur after retracting mucoperiosteal flap. We were careful not to perforate the cortical plate as recommended by Wilcko. And then, we applied 150g of simple tipping force by NiTi coil spring to move the third bicuspids forward on upper jaw in both experimental and control group. On lower jaw, with a .019 X .025 S.S. guiding wire and .022 bracket, the same 150g of force by NiTi coil spring was also applied. With a digital vernier caliper, we measured the amount of tooth movement every week for 8 weeks and compared the differences of tooth movement between the experimental and the control group.

For the observation of histologic micro structural changes of surrounding periodontal tissue and bone after cortical activation, we extracted second bicuspids and waited four weeks to minimize the influence of extraction and bone healing on tooth movement. We performed the same cortical activation procedure and applied 150g of simple tipping force by NiTi coil spring to move the third bicuspids forward .

We compared 4 cases respectively. The first one underwent tooth extraction only without any experimental procedure and was sacrificed 6 weeks later after extraction. The second and the third one underwent two weeks and four weeks of tooth movement after cortical activation respectively. The fourth animal underwent 2 weeks of tooth movement after 8 weeks of experimental procedure followed by 6 months of observation.



The results are as follows:



1. The experimental group that was applied with orthodontic force after cortical activation showed more rapid tooth movement during 8 weeks of periods compared to control group.

2. The cases those applied with orthodontic force for 2 and 4 weeks respectively after cortical activation showed higher cellular activities compared to the case that underwent tooth extraction only without any experimental procedure.

3. The case that underwent 2 weeks of tooth movement after 8 weeks of experimental procedure followed by 6 months of observation showed lower cellular activities compared to the case that underwent tooth extraction only without any experimental procedure.



In summary, it is obvious that rapid initial tooth movement was apparent following the application of orthodontic force after cortical activation. However, when we re-applied orthodontic force after certain period of time followed by cortical activation, the cellular activities of surrounding periodontal tissue became lower compared to those in the case that underwent tooth extraction only without any experimental procedure.

Any intentional trial on cortical bone to fasten tooth movement needs more further study on both initial reaction and long term changes of surrounding tissue, bone and cellular responses as well as to minimize any undesirable changes.
