진행성 담도암 치료에 있어서 Gemcitabine과 Cisplatin의 병합요법

dc.description.abstract[한글] 본 연구의 목적은 진행성 담도암에 있어서 gemcitabine과 cisplatin의 효과와 독성을 알아보기 위함이다. 2002년 4월부터 2003년 3월까지 연세의료원에 진행성 담도암으로 내원한 환자를 대상으로 gemcitabine 1000mg/m2 을 매 4주마다 각각 제1일, 8일과 15일째 정주하였고 cisplatin 80mg/m2 을 매 1일 마다 정주하였다. 치료 도중 질병이 진행할 경우 치료를 중단하였고 관해의 증거가 있거나 질병이 진행되지 않는 경우에는 6회까지 투여하였다. 총 12명의 환자가 연구에 포함 되었고 약물에 대한 반응의 평가가 가능한 환자는 10명이었다. 완전 관해를 보인 환자는 없었고 5예에서 부분 관해를 보였으며 1명에서 불변, 4예에서는 질병의 악화를 보여서 총 50%의 관해율을 보였다. 관해를 보인 환자의 반응기간의 중간값은 28주였다. Gemcitabine 의 실제 용량 강도의 중앙치는 667.9 mg/m2/week 였으며, 상대 용량 강도의 중앙치는 88.2% 였다. Cisplatin의 경우에는 실제 용량 강도의 중앙치는 17.5 mg/m2/week 였으며, 상대 용량 강도의 중앙치는 87.5% 였다. 12명의 환자에게 총 36회의 항암 화학요법이 시행 되었으며 WHO 기준으로 3/4도에 해당하는 부작용은 백혈구 감소가 총 항암 요법 주기의 2.8% 빈혈이 2.8% 혈소판 감소가 5.6% 였다. 결론적으로 gemcitabine과 cisplatin은 진행성 담도암의 항암요법 약제로서 유용하며 적은 독성을 나타내었다. [영문] The efficacy and survival were evaluated in advanced biliary tract carcinoma patients treated with gemcitabine plus cisplatin chemotherapy. Twelve patients with locally unresectable or metastatic biliary tract cancer were enrolled. Gemcitabine was intravenously infused on days 1, 8 and 15 with a dose of 1000 mg/m2, and cisplatin on day 1 with a dose of 80 mg/m2 every four weeks. A total of six cycles of chemotherapy was performed, but in case of disease progression, chemotherapy was terminated. Ten patients were available for response evaluation. Five patients experienced partial response, 1 had stable disease, and disease progression was noted in 4, showing an overall response of 50%. Median progress-free survival was 28 weeks. (2-54 weeks, 95% CI). The median delivered dose was 667.9 mg/m2/week for gemcitabine and 17.5 mg/m2/week for cisplatin, corresponding to a relative dose intensity of 88.2% and 87.5% respectively. Hematologic toxicities were grade 4 neutropenia in 1 patient (2.8%), grade 3 anemia in 1 patient (2.8%), and grade 3 thrombocytopenia in 2 patient (5.6%) during a total of 35 cycles of chemotherapy. These results suggest that gemcitabine and cisplatin combination chemotherapy is a potentially effective, safe, and well tolerated regimen for treating patients with advanced biliary tract cancer and gall bladder cancer.-
