만성폐쇄성폐질환 환자의 총체적 사정 도구 개발

본 연구의 목적은 만성폐쇄성폐질환(COPD) 환자의 효율적인 관리를 위해 총체적인 COPD 환자 사정 도구를 개발하는 것이다. 본 연구는 WHO(1998)에서 발표한 건강의 다차원적 측면 즉, 신체적, 정신적, 사회적, 영적 영역을 본 연구의 개념적 기틀로 하여 COPD 환자에서 사정할 수 있는 신체적, 정신적 사회적, 영적인 다차원적 측면을, 국내외의 문헌과 국외 호흡기 단체에서 언급된 COPD 환자의 사정 내용, 그리고 국외의 3차 병원 4곳에서 사용중인 COPD 환자 사정 도구 내용을 토대로 예비 사정 내용을 작성하여 2차에 걸친 전문가 타당도 및 임상 적용 가능성을 검증한 후 최종 COPD 환자의 총체적 사정 도구를 개발하는 과정을 거쳤다.본 연구의 결과는 다음과 같다. 1. WHO(1998)에서 발표한 건강의 다차원적 측면 즉, 신체적?정신적?사회적?영적 영역을 대분류라 명칭하고, 이에 따른 중분류와 소분류는 국외 호흡기 학회에서 제시한 COPD 사정내용과 문헌고찰을 토대로 작성하여 대분류 4항목, 중분류 17항목, 소분류 66항목이 본 연구의 개념적 준거틀로 확정되었다. 2. 대분류-중분류, 중분류-소분류 흐름의 적절성에 대한 전문가 1차 타당도 검증 결과 CVI(Index of content validity) 78% 미만인 항목을 삭제하여 대분류 4항목, 중분류 16항목, 소분류 46항목으로 정리되었다. 3. 문헌고찰을 통해 각 소분류에 해당하는 세부내용을 작성한 결과 소분류 46항목에 따른 세부내용 110항목이 도출되었다.4. 각 소분류에 따른 세부항목의 적절성에 대해 전문가 2차 타당도를 실시하여 CVI 78% 미만의 항목을 삭제한 결과 세부항목 22항목이 삭제되어 총 88항목이 최종 COPD 환자 사정 예비 내용으로 선정되었다. 5. 소요시간과 항목간 배열의 적절성, 사정도구 내의 용어의 적절성, 자료수집의 용이성을 토대로 임상 적용 가능성을 검증한 결과, 신체적 영역의 세부항목에서 1항목, 사회적 영역의 세부항목에서 3항목이 다른 항목들과 중복되어 삭제하였다.6. 전문가 타당도와 임상 적용 가능성 검증을 통해 작성된 COPD 환자의 총체적 사정 도구는 신체적, 정신적, 사회적, 영적 영역을 측정하는 내용이 모두 포함되어 있고, 신체적 영역으로 내과력, 신체검진, 진단검사, 영양, 처치력, 증상, 일상생활활동, 운동, 수면력과, 정신적 영역으로 정서상태, 인지상태, 사회적 영역으로 삶의 질, 지지, 환경, 경제, 그리고 영적 영역으로 영적건강을 사정할 수 있도록 구성하였다. 본 연구는 COPD 환자의 총체적 사정을 위해 신체적, 정신적, 사회적, 영적 영역을 모두 포함한 도구를 개발하도록 시도되었다. 이 도구는 COPD 환자의 현 상태를 파악하고 간호의 구체적인 방안을 제공하는 데 도움이 될 것이다.

The purpose of this study was to develop a holistic assessment tool for patient with chronic obstructive pulmonary disease(COPD) in order to offer COPD patients an effective care. A conceptual framework was developed through a multiple dimension of health, physical, psychological, social, and spiritual dimension, which was announced by WHO in 1998. A preliminary tool was developed through a review of literature, announcement of international respiratory society about contents of assessment for COPD patients, and existing instruments that are currently used in 4 hospitals for assessing the patients with COPD.Through assessing the two expert validity tests and a feasibility test of the preliminary tool, the holistic assessment tool for COPD patients was finalized.The results or this study are as follows: 1. After a review of literature, announcement of international respiratory society, and existing instrument, 4 items of the first category, 17 items of the second category, 66 items of the third category were developed. 2. The first expert validity test showed that 4 items of the first category, 16 items of the second category, and 46 items of the third category received over 78% agreement. 3. After a review of literature, announcement of international respiratory society, and existing instrument, 110 items of the fourth category, which were more specific, were developed. 4. The second expert validity test showed that 88 items of the fourth category received over 78% agreement, and these items were used to develop a preliminary assessment tool for patients with COPD. 5. After a feasibility test, whether the time to finish the tool, the order of items, and the vocabulary of the content were adequate, and the data collection was easy, 4 items of the fourth category was deleted from the preliminary assessment tool because of the redundancy. 6. The holistic assessment tool for patients with COPD was finalized after two expert validity tests and a feasibility test. This assessment tool was included in physical, psychological, social, and spiritual dimension. Specifically, this tool was included that medical history, physical assessment, diagnostic test, nutrition, treatment history, symptom, activity of daily living, exercise, and sleeping history were for the physical dimension, emotional status and cognitive status were for the psychological dimension, quality of life, support, environment, and economy were for the social dimension, and spiritual well-being was for the spiritual dimension. This study was tried to develop an assessment tool, which contains all the physical, psychological, social, and spiritual dimension to assess the patients with COPD holistically. This tool will be effective to check the current health status of COPD patients, and to set a specific nursing plan for those patients.
