혈관연축에 의한 지연성 뇌허혈은 지주막하 출혈후에 생기는 가장 중요한 합병증으로 혈관연축은 뇌혈관 조영술에 50-70%가 나타나고 지연성 뇌허혈은 19-46%가 나타난다. 그래서 지연성 뇌허혈을 예측하려는 많은 연구가 이루어졌고 초기 뇌전산화 단층촬영(computed tomography)상에서 보이는 혈액량이 많을수록 지연성 뇌허혈이 잘 생긴다는 연구는 동물 실험과 뇌혈관 조영술을 통해 잘 알려져 있다. 20년 전부터 비침습적으로 뇌기저동맥들의 혈류의 역학적인 변화를 간편하게 시행할 수 있는 경두개 도플러 검사(transcranial Doppler)가 발전되어 왔는데 초기 뇌전산화 단층촬영에서 보이는 혈액량과 경두개 도플러 검사상의 평균 혈류 속도(mean flow velocity)와의 관계에 대한 연구들이 진행되어 왔는데 서로 이견이 많다. 본 연구에서는 1999년 3월부터 2002년 2월 사이에 자발성 지주막하 출혈로 입원한 환자 중 72시간내 뇌전산화 단층촬영을 시행하였고 조기 수술, 대증적인 치료 혹은 aneurysmal coiling을 시행 받은 환자를 대상으로 하였다. 초기 뇌전산화 단층촬영상의 출혈량을 Ljunggren의 방법과 Fisher의 방법으로 각각 측정한 후 경두개 도플러 검사상의 중간 대뇌 동맥(middle cerebral artery)의 평균 혈류 속도의 최고값과 연관성을 분석하여 다음과 같은 결과를 얻었다.1. 대상 환자는 총 81명, 남자 28명과 여자 53명이었으며, 평균 연령은 54.67±13.36세였다. 2. 지주막하 출혈 후 중간 대뇌 동맥의 평균 혈류 속도는 10일에서 12일 사이에 가장 높았다.3. 출혈량이 적은 Fisher grade 1은 출혈량이 많은 Fisher grade 2 나 3보다 중간 대뇌 동맥의 평균 혈류 속도의 상승이 적었으나 단순 회기 분석상 Fisher의 방법과 Ljunggren의 방법 모두에서 출혈량과 평균 혈류속도의 최고값과는 통계학적으로 의미있는 연관성은 없었다.4. 나이가 젊을수록 최고 평균 혈류 속도의 값은 증가하였다. 성별, Hunt & Hess grade와 Fisher grade는 평균 혈류 속도와 통계학적인 연관성은 없었다.결론적으로 지주막하 출혈 후 초기 뇌전산화 단층촬영상의 출혈량은 중간 대뇌 동맥의 평균 혈류 속도의 상승과 연관성 있는 강력한 예측인자는 아닌 것으로 사료된다.

[영문]

Purpose: Delayed ischemia due to vasospasm is major morbidity of patients with subarachnoid hemorrhage(SAH) due to ruptured aneurysm. Angiographical vasospasm is developed 50-70%, symptomatic vasospasm 19-46% of patients with SAH. Many studies have confirmed that vasospasm is related with the amount and distribution of blood clots in subarachnoid space. but, the relationship between mean flow velocity(MFV) of middle cerebral artery(MCA) by transcranial Doppler(TCD) and blood clots amount on initial computed tomogrpahy(CT) is still controversial. The aim of this study is to clarify the relationship between the amount of blood clots on initial CT and blood flow velocity changes in the MCA as measured by TCD.Methods: We studied serial MCA velocities by TCD in 81 patients with subarachnoid hemorrhage from March, 1999 to February, 2002. With all patients, the first CT was taken within 72 hours after symptom onset and all managed by early operation, aneurysmal coiling or conservative treatment. The amount of blood clots on initial CT was measured by both Ljunggren method and Fisher Method. Result 1. Total 81 patient, male 28 and female 53, were involved. Mean age was 54.67(mean)±13.36(standard deviation) years.2. MFV of MCA on affected side reached maximum peak between 10 to 12 days after SAH.3. Average MFV of Fisher grade 1 was higher than that of Grade 2 or 3. But, there was no statistically significant relationship between the amount of blood clots measured by Ljunggren method and Fisher method on initial CT and maximal MFV of MCA on affected side.4. The younger the patient was, the higher maximal MFV was. Conclusion: It is concluded that the amount of blood clots on the initial CT is not a powerful predictor of cerebral blood flow velocities measured by TCD.
