이중혈류유발 심폐소생술은 흉골 압박과 흉곽 수축을 동시에 유발하는 새로운 심폐소



생술 방법으로서, 표준 심폐소생술에 비하여 혈역학적 효과가 우수하고 심정지 후 생존율을 증가시키는 것으로 알려졌다. 그러나 이중혈류유발 심폐소생술에 의한 혈역학적 효과의 기전과 이중혈류유발 심폐소생술을 할 때 혈역학적 효과를 극대화시키기 위한 압박 회수 및 압박/이완의 비율에 대한 연구는 없었다. 본 연구의 목적은 이중혈류유발 심폐소생술에 의해 최적의 혈역학적 효과가 발생하는 흉부압박 회수와 압박과 이완의 시간적 비율을 결정하고, 동시에 이중혈류유발 심폐소생술의 주된 혈류유발 기전을 밝히는 것이다.12마리의 개(잡견, 체중 18-25kg)를 압박주기 변화 실험군과 압박회수 변화 실험군으로 나누었다. 압박주기 변화 실험군은 흉부 압박회수를 분당 80회로 유지하면서 압박과 이완의 비율을 30%:70%, 40%:60% 및 50%:50%로 변화시켜 심폐소생술을 시행하였다. 압박회수 변화 실험군은 압박과 이완의 비율을 50:50으로 유지하면서 분당 흉부압박 회수를 60회, 80회, 100회 및 120회로 변화시켜 심폐소생술을 시행하였다. 각 군으로부터 측정된 호기말 이산화탄소 분압, 수축기 및 이완기의 대동맥압과 우심방압, 수축기 총경동맥 혈류량, 관상동맥 관류압(대동맥이완기압-우심방이완기압)을 서로 비교하였다. 압박회수 변화에 따른 경동맥 혈류량은 분당 흉부압박 회수가 60회일 경우 4530±10.0 mL/min, 80회일 경우 53.0±11.7 mL/min, 100회일 경우 61.9±9.0 mL/min, 120회일 경우 45.3±7.1 mL/min으로 분당 100회에서 가장 높은 경동맥 혈류량이 유발됨으로써 흉부압박 회수에 따라 변화함을 알 수 있었다(p=0.03). 압박주기 변화군에서 호기말 이산화탄소 분압은 압박과 이완의 비율이 30:70일 경우 10.33±1.51 mmHg, 40:60일 경우 12.33±3.67 mmHg, 50:50일 경우 14.5±2.81 mmHg로 흉부압박과 이완의 비율이 50:50에서 가장 높은 호기말 이산화탄소 분압을 보임으로써 압박기간이 증가할수록 혈류량이 증가함을 알 수 있었다(p=0.03).결론적으로, 이중혈류유발 심폐소생술에 의한 개의 심정지 모델에서 흉부압박과 이완의 비가 50:50, 분당 압박회수가 100회일 때 가장 효율적인 혈역학적 효과가 유발됨을 알수 있었으며, 이중혈류유발 심폐소생술의 혈류유발 기전은 심장펌프와 흉강펌프가 동시에 작동하는 심폐소생술 기법임을 증명하였다.

Simultaneous sterno-thoracic cardiopulmonary resuscitation(SST-CPR) is a new method of CPR, which performs sternal compression and thoracic constriction simultaneously. Animal experiments have shown that better hemodynamic effects are obtained with SST-CPR compared to standard CPR. SST-CPR also improves the short-term survival rate in canine cardiac arrests. However, the exact mechanism of the enhanced hemodynamic effect with SST-CPR needs to be understood, and the compression rate and compression relaxation ratio to produce the maximal hemodynamic effect with SST-CPR remains to be determined. This study was designed to determine the compression rate and compression relaxation ratio to produce the optimal hemodynamic effect with SST-CPR and to investigate the mechanism of blood flow generated by SST-CPR. A canine model of ventricular fibrillation was used. Twelve mongrel dogs(weight range: 18-25 kg) were divided into two groups. The first six animals were resuscitated by SST-CPR with compression durations varied randomly at 2-minute intervals for 30%, 40%, and 50% of the CPR cycle, at a constant rate of 80/min. In the other six dogs, SST-CPR was performed with randomly varied compression rate of 60, 80, 100, and 120 per minute, at a 50:50 compression/relaxation ratio. Increasing the compression-relaxation ratio from 30:70 to 50:50 resulted in an increase of end tidal CO2 during SST-CPR from 10.33±1.51 mmHg to 14.5±2.81 mmHg. Increasing the compression rate from 60 to 100 tended to improve the carotid blood flow. However, further increase in compression rate resulted in reduction of carotid blood flow from 61.9±9.0 mL/min to 45.3±7.1 mL/min. In conclusion, the maximal hemodynamic effects with SST-CPR was generated when compression rate was 100 per minute and compression/relaxation ratio was 50:50. A combination of both cardiac and thoracic pump theory may be the mechanism of blood flow produced by SST-CPR.
