구강 점막의 전암병소는 악성으로 이행 가능성이 높은 병소를 의미하며, 백반증은 가장 대표적인 전암병소로 침윤성 암종으로 진행할 수 있다. 상피의 이형성 정도에 따라 5%에서 43%까지 암종으로 진행하며 50%이상은 자연적으로 소실되거나 악성 이행을 일으키지

않는다. 이러한 악성 이행의 위험성을 예측할 수 있는 인자로는 상피 이형성이 가장 신뢰할 만 하지만, 이형성의 경우 판단이 주관적이며 등급 결정에 명백한 기준이 없어 문제가 되고 있어, 생물학적 표지자를 이용하여 종양의 진행 여부를 예측 할 수 있다면 객관성과 신뢰도를 높일 수 있다. 이 중 종양 유전자 혹은 종양 억제 유전자를 이용한 지표는 암으로의 진행 단계를 분자생물학적 차원에서 분석할 수 있다는 점 때문에 종양 발생 위험도를 예측할 수 있는 표지자로 활용 가능성이 높은 것으로 생각할 수 있다.

이 연구에서는 종양 억제 유전자로서 구강 편평 세포 암종에 가장 높은 변이를 보이는 유전자인 p53과 증식성 기저 세포에서 높게 발현되는 p63의 발현 정도를 분석하여 암 발생 위험율을 예측할 수 있는 지표로서의 가능성을 알아보고자 하였다. 연세대학교 치과대학 구강병리학교실에 의뢰된 생검 표본 중에서 백반증으로 진단 받고 2년 이상 추적 관찰한 환자 28예를 대상으로 하였고 이들 중 암종으로 이행하지 않은 21예와 암종으로 이행한 7예, 대조군으로 정상 치은 조직 6예를 면역조직화학적으로 p53과 p63 발현을 조사하여 다음과 같은 결과를 얻었다.

1. p53의 경우 정상 조직에서는 발현되지 않았으며, 전체 백반증 조직에서 28예 중 9예(32%)가 양성 반응을 보였고, 악성 이행이 없는 경우는 21예 중 6예(29%), 악성 이행이 있는 경우는 7예 중 3예(43%)로 나타났다.

2. p63의 경우 전체 백반증 조직에서 28예 중 14예(50%)가 양성 반응을 보였고, 악성 이행이 없는 경우는 21예 중 9예(42%), 악성 이행이 있는 경우 7예 중 5예(71%)가 양성으로 나타나 악성 이행이 있는 경우 발현율이 높게 나타났다.

3. 병리조직학적 분석 결과 상피 이형성이 있는 경우 16예 중 7예(43%)가 악성 이행을 하였고 이 중 경도의 이형성인 경우 12예 중 3예(25%)가 중등도 이상인 경우 모두 악성 이행을 하여 이형성 정도가 암 발생 위험도 예측에 신뢰성이 높았다.

이상의 결과로 p53과 p63의 단백 발현이 전암병소에서 암 발생 위험도를 예측하는 표지자로서의 가능성이 있다고 생각된다. 하지만 암종으로 이행되면서 이형성 정도와 발현율이 비례하지 않았다. 따라서 효과적인 표지자로서의 가능성을 확인하기 위해서는 앞으로 더 많은 증례에 대하여 추적 조사와 관찰이 필요하다고 생각된다.

[영문]

The oral precancerous lesions have been considered as a target tissue for the early detection and chemoprevention study. The risk of malignant transformation in oral premalignant lesions should be accurately evaluated and properly treated to prevent from cancer development. Accordingly, the risk assessment and monitoring of chemoprevention effect are critically important to reduce cancer incidence.

Epithelial dysplasia has been considered as a reliable histological hallmark to define premalignant lesions. But the subjectivity of evaluating the degree of epithelial dysplasia and the fact that the degree of epithelial dysplasia may not directly correlate to the rate of malignant transformation, require more reliable biomarkers to represent their biologic behavior.

This study aimed to evaluate the usefulness of p53 and p63 expression as biomarkers for the risk assessment to malignant transformation in leukoplakia. This study used 21 cases of oral leukoplakia without malignant transformation and 7 cases with

malignant transformation during more than 2 years follow-up period which were examined at the Department of Oral Pathology, Yonsei University College of Dentistry. Six cases of normal mucosa were used as a control. Monoclonal p53 and p63 antibodies were applied for immunohistochemical study.

The results were as follows:

1. p53 expression was shown in 6 of 21 cases(29%) of the leukoplakia without malignant transformation, and in 3 of 7 cases(43%) of the leukoplakia transformed to squamous cell carcinoma. Normal oral mucosa showed no detectable p53 expression.

2. p63 expression was detected in 9 of 21 cases(42%) of the leukoplakia without malignant transformation, and 5 of 7 cases(71%) of the leukoplakia transformed to squamous cell carcinoma. In normal mucosa 2 of 6 cases(33%) showed p63 expression in basal and parabasal layer.

3. Malignant transformation occurred in 7 out of 16 cases(43%) of the leukoplakia with epithelial dysplasia, 3 out of 12 cases(25%) of mild epithelial dysplasia and all of moderate to severe epithelial dysplasia were transformed to squamous cell carcinoma.

These results suggest that p53 and p63 expression requires more intensive further study based on the longer period of follow-up and the larger scale-examination for the verification of the usefulness as a predictable biomarker assessing the risk of malignant transformation in oral precancerous lesions.
