비당뇨병성 만성 신질환 환자에서 안지오텐신 수용체 길항제의 단백뇨 감소 효과에 대한 안지오텐신 전환효소 유전자 다형성의 영향

dc.description.abstract[한글] 레닌-안지오텐신 계(renin-angiotensin system)는 신질환의 손상 및 진행에 중요한 역할을 하는 것으로 알려져 있다. 이중 안지오텐신 II (angiotensinⅡ)는 레닌-안지오텐신 계의 주요 매개체일 뿐만 아니라 강력한 혈관 수축제로서 각종 만성 신질환에서 신 손상 및 진행에 중요한 역할을 한다. 안지오텐신 수용체 길항제(angiotensin receptor antagonist)는 이러한 일련의 안지오텐신 II 매개 생리적 작용, 즉 염분의 재흡수 및 부신에서 알도스테론(aldosterone)과 아드레날린 호르몬(adrenergic hormone)의 분비 촉진 등을 억제함으로써 혈압을 낮추는 효과를 나타낸다. 본 연구에서는 비당뇨병성 만성 신질환 환자에서 안지오텐신 수용체 길항제의 단백뇨 감소 효과에 대해 알아보고 레닌-안지오텐신 계 구성요소의 유전자 다형성 중 안지오텐신 전환효소[angiotensin -converting enzyme (ACE)]의 유전자 다형성을 검사하여 각각의 유전자형에 따른 안지오텐신 수용체 길항제의 단백뇨 감소효과에 차이가 있는지를 분석하여 다음과 같은 결과를 얻었다. 대상 환자는 연세대학교 세브란스병원 신장내과에 내원하거나 입원한 환자 중 일일 500 mg 이상의 단백뇨를 동반한 비당뇨병성 만성 신질환 환자중 본 연구에 동의한 총 70명의 환자를 대상으로 전향적 연구를 시행하였다. 안지오텐신 수용체 길항제인 losartan을 연구시작 시점부터 12주까지 50mg, 12주부터 24주까지 100mg으로 증량하여 투여하면서 4주 간격으로 추적관찰하였다. 대상환자 70명에서 losartan 투여 전에 비해 투여 12주후 및 24주후 수축기혈압(135.9±16.4mmHg vs. 119.5±15.0mmHg, 119.0±15.7mmHg, p<0.05), 이완기혈압(88.8±10mmHg vs. 77.4±8.8mmHg, 75.8±8.2mmHg, p<0.05), 평균동맥압(104.5±10.3mmHg vs. 91.5±9.4mmHg, 90.2±9.2mmHg, p<0.05), 24시간 요단백 배설량(2243.0±1554.2mg/day vs. 1473.3±1336.5 mg/day, 1187.2±1582.1mg/day, p<0.05), 혈청 콜레스테롤(215.3±42.9mg/dL vs. 191.2±31.1mg/dL, 183.9±30.3mg/dL, p<0.05), 그리고 혈청 중성지방(231.8±189.9mg/dL vs. 174.1±101.6mg/dL, 182.9 ±134.2mg/dL, p<0.05)은 유의하게 감소하였다. Losartan 투여전 24시간 요단백 배설량과 투여 12주후 및 24주후의 요단백 배설량 감소 정도 사이에는 유의한 상관관계가 있었다(12주;R2=0.318, p<0.05, 24주;R2=0.228, p<0.05). Losartan 투여 12주후 30% 이상의 단백뇨 감소 효과를 보인 환자군을 반응군으로 설정하여 비반응군과 비교 분석한 결과 losartan 투여전 혈압, 혈청 요소질소, 혈청 크레아티닌, 그리고 24시간 요단백 배설량에는 유의한 차이가 없었으나, 12주 및 24주후 수축기 혈압, 평균동맥압은 반응군에서 의의있게 낮았으며, 12주후 24시간 요단백 배설량도 반응군에서 유의하게 낮았다(1147.4±1055.2mg/day vs. 2288.2±1623.8mg/day, p<0.05). 안지오텐신 전환효소 유전자형에 따른 기저 혈압 및 24시간 요단백 배설량에는 차이가 없었으며, losartan 투여 12주 및 24주후 혈압의 변화, 24시간 요단백 배설량의 변화, 크레아티닌 청소율(creatinine clearance)의 변화, 그리고 반응을 보인 환자의 빈도에는 유의한 차이가 없었다. /이상의 연구결과로 비당뇨병성 만성 신질환 환자에서 안지오텐신 수용체 길항제는 혈압 및 요단백 배설을 의의있게 감소시킴을 관찰하였으며, 안지오텐신 전환효소 유전자 다형성에 따른 안지오텐신 수용체 길항제의 혈압 및 단백뇨 감소 효과에는 유의한 차이를 관찰할 수 없었다. 향후 좀더 많은 환자를 대상으로 안지오텐신 전환효소 유전자 다형성에 따른 안지오텐신 수용체 길항제의 장기적인 효과에 대한 추가연구가 필요할 것으로 사료된다. [영문] Background. Renin-angiotensin system plays an important role in renal injury and in the progression of renal disease. Angiotensin II, a potent vasoconstrictor, has a key role in renal injury and in the progression of chronic renal disease of diverse causes. In every organ system, the biologic effects of angiotensin II are mediated through its interaction with specific receptors on cell membranes. Angiotensin II receptor antagonist specifically inhibits angiotensin II-mediated physiologic responses such as systemic and renal vasoconstriction, sodium reabsorption by renal proximal tubule, and stimulation of aldosterone and adrenergic hormone release by the adrenal gland. Angiotensin II receptor antagonist, losartan has been reported to have a significant antiproteinuric effect in patients with diabetic and non-diabetic renal disease. This study was carried out ascertain whether angiotensin -converting enzyme(ACE) gene polymorphism might affect the renal response to angiotensin II receptor antagonist in non-diabetic proteinuric chronic renal patients. Methods. Seventy patients with non-diabetic chronic renal disease with urine protein excretion greater than 500 mg/day were enrolled in this prospective study. Subjects were given losartan 50mg o.d. for first 12 weeks, and then 100mg o.d. and were followed up to 24 weeks. Results. Twelve weeks and twenty-four weeks later, blood pressure, urinary protein excretion, total cholesterol, and triglyceride significantly decreased compared with baseline values. There was a significant correlation between the levels of baseline urinary protein excretion and the magnitudes of the reduction of urinary proteinuria excretion after treatement with angiotensin II receptor antagonist. Baseline blood pressure, BUN, serum creatinine, and urinary protein excretion were not different in the responder group (patients with 30% or more reduction of urinary protein excretion after losartan treatment) compared with the nonresponder group. Systolic blood pressure and mean arterial pressure in the responder group were significantly less than the nonresponder group after twelve and twenty-four weeks of losartan treatment. Urinary protein excretion in the responder group was significantly lower than the nonresponder group after twelve weeks. There were no significant differences in blood pressure, urinary protein excretion, creatinine clearance, distributions of responders among three groups in accordance with ACE genotypes after angiotensin II receptor antagonist treatment. Conclusions. Our results suggest that angiotensin receptor antagonist, losartan significantly reduced blood pressure and proteinuria in patients with non-diabetic chronic renal disease. The magnitude of antiproteinuric effect of losartan was not influenced by ACE gene polymorphism. However, further large scale studies are required to confirm the issues regarding the ACE gene polymorphism and the antiproteinuric effects of angiotensin II receptor antagonist in non-diabetic chornic renal disease.-
