천포창(pemphigus)은 피부와 점막에 수포를 형성하는 만성 수포성 질환으로서 대부분의 환자는 혈청내에 각질형성세포 표면의 교소체(desmosome)에 존재하는 desmoglein(Dsg)과 결합하는 자가항체를 가지고 있으며 이 자가항체가 천포창의 발병에 직접 관여한다는 점에서 대표적인 자가항체에 의한 자가면역질환이라 할 수 있다. 따라서 천포창의 근본적인 병인으로 작용하는 자가항체를 검출, 정량하는 것은 질병의 진단 뿐 아니라, 질병의 활성도를 예측, 판정하여 치료에 도움을 줄 수 있기 때문에 매우 중요하다. 최근 널리 쓰이는 효소면역흡수법(Enzyme linked immunosorbant assay ; ELISA)은 특정 항체를 정량적으로 측정할 수 있는 방법으로, 천포창에서도 심상성 천포창과 낙엽상 천포창의 각각의 표적항원에 대한 특이항체의 검출이 가능할 뿐 아니라 질병의 활성도나 중증도 예측에 있어서도 기존의 간접면역형광법(Indirest immunofluorescence ; IIF)보다 민감함이 여러 연구에서 제안된 바 있다. 연구자는 천포창의 진단 뿐 아니라 질병의 활성도 측정에 있어서의 ELISA의 유용성을 검토하고자 심상성 천포창 환자 4명과 낙엽상 천포창 환자 4명을 대상으로 하여 DsgELISA를 시행하여 DsgELISA로 측정한 Dsg1,3에 대한 자가 항체의 역가와 임상 중증도와의 상관성을 규명하고, IIF에 의해 측정된 항체 역가와 비교함으로써 다음과 같은 결과를 얻었다.

심상성 천포창 환자 4명에서 Dsg1 ELISA 항체가는 피부 병변의 유무나 중증도의 변화와 상관성을 보였으며 Dsg3 ELISA는 한명의 환자를 제외한 세 명의 환자에서 점막의 임상 중증도의 변화와 상관성을 보였다. 그러나 IIF의 경우 임상 중증도와의 상관성이 부족하였는데, 이는 IIF는 Dsg1과 Dsg3를 구분하지 못하며 저자들이 사용한 IIF의 기질인 인간 포피의 상태가 일정하지 못하였기 때문에 심상성 천포창에서 ELISA에 비해 신뢰도가 더욱 떨어졌다고 추측된다 낙엽상 천포창 환자 4명에서는 대부분 임상 중증도의 변화와 IIF로

측정된 항체의 역가나 Dsg1 ELISA로 측정된 항체의 역가와 상관성이 있었고 그 상관 정도는 ELISA가 IIF보다 높은 결과를 보였다. Dsg1,3 ELISA는 IIF보다 우수하게 임상 양상의 중증도와 상관성을 보였으나, 천포창의 예후를 미리 예측하거나 치료의 방향을 제시하는 절대적인 지표가 되기에는 좀 더 충분한 연구가 필요하다.

[영문]

Pemphigus is a group of chronic bullous diseases involving skin and mucous membranes, characterized by the presence of circulating antibodies binding to desmoglein(Dsg) located in desmosomes of the keratinocytes. It could be categorized as a autoimmune disease, because these antibodies are directly involved in the pathogenesis of pemphigus. The detection and titration of these autoantibodies are not only crucial for the diagnosis of pemphigus but also for the determination of the disease activity and the course.

ELISA(Enzyme linked immunosorbant assay ; ELISA) is the method that could measure quantitatively the specific antibodies. It enables the titration of each antibodies to a specific pemphigus antigen. Some studies even suggest a higher sensitivity of ELISA in prediction of the activity and severity than indirect

immunofluorescence(IIF).

The authors aimed to determine the relevance of the anti-Dsg 1, 3 antibody titers in relation to the clinical severity of the disease using the Dsg ELISA, comparing with the relevance of anti-Dsg antibody titers in relation to that using IIF and decide the practicality of the ELISA in diagnosis and determination of the activity compared with IIF. We conducted the study on two pemphigus groups, namely pemphigus vulgaris and pemphigus foliaceous, with four patients in each group.

In the group with pemphigus vulgaris, anti-Dsg 1 antibody titer obtained in ELISA correlated well with the severity of skin manifestation, and anti-Dsg 3 antibody titer obtained in ELISA did with the severity of mucosal manifestation in all but one patient. However, the IIF titers failed to correlate with clinical courses.

This could probably be explained by the facts that an IIF does not selectively differentiate Dsg1 from Dsg 3 and that we used the human epidermis as substrates in IIF.

In the second group with pemphigus foliaceous, both ELISA and IIF titers ran parallel with clinical courses, corresponding to the previous reports. However, a higher correlation ratio was demonstrated in ELISA than in IIF.

We expect a further investigation in the near future, to establish the utilization of Dsg ELISA as a standard method in the prediction of prognosis and for the therapeutic plan.
