브라켓 제거에 따른 입술과 주위 연조직의 변화

교정치료에 따른 연조직 변화 양상은 개개인의 차이가 심하므로 교정치료중 흔히 재평가가 필요하다. 또한 고정식 교정장치가 부착되어 있는 환자에서, 현 상태의 입술의 전후방적, 수직적 위치와 교정치료 종료후의 위치에 대한 비교 예측을 필요로 하는 상담을 흔히

접하게 된다. 그러나, 이에 대한 연구가 거의 없어 교정의는 주관적인 견해를 피력하는 경우가 많으므로, 고정식 교정장치의 제거에 따른 입술과 주위 연조직의 변화에 대한 근거있는 예측이 필요하다.

따라서, 교정치료가 종료되어 고정식 교정장치를 제거한 88명을 대상으로 T1(장치제거 전), T2(장치제거 직후), T3(장치제거 1.5개월 후)의 측모 두부 방사선사진상에서 입술과 그 주위 연조직을 분석한 결과 다음과 같은 결론을 얻었다.

1. 장치제거 직후(T2-T1) 하순(Li)이 후퇴하였으며, 장치제거 평균 1.5 개월 후(T3-T2)에도 하순이 계속 후퇴하였다.

2. 장치제거 후 구순 주위 연조직과 상순은 유의성있는 전후방적 변화를 보이지 않았다.

3. 기준선(G-perpendicular line)에 대한 입술의 후퇴양은 상순의 최전방정(Ls)에서 평균 0.38mm이고(p>0.01), 하순(Li)은 평균 0.88mm이었다(p<0.001).

4. 장치제거 후 입술과 주위 연조직의 수직적인 변화는 보이지 않았다.

5. 장치제거에 따른 입술의 변화는 남녀간에 전후방적, 수직적인 차이가 없었다.

Soft tissue changes according to orthodontic treatment is questionable due to individual variation, so re-evaluation during orthodontic treatment is needed. Some patients with fixed orthodontic appliance want to know if their lip positions would change after the removal of brackets, but there were few studies about it. The objectives of this study was to evaluate the changes of the lips and perioral soft tissue after bracket removal.

The sample used in this study was 32 males and 56 females. Cephalometric X-rays were taken for 3 times - T1(before debonding), T2(just after debonding), T3(1.5 months after debonding).

The results were as follows.

1. The lower lip was retruded just after the debinding(T2-T1), and 1.5 months after the debonding(T3-T2).

2. The anteroposterior changes of perioral soft tissue and upper lip were not significant after the debonding.

3. The amounts of retrusion from the vertical reference plane (G-perpendicular line) were about 0.38mm on the upper lip and 0.88mm on the lower lip.

4. There were no vertical changes of the lips and perioral soft tissue after debonding.

5. There was no significant difference between males and females in the antero-posterior and vertical changes of the lips and perioral soft tissue after debonding.
