간호사와 환아의 상위의사소통 분석

dc.description.abstract[한글] 본 연구는 아동병동에서의 간호사-환아 상호작용을 관찰하여 그 양상을 분석하고 상호작용에서 보이는 상위의사소통을 규명하는 관찰연구이다. 자료수집은 2001년 12월부터 2002년 2월까지 S시에 소재한 일 종합병원의 소아과 병동에서 실시하였다. 연구대상자는 소아과 병동의 간호사 6명과 5-6세의 담당환아 8명과 그 어머니들이었다. 자료수집을 위하여 연구자가 비참여관찰하면서 간호사-환아 상호작용 상황을 비디오카메라를 이용하여 녹화하였다. 관찰자료는 상호작용 참가자의 언어적 비언어적 행동으로 전사하였고 전사된 자료를 Bales의 상호작용 과정분석법의 12가지 상호작용범주를 이용하여 분류(coding)하였고, 분류된 자료를 기반으로 간호사-환아 상호작용 양상을 분석하였다. 간호사-환아 상호작용 양상은 상호작용의 횟수, 시간, 목적, 사례별 상호작용 과정분석, 시간경과에 따른 상호작용과정 그래프로 제시하였고, 간호사와 환아의 상위의사소통 행위는 내용분석법을 이용하여 추출하였다. 본 연구의 결과는 다음과 같다. 간호사-환아 8사례가 2일에 걸쳐 8시간 관찰동안 직접적인 상호작용 에피소드는 총 75회였고 각 사례별 상호작용의 평균시간은 39.75분이었다. 1회의 상호작용 에피소드의 평균시간은 4.24분이고 상호작용의 목적이 된 직접적인 간호활동은 정맥주사 시작과 채혈, 입원간호, 병실순회(상태, 수액, 주사부위 확인), 간호처치, 경구투약, 활력징후 측정, 정맥주사를 통한 투약, 일반적 대화로 이루어졌다. 간호사-환아-보호자 상호작용의 주된 주도자는 간호사였고 간호사와 환아간의 상호작용이 가장 빈번하게 나타났다. 상호작용 범주별로는 주목/동의를 보임, 친근하게 보임, 정보를 구함, 정보제공, 의견제시, 긴장을 보임, 긴장해소를 보임, 주목/동의하지 않음, 의견을 구함, 비우호적으로 보임, 제안을 구함의 순으로 나타났다. 간호사와 환아의 상호작용은 감정적인 상호작용 행위가 빈번하였다. 간호사와 보호자와의 상호작용은 중립적인 상호작용 행위가 빈번하였다. 전체적으로 1일에서 2일로 시간이 경과함에 따라 긍정적 반응이 증가하고 부정적 반응이 감소하였다. 사례별 상호작용 과정 분석 결과, 환아의 연령과 입원경험 등의 특성에 따라서 상호작용 양상이 다르게 나타났다. 간호사측 요인으로 4년제 간호대학을 졸업한 간호사들이 환아와의 상호작용에서 상호작용 양상이 다르게 나타나며, 상위의사소통 행위를 더 빈번하게 보였다. 또한 더 어린 환아에게 상위의사소통 행위와 정서적 반응의 상호작용 행위를 더 빈번하게 보였다. 상위의사소통 행위를 많이 보인 간호사-환아 사례일수록 상호작용 과정이 시간경과에 따라 긍정적 반응은 증가하고 부정적 반응은 감소하는 변화가 있었다. 간호사가 환아와의 상호작용에서 보이는 상위의사소통 행위는 다가가기, 눈높이 맞추기, 눈맞추기, 접촉하기, 격려나 칭찬하기, 되돌리기, 목소리 바꾸기, 선택하게 하기, 친근하게 요구하기, 확장하기, 덧붙이기, 반복 및 확인하기, 동질감 표현하기, 반영하기, 아동처럼 말하기, 상징으로 표현하기, 행위기술하기, 이완된 자세 취하기, 전환하기가 추출되었다. 이는 관심끌기, 반응을 유발하기, 공감하기, 긴장완화하기의 4가지 영역으로 범주화되었다. 결론적으로 아동병동에서의 간호사-환아 상호작용에서 간호사들은 상위의사소통 행위를 빈번하게 보였고 이는 관계형성을 촉진하는 것으로 나타났다. 그러므로 간호교육에 상위의사소통 행위를 도입하여 간호사의 효과적인 의사소통 기술을 촉진함으로써 간호사가 다른 특성을 가진 대상자에 대해 민감성을 가질 수 있도록 할 것이다. [영문] The purpose of this study was to analyze the interaction pattern and to identify metacommunicative behaviors between nurses and patients in a pediatric unit. The research method included observation using videotaping. Data were collected from December, 2001 to February, 2002. Total six nurses, and eight patients and their mothers in a pediatric unit participated in this study. The interaction was videotaped under the participants'' consent. The participants were observed for total 8 hours over 2-day period. Taped interaction was transcribed. Transcription included verbal and nonverbal interactions. Bales'' Interaction Process Analysis(1950, 1970) was used as a framework for data analysis. The interaction pattern studied in this research included frequency, length of time, and purpose of interaction, the dynamic of interaction, the changes occuring during the interaction, and metacommunicative behaviors. Metacommunicative behaviors were also identified, and categorized by the Mitchell(1991)''s definition of metacommunication. The results were as follows: The average time spent for each nurse to interact with one patient was 39.75minutes. All interactions took place in the context of nursing activities such as rounding, doing nursing interventions, checking vital sign, and having small talks. Most of the interaction was initiated by nurses. Affective behaviors were more frequent between nurses and children, and neutral behaviors were more frequent between nurses and mothers. Positive responses increased while negative responses decreased as time goes by. There was distinct difference in interaction pattern based on educational level and experience of nurses, the age and number of hospitalization of children. Nineteen metacommunicative behaviors identified and organized into four categories; call for attention, facilitating response, empathy, and tension release. It was shown that increased use of metacommunicative behaviors was related to increased positive interaction. In conclusion, nurses in this study used metacommunicative behaviors frequently and it was effective in developing relationship between nurses and patients. It is suggested that any educational programs to teach communication skills to nurses need to include techniques on metacommunicative behaviors, so nurses can be more sensitive to different characteristics of their patients.-
