본 연구는 가설적인 윤리적 딜레마 사례에 대한 간호사의 윤리적 의사결정은 어떠한지를 분석하기 위하여 4가지 윤리적 딜레마 사례에 대한 간호사의 의사결정을 Cameron(2000)의 ‘Value, Be, Do 윤리적 의사결정 모형‘을 활용하여 내용 분석하였다.

연구참여자는 현재 중환자실에 근무중이거나 과거에 중환자실 근무 경력이 있는 간호사를 대상으로 하였고 총16명이었다. 자료수집 기간은 2002년 4월 10일부터 5월 10일까지였고, 자료수집 방법은 간호부서의 동의를 받아 연구의 목적을 설명한 후 면담법과 구조화된 면담조사지를 사용하였다. 면담장소는 피면담자가 근무하는 중환자실이나 병동의 간호사 휴게실을 사용하였고, 면담시간은 1회 평균 40분 정도로 총 면담횟수는 3회였다. 자료분석은 질적 연구의 자료분석 절차를 참고로 내용을 분석하였다.

4개의 가설적인 윤리적 딜레마 사례에서 간호사의 윤리적 의사결정 시에 판단의 기준이 된 가치(Value)와 되고자 하는 바람직한 간호사의 모습(Be)과 행해야한다고 생각하는 행동(Do)에 대한 의사결정을 분석하였고 연구결과는 다음과 같았다.

의사결정 과정의 일관성을 파악한 결과 딜레마 사례별로 간호사의 대부분은 Value, Be, Do에 있어서 일관성 있는 의사결정을 하였는데 전체 사례에서 총 8명의 간호사들이 일관성이 결여된 의사결정을 하였고, 사례별로 일관성이 결여된 의사결정을 한 간호사를 보면 회복이 불가능한 환자에 대한 적극적인 치료 사례에서는 3명, 말기환자에게 사실 말하기 사례에서는 1명, 의료진과 상호 비협조적인 관계로 인한 갈등 사례에서는 3명, 임종환자와 안락사 사례에서는 3명의 간호사가 있었다. 간호사 개인별로 보았을 때는 한 명의 간호사가 3개의 딜레마 사례에서 일관성이 결여된 의사결정을 하였다. 일관성이 결여된 의사결정의 경우에서 판단의 기준이 된 가치가 명확하지 않은 경우, 가치와 모습과 행동이 연계되지 않은 경우, 가치와 모습은 연계되었으나 행동으로 통합되지 않는 경우 등이 있

었다.

가치(Value)와 모습(Be)과 행동(Do)의 의사결정 내용을 분석하여 도출된 Value, Be, Do의 중심개념은 사례별로 다음과 같았다.

회복이 불가능한 환자에 대한 적극적인 치료 사례의 간호사 가치(Value)의 중심개념은 적극적 처치의 무의미성, 인간의 존엄성, 가족의 부담 경감, 환자와 보호자 존중, 환자?보호자?치료진간의 합의 존중, 전문직 간호사의 의무 등이었다. 간호사의 모습(Be)의 중심개념은 옹호자와 중개자로 구분되었다. 행동(Do)의 중시개념은 충실한 직무수행, 불필요한 연명술 중단, 의료진과 협동, 환자?보호자 옹호, 가족과 의사 설득 등이었다.

말기 환자에게 사실을 말해야 하는 경우 사례에 대한 간호사 가치(Value)의 중심개념은 환자의 알권리, 생을 정리할 기회, 환자?보호자 존중, 환자의 자기결정권 존중 등이었다. 간호사의 모습(Be)의 중심개념은 옹호자와 중개자와 협력자로 구분되었다. 행동(Do)의 중심개념은 사실알리기, 환자와 가족의지지, 보호자와 의사 설득, 호스피스 간호 제공, 보호자의 요구존중, 보호자와 환자간에 대화 중재 등이었다.

의료진과의 상호 비협조적인 관계로 인한 갈등 사례에 대한 간호사 가치(Value)의 중심개념은 환자존중, 생명존중, 협동관계, 환자와 보호자 존중, 전문직 간호사의 의무 등이었다. 간호사의 모습(Be)의 중심개념은 협력자, 옹호자, 제안자로 구분되었다. 간호사의 행동(Do)의 중심개념은 충실한 직무 수행, 대화로 타협, 공식적 대화채널 이용, 의료진과 협동, 환자 배려 등이었다.

임종환자와 안락사 사례에 대한 간호사 가치(Value)의 중심개념은 합법성, 인간의 존엄성, 정직성, 전문직 간호사의 의무, 환자요구 존중 등이었다. 간호사 모습(Be)의 중심개념은 옹호자, 준법자, 중개자, 협력자로 구분되었다. 간호사 행동(Do)의 중심개념은 허위

기록 거부, 의료진과 협동, 의사잘못 묵인, 적극적인 안락사 반대, 상관에게 보고 등이 있었다.

Value, Be, Do의사결정 모형의 적용에 대한 평가에서 간호사들은 딜레마 상황에서 의사결정 시에 일관된 가치관을 수립하여 바람직한 행동을 수행하고 갈등상황에 대처하는데 도움이 되었다고 하였다.

앞으로 간호사들이 명확한 윤리적 가치관을 가지고 윤리적 의사결정을 할 수 있도록 간호윤리 교육을 강화하고 졸업 후 보수교육을 통해 계속적인 윤리 교육이 제공되어야 하겠고, 의사결정 시에 윤리적으로 옳고 체계적인 의사결정을 할 수 있도록 윤리적 의사결정 모형의 임상 적용이 활성화되어야 하겠다.

This study was done to analyze the contents of nurses'' ethical decision-making in four of hypothetical dilemma cases using the Cameron''s Ethical Decision-Making Model of ''Value, Be, Do''. Sixteen nurses who work at ICU at present or worked before, participated from April 10 to May 10 in 2002. The participants were interviewed three times each and for 40 minutes at once, with a structured questionnaire at their working places and locker rooms. The data was analyzed by a procedure of qualitative content analysis into three categories ; what should I value, who should I be, what should I do. The results were as follows : 1) In consistency, most of subjects showed a unified voice in ''Value, Be, Do''. Exceptionally 8 subjects showed inconsistency such as 3 in active treatment to the incurable patients(case 1), 1 in treatment truth-telling to the terminally ill patients(case 2), 3 in conflict with uncooperative doctors(case 3), 3 in dying

patients and euthanasia(case 4). Only one subject showed inconsistency in 3 dilemma cases. 2) The key concepts were extracted case by case. Case 1 - (Value) meaninglessness of active treatment, humanitarian dignity, alleviating burden of

patient''s family, respect for patient and patient''s family, joint decision-making, professional duty/ (Be) advocator, mediator/ (Do) fidelity to job, withholding unnecessary life-saving, collaboration with medical team, advocate patient and patient''s family, mediation between patient''s family and doctor. Case 2 - (Value) patient''s right to know, providing patients with opportunity to prepare his/her death, respect for patient and patient''s family, respect patient''s right to self-determination/ (Be) advocator, mediator, collaborator/ (Do) truth-telling, support patient and patient''s family, mediation between patient''s family and doctor, providing hospice care, respect needs of patient''s family, facilitating communication between patient and his/her family. Case 3 - (Value) respect patient, respect life, collaboration, respect patient and patient''s family, professional duty/ (Be) collaborator, advocator, suggestion-maker/ (Do) fidelity to job, communicative negotiation, official communication channel, collaboration with medical team, involving patient in decision-making. Case 4 - (Value) legality, humanitarian dignity, veracity, professional duty, respect patient''s needs/ (Be) advocator, keeping law, mediator, collaborator/ (Do) refusal false documentation, collaboration with medical team, ignoring doctor''s malpractice, opposing active euthanasia, report to supervisor. 3) Closing the interview procedure, the subjects evaluated Cameron''s Model as it would help them build consistent value, carry right action, and cope to conflicts. On the basis of the results, it is recommended that nursing ethics should adopt the ethical decision-making model, and be applied to the curriculum of nursing colleges and continuing education program for clinical nurses.
