악교정 수술에 있어서 술후 예방적 항생제 투여 기간에 관한 연구

본 논문은 악교정 수술에 있어서 술후 예방적 항생제 사용의 효용성에 대하여 연구하였다. 술후 항생제 투여 기간에 따른 술후 창상 감염여부와 그 발생빈도 및 이차적인 술후 창상감염에 대한 감수성을 평가하기 위해 술후 회복시 환자의 증상 변화를 관찰하였다. 본 연구에서는 26명의 환자들을 무작위로 두 집단으로 나누었다. 모든 환자들에 세프피라마이드(Cefpiramide) 항생제 1.0g을 술전 및 수술직후에 한번씩 정맥내 투여하였으며 그중 13명의 대조군 환자들에 대해서는 추가하여 술후 3일까지 지속적으로 하루에 두 차례 같은 항생제를 투여하였다. 창상은 미국 질병관리국에서 제안된 창상 감염에 대한 평가 기준을 변형하여 정기적으로 평가하였다. 이와 더불어 술후 회복시 환자의 증상변화를 관찰하기 위하여 각군의 환자들에 대해 술후 회복정도 지표로서 7가지 증상별로 각 5점 단위 평가를 시행하였다. 술후 창상감염은 어느 집단에서도 발견되지 않았다. 악교정 수술후 환자의 증상은 술후 초기부터 회복되는 양상이 관찰되어 환자의 술후 창상 감염에 대한 감수성도 감소하는 것으로 판단되었다. 악교정 수술에 있어서는 술후 단기간의 예방적 항생제 요법이 추천된다.

This study evaluated the need and duration for antibiotic prophylaxis in orthognathic surgery. This included post-operative wound infection incidence without antibiotic usage after surgery and patient''s susceptibility for post-operative wound infection. The patients were distributed randomly between two groups. Each group received Cefpiramide 1.0g 30min before and immediately after operation. Control group received same antibiotics for 3 days after the operation continuously. The wounds were inspected on a regular schedule and evaluated for infection according to criteria. Patients'' perception of recovery after orthognathic surgery was assessed on 5-point scales for 7 symptoms. None of patient became infected. There was no significant difference in rates of infection between the two groups. Patients'' symptoms after orthognathic surgery became satisfactory from immediate post operative period. Therefore, short-term antibiotic prophylaxis is preferred for orthognathic surgical procedures.
