척수손상인의 사회적 재통합 설명모형

dc.description.abstract[한글] 본 연구는 척수손상인의 사회적 재통합을 설명하기 위해 관련 요인들을 규명하고, 요인들간의 경로와 영향력을 파악하여 인과관계를 규명함으로써 척수손상인의 사회적 재통합을 증진할 수 있는 간호중재의 근거를 제공하기 위한 공분산 구조분석 연구이다. 가설적 모형은 척수손상인의 사회적 재통합에 영향을 미치는 개인적·환경적 요인 중 자아 존중감, 지각된 사회적 불이익, 신체기능, 지각된 가족지지 및 지각된 정보지지의 5개의 외생변수와 척수손상에 대한 개인의 인지적 평가인 지각된 스트레스와 이에 대한 대처기전인 정서중심 대처와 문제중심 대처, 그리고 사회적 재통합의 4개의 내생변수들로 구성되었다. 연구의 자료수집은 2001년 8월 20일부터 11월24일까지 자가보고식 질문지를 이용하여 이루어졌으며 총 145개의 자료가 분석되었다. 가설적 모형검증과 수정모형의 검증은 LISREL 8.30 WIN program을 이용하였으며, 가설적 모형의 검증결과 수정이 필요하였고 수정과정 을 거쳐 채택된 척수손상인의 사회적 재통합 설명모형은 수집된 자료와 잘 부합하였다. 본 연구결과에 따르면, 척수손상인의 사회적 재통합에 직접영향을 미치는 변수는 정서중심 대처, 지각된 가족지지, 지각된 정보지지, 지각된 스트레스 및 지각된 사회적 불이익이었으며 이 중 가장 영향력이 큰 변수는 정서중심 대처였다. 사회적 재통합이 이상의 예측변수들에 의해 설명되는 정도는 65%이었다. 또한 자아 존중감은 지각된 스트레스와 정서중심 대처를 경유하여 사회적 재통합에 영향을 미치는 주요변수임이 확인되었다. 연구결과에 비추어 볼 때 척수손상인의 사회적 재통합을 증진시키기 위해서는 본 연구에서 밝혀진 사회적 재통합에 영향을 미치는 요인들, 특히 정서중심 대처를 증진하는 중재에 초점을 맞출 것을 제안한다. 특히 자아 존중감은 지각된 스트레스를 감소시키고 정서중심 대처를 증진함으로써, 그리고 지각된 가족지지는 정서중심 대처를 증진함으로써 결과적으로 사회적 재통합을 증진하는 주요변수임이 확인되었고, 지각된 사회적 불이익이 사회적 재통합을 저해하는 주요 변수로 밝혀졌으므로 척수손상인의 사회적 재통합 증진을 위한 간호중재는 개인뿐만 아니라 가족과 사회적 차원까지 고려하여야 할 것이다. -------------------- 핵심어 : 척수손상인, 사회적 재통합, 설명모형, 정서중심 대처 [영문] The main purpose of this study was to identify factors related to social reintegration and to develop an explanatory model for social reintegration of persons with spinal cord injury. A hypothetical model which was constructed based on previous studies and Stress-Coping Model of Lazarus and Folkman was built around nine variables. Exogenous variables included in this model were self esteem, perceived social barriers, physical function, perceived family support and perceived informational support. Endogenous variables were perceived stress, emotion-focused coping, problem-focused coping and social reintegration. Empirical data for testing the hypothetical model were collected from 145 persons with spinal cord injury using self-administerd questionnaires between August 20 and November 24, 2001. For inclusion in this sample, all subjects were required to have sustained traumatic spinal cord injury for more than 6 months, reside in the community for more than 3 months after discharge from the hospital, and be at least 20 years old. SPSS 10.0 WIN Program and LISREL 8.3 WIN Program were used for data analysis. The result of this study showed that the emotion-focused coping was the strongest predictor for social reintegration in this sample. Perceived family support, perceived informational support, perceived stress and perceived social barriers were also significantly related to social reintegration. Overall, the model explained 65% of the variance in social reintegration. The results suggest that nursing interventions to promote social reintegration of the persons with spinal cord injury need to focus on the emotion-focused coping of each individual. And interventions at various levels, including family and community as well as individual with spinal cord injury are needed to promote social reintegration of this sample. Also, further studies are needed to develop more-refined explanatory model for social reintegration of persons with spinal cord injury and to investigate the effect of nursing interventions promoting social reintegration of this sample.-
