상악 제 1대구치 원심견인이 제 2대구치 맹출에 미치는 영향

성장기 아동에서 headgear를 사용하여 상악 제 1대구치의 후방견인을 시행한 후 상악 제 2대구치가 정상맹출군과 이소맹출군간의 치료 전, 후 유의성 있는 차이를 나타내는

요소들을 후향적 연구를 통해 조사하고자 하였다.

연세대학교 영동세브란스병원 치과 교정과에 내원한 혼합치열 말기 혹은 영구치열 초기 환자들 중 골격성 II급 부정교합으로 진단받고, headgear를 사용하여 상악 제 1대구치를 원심이동시켜 치료한 120명을 연구대상으로 하였다. 이들의 치료 후 석고모형에서 상악 제 2대구치의 맹출양상에 따라 정상맹출군과 이소맹출군으로 나누고 치료 전, 후 lateral cephalogram을 계측, 분석하여 다음과 같은 결론을 얻었다.

1. 치료 전 이소맹출군의 제 1, 2대구치 원심 풍융부를 연결한 선과 수평기준 선이 이루는 각(HRL/M6-M7)이 컸으며(남자: P<0.01, 여자: P<0.05) 구개평 면은 정상맹출군에 비해 수평기준선(HRL/PL)과 더 큰 각을 이루었다 .(P<0.05)

2. 치료 후 이소맹출군의 제 1, 2대구치 원심 풍융부를 연결한 선과 수평기준선이 이루는 각(HRL/M6-M7)이 컸으며(P<0.001) 구개평면은 정상맹출군에 비해 수평기준선(HRL/PL)과 더 큰 각을 이루었다.(P<0.05)

3. 수직기준선에 대한 제 1대구치의 수평적 위치(VRL-M6, PTV-M6)는 치료전과 치료 후 두 군간에 차이가 없었다.(P>0.01)

Distalization of the upper first molar is popularly used method in borderline cases of extraction and non-extraction in recent days. But this can cause ectopic eruption of the upper second molar because it increases posterior discrepancy.

120 skeletal Class II patients, who had been treated with headgear for distalization of the upper first molar, were selected and subdivided into good eruption group and poor eruption group according to the eruption state of upper second molar after treatment. Lateral cephalometric radiographs were taken before and after treatment and statistically analyzed.

The results were as follows:

1. The eruption angle (HRL/M6-M7) and steepness of the palatal plane of ectopic eruption group was greater than that of normal eruption group before treatment.

2. The eruption angle (HRL/M6-M7) and steepness of the palatal plane of ectopic eruption group was greater than that of normal eruption group after treatment.

3. Before and after treatment, there was no significant difference in horizontal measurements of upper first molar (VRL-M6, PTV-M6) on both group.
