간호과정 임상적용을 위한 지식관리시스템 개발 연구

Abstract

[한글]간호는 자연과학, 행동과학 및 인문과학을 바탕으로 통합된 지식들을 기본으로 하여 전인적 접근을 추구하고 있다. 전인적 간호를 제공함에 있어서 간호사는 대상자에 대한 자료를 수집하여 문제를 확인하고 문제해결접근법을 선택하여 수행한 후 대상자에게 나타나는 결과로 이를 평가하는 문제해결과정을 따르고 있다. 문제해결 방법의 기초이론에 근거를 둔 간호과정은 임상간호의 의사결정과정에서 중추적인 역할을 담당하고 있으며, 대부분의 간호교육기관의 교육과정에서 중심틀로 사용되고 있다.

이렇듯 간호과정의 중요성이 강조되고 있으나 간호과정의 임상적용은 아직 일반화되지 못하고 있으며, 이를 해결하기 위한 방안으로 간호과정을 정보시스템으로 구축하려는 연구들이 지속되고 있으나 그 활용도는 낮은 실정이다.

본 연구에서는 지식관리시스템을 적용하여 환자특성, 관련요인, NANDA 간호진단, 간호중재분류, 간호결과분류 및 간호전문가의 지식 등을 이용한 간호과정 지식관리시스템을 개발하였으며, 시스템의 임상적용 타당성을 확인하기 위하여 시스템 접속빈도와 사용자 만족도를 조사하였고, 사용자가 시스템을 사용하면서 입력한 자료를 분석하여 시스템의 실용성을 평가함으로써 임상에서의 간호과정 적용과 간호의 질 향상을 도모하고자 하였다.

지식관리시스템의 개발에 관한 문헌과 소프트웨어 공학의 전통적인 수명주기를 근거로 하여 간호과정 지식관리시스템을 구축하였다.

지식요건 상세화 단계에서는 지식분류체계, 지식지도, 지식베이스 설계 와 간호진단·간호중재·간호결과 및 간호진단 전문가 지식 등에 대한 컨텐츠를 개발하였다. 간호진단 전문가 지식 추출을 위한 자료수집을 위해 2000년 6월 15일부터 8월 14일까지 2달간 조사지를 이용하여 실제 환자의 임상사례를 수집하였으며, 수집한 임상사례를 패턴화하여 간호진단 규칙을 도출하였다.

기술구조확정 단계에서는 Windows NT2000 운영체제하에서 MS-SQL로 지식저장소를 구축하고 A.S.P.로 Web 프로그램을 구현하여 Web Server, Compaq Proliant 5500으로 사용자에게 시스템을 제공하는 기술구조를 확정하였다.

지식베이스 개발 단계에서는 지식베이스 관리시스템을 개발하였으며, UI 개발 단계에서는 사용자를 위한 환경을 구축하였다.

구축된 간호과정 지식관리시스템이 연구목적에 부합되는지를 확인하기 위하여 2000년 10월 29일부터 11월 11일까지 2주동안 가상의 자료로 시스템 구축환경하에서 시스템을 검증하였으며, 11월 12일부터 11월 16일까지는 인터넷상에서 검증하였다.

개발된 간호과정 지식관리시스템의 임상적용 타당성을 평가하기 위해 본 시스템을 홈페이지에 개설하고(http://www.nursingprocess.com) 2000년 11월 17일부터 12월 7일까지 3주 동안 Web상에서 설문조사를 하여 병원에 근무하는 간호사, 책임간호사, 수간호사로써 본 시스템을 사용한 51명의 사용자 만족도를 분석하였다. 5점 척도의 Likert scale에 의한 평가에서 정확성/신뢰성이 3.82로 가장 높았고 그 다음으로 웹사이트(3.78), 편의성(3.74), 기준문항(3.72), 능률성(3.61), 시스템 개선(3.32) 및 형식/충분성(3.23) 순이었다. 사용자들이 시스템 사용에 대해 만족하고 있음을 알 수 있었고, 이를 통해 간호과정 지식관리시스템의 유용성을 확인할 수 있었다.

또한 사용자가 시스템을 사용하면서 저장한 110예의 임상사례에 대한 간호진단·간호중재·간호결과 자료를 분석한 결과 간호진단 규칙이 있는 34개 간호진단에서 총 416건의 자료 중 단 6건만이 간호진단 규칙에 해당되지 않는 자료로 나타나는 등 간호과정 지식관리시스템이 간호과정에 대한 지식관리를 잘 수행하고 있음을 확인할 수 있었다.

이상의 연구결과를 시스템에 반영하여 시스템을 수정, 보완한 후 최종적인 간호과정 지식관리시스템을 제시하였다.

[영문]The nursing process is the process of decision-making of clinical nursing and is being used as the central framework in the curriculum of the majority of nursing education institutions. Although the importance of nursing process is being emphasized, teaching methods of the clinical application of nursing process are very limited. There are some researches attempt to develop computerized programs of nursing process, but they fail to receive wide acceptance.

In this study, in order to increase the clinical application of nursing process, the Knowledge Management System of Nursing Process, which included the patient`s characteristics, 'related to' factors, NANDA nursing diagnosis, Nursing Intervention Classification(NIC), Nursing Outcome Classification(NOC), and the nursing expert's knowledge is developed. In order to ascertain the validity of clinical application of the system, frequency of the system access and the end-user satisfaction were analyzed. Finally, evaluation of knowledge management process of the system user was done to improve quality of nursing.

The development of Knowledge Management System followed the existing steps of softwear engineering.

In the first stage, contents in relation to knowledge classification system, knowledge map, knowledge-base design, nursing diagnosis, nursing intervention, nursing outcome and the expert's knowledge of nursing diagnosis are developed. To collect data for a spot check of nursing diagnosis expertise, actual patients’data at the investigation site from June 15 through August 14, 2000 were collected. By classifying collected cases by the pattern and adding the nursing expert's knowledge, rules of nursing diagnosis were identified.

In the stage of deciding upon technical structure, I have decided to build up knowledge repository with MS-SQL and under the operating system of Windows NT 2000. I have decided upon technical structure to afford the system to the end-users with Web Server, Compaq Proliant 5500 by making web program of A.S.P.

In the stage of developing knowledge base, Knowledge-base Management System was developed. And in the stage of developing User Interface, the environment for the end-users was established.

In order to examine the validity of clinical application of the Knowledge Management System of Nursing Process I have opened a homepage at http://www.nursingprocess.com and installed the developed system. The end-user satisfaction analysis was done using 5 point likert scale was done with the responses of 51 users of this system. The results showed the accuracy/trustworthiness of the system the highest average score which is 3.82. Website (3.78), user friendly (3.78), standard inquiry item (3.72), and efficiency (3.61) showed high satisfaction scores.

Finally, analysis of the data of the nursing diagnosis, nursing intervention and nursing outcome of 110 cases used by the system users, there were only 6 cases which violate the rules of nursing diagnosis. This indicates that the developed Knowledge Management System of Nursing Process have meet the teaching objectives.
