Abstract
[한글]병원감염에 관한 대부분의 연구들은 병원감염의 발생율, 일반적인 위험인자, 유형,

그리고 원인균의 분포와 종류에 관련된 것들이었다. 최근에 이르러 국내에서도 체계적

이고 객관적인 연구를 통하여 병원감염의 발생율, 감염의 유형, 원인균의 종류와 분포,

그리고 진료과에 따른 병원감염의 특징을 알 수 있었다. 뇌졸중 환자에서 병원감염 동

반시 신경학적 결손을 악화시키는 경우가 많았고, 감염이 비교적 흔한 뇌졸중의 합병증

으로 알려져 있으나 뇌졸중 환자에서 발병한 병원감염에 대한 연구는 드물었다. 그러므

로 급성기 뇌졸중에서 발병한 병원감염의 연구가 필요하다고 판단되어 1998년 10월부터

1999년 6월까지 연세대학교 원주의과대학 원주기독병원에 급성 뇌졸중으로 입원치료했

던 환자에서 발병한 병원감염의 연구를 통하여 아래와 같은 결과를 얻었다.



1. 급성기 뇌졸중 환자에서 가장 흔한 병원감염은 폐렴이었다.

2. 뇌경색 환자보다 뇌출혈 환자에서 병원감염의 발생 빈도가 높았다.

3. 연령, 성별, 당뇨의 동반 및 뇌졸중의 기왕력은 대조군과 병원감염군 사이에 통계학

적으로 유의한 차이가 없었다.

4. 각 병원감염의 종류별 원인균의 분포와 빈도는 다른 연구의 결과와 유사하였다.

5. 대상환자에서 대조군과 감염군 사이에는 Glasgow Coma Scale과 Scandinavian

Stroke Scale이 통계학적으로 유의한 차이가 있었다.

6. 기관내 삽관 후 폐렴이 발생한 군과 미발생 군 사이에 Glasgow Coma Scale과

Scandinavian Stroke Scale은 통계학적으로 유의한 차이가 없었다.

7. 도뇨 후 요로 감염이 발생한 군과 미발생 군 사이에 지속적 도뇨의 기간은 통계학적

으로 유의한 차이가 있었다.



결론적으로 신경학적 결손이 심한 뇌졸중 환자에서 병원감염의 발생이 많았고, 심한

신경학적 결손이 급성기 뇌졸중에서 위험인자라는 것을 알 수 있었으며, 향후 전향적인

연구를 통하여 보다 포괄적인 뇌졸중에 관련된 병원감염의 연구가 필요하다고 사료된

다.



[영문]There has been much researches on nosocomial infections. Most of it involved

the incidence rate, risk factors, types of the nosocomial infections, and classes and

distribution of the causative organisms. Also, we can know the incidence rate and

types of the nosocomial infection, classes and distributions of the causative

organisms, and characteristics of the infection among the departments from recent

systematic and objective research in Korea. But researches about nosocomial

infections, especially associated with stroke have been rare, despite factors

aggravating neurologic deficits and common medical complications of acute stroke.

We analyzed nosocomial infections in stroke patients, who were admitted to Wonju

Christian Hospital, Wonju College of Medicine, Yonsei University from October 1998

to June 1999, and the results are as follows:



1. Pneumonia was the most frequent infection in patients with acute stroke.

2. The infection was more frequent in patients with hemorrhage group than those

with infarction group.

3. Age, sex, diabetes mellitus, and previous history of stroke showed no significant

statistical difference between control group and infection group.

4. The distribution and frequency of the causative organisms in each infection was

similar to the reports of previous research.

5. Glasgow Coma Scale and Scandinavian Stroke Scale showed significant statistical

difference between the control group and infection group.

6. There was no significant statistical difference in Glasgow Coma Scale and

Scandinavian Stroke Scale between the control group and pneumonia group

after endotracheal intubation.

7. There was significant statistical difference in the period of indwelling urinary

catheterization between the control group and urinary infection group after

urinary catheterization.



There was more frequent development of the nosocomial infection in patients

with severe neurologic deficits. Therefore, we deduced that severe neurologic deficits

were a specific risk factor for developing nosocomial infection in patients with acute

stroke. We suppose future research about nosocomial infection is needed by means

of prospective study.
