일 종합병원의 지역사회 보건간호사업에 대한 지역사회 간호써비스 수혜자 및 의뢰자의 태도 조사 연구

사회의 변화는 건강문제의 변화를 가져왔고, 건강 문제의 변화는 건강관리사업의 변화를 요구하게 되었다. 따라서 입원환자를 중심으로 하던 병원치료사업은, 가족 및 지역사회를 대상으로 한 건강관리 사업을 요구하게 되었다.

이런 사회적 요구를 해결하기 위한 한 방법으로, W병원에서는 1974년 9월부터 병원 내에 지역사회 보건 간호과를 설치하여 입원환자들을 가능한 한 조기 퇴원시키며, 입원 전후의 환자 및 가족에게 지역사회보건간호사업을 실시하여 왔다.

본 연구는 이 사업에 대한 평가의 일환으로, 사업수혜자 및 사업 의뢰자의 이 사업에 대한 태도를 조사한 것이다.

본 연구의 구체적 목적은, 사업수혜자 및 의뢰자의

1. 조기 퇴원에 대한 태도

2. 가정에서의 환자치료 가능성에 대한 태도

3. 보건간호원의 15가지 선택된 가정간호진료활동에 대한 태도

4. 보건간호원의 15가지 선택된 가정간호진료활동에 대한 수가 파악이다.

연구대상은

1. 수혜자; 1974년 9월부터 1975년 12월까지 W병원 지역 사회 보건 간호과에 의뢰된 환자 중 면접이 가능한 환자 가정을 전수조사 하였다.

의뢰자Ⅰ와 Ⅱ는 가정간호진료가 필요한 환자를 지역 사회 보건 간호과에 의뢰한 간호원, 의사를 말하며 사업기간 동안 근무 하였고, 자료수집기간 동안 근무하고 있었던 자이다.

2. 의뢰자 Ⅰ; 총 81명의 간호원중 의사와 같은 수인 37명을 무작위 추출하여 표본조사 하였으며, 32명이 회수 되었다.

3. 의뢰자 Ⅱ; 총 37명(인턴제외)중 질문지 작성이 가능한 24명만이 회수 되었다. 자료수집방법은; 수혜자는 직접 방문하여 면접지를 통해 면접하였고 의뢰자 Ⅰ과 Ⅱ는 질문지를 사용하였다.

연구결과 분석은 배분율과 X**2 test를 사용하였다.

연구 결과는

1. 조기퇴원에 대한 태도; 조기퇴원에 대해 지역사회간호써비스 수혜자와 의뢰자가 모두 85%이상 찬성하였다.

2. 가정에서의 환자치료 가능성에 대한 태도; 지역사회간호써비스 수혜자 및 의뢰자의 75%이상이 가정에서도 환자의 치료가 가능하다고 하였다.

3. 가정진료담당자에 대한 태도; 지역사회간호써비스 수혜자 및 의뢰자가, 가정에서의 환자진료는 "간호원"이 담당해야 한다는 반응이 가장 많았다.

4. 보건간호원의 15가지 선택된 가정간호진료활동에 대한 태도; 지역사회간호써비스 수혜자나 의뢰자가, 보건간호원이 가정에서 간호진료활동을 하는데 대해 과반수이상 찬성하였다.

5. 보건간호원의 15가지 선택된 가정간호진료활동에 대한 수가: 보건간호원이 가정에서 15가지 선택된 간호진료활동을 하는데 대해 지역사회 간호 써비스 의뢰자의 85%이상이 진료비를 지불해야 한다는 태도를 보였다.

무료로 제공되어야 한다는 반응은, 수혜자는 12.8%, 의뢰자 Ⅰ, Ⅱ는 5%뿐이었다.

결론적으로 W병원의 지역사회보건간호사업에 대한 수혜자 및 의뢰자의 태도는 매우 긍정적일 뿐 만 아니라, 가정에서도 환자진료자는 간호원이어야 하며, 선택된 15가지 간호진료활동은 가정에서도 시행 가능하고 수용되는 것임을 말해주고 있다. 아울러 이 활동들의 유료화가 가능함을 증명하고 있어, 앞으로 병원을 중심으로 한 지역사회보건간호사업의 필요성과 가능성을 제시해 주고 있다.

The change of society has brought about various health problems which have become to demand comprehensive health services not only for individuals but also for families and communities as the unit.

To meet the societal needs W Hospital in a rural Korea established a Department of Community Health Nursing Services and provided nursing services to patients under a early discharge program.

This study is to evalute the community health nursing services carried out the department in general, and has following specific objectives:

1. To learn attitude of the consumers and the "referers" toward early discharge program.

2. To find out attitude toward home care and cure services.

3. To find out the consumers and the "Referers" attitude toward the 15 selected home care and cure activities by public health nurse.

4. To investigate possibility of charging fee for the 15 selected home care and cure activities by public health nurse.

Three different study population were chosen: namely the consumer, the "referer" Ⅰ and Ⅱ.

Excluding families moved out and not able to be contacted, the total families (77) referred tot he department during the study period of September 1974-december 1975 are defined as the consumer.

Thirty seven nurses among 81 nurses who have been working in W Hospital since the inception of the Community Health Nursing Service Program were ramdomly selected. Thirty two nurses were defined as the "referer" Ⅰ, because 5 questionnaires were

not able to be collected.

Twenty four doctors out of 37 who have been working since the Community Health Nursing Service Program started and who were able to contact were called as the "referer" Ⅱ.

Data collection method employed for the consumers was direct interviewing with prepared interview schedules, and for the "referers" questionnaire method was utilized.

X**2 test and 100-percentage were employed in analyzing the data.

The study findings are follows:

1. Attitude toward early discharge: Above 85% of the consumers and the "referers" approved the early discharge program.

2. Attitude toward the home care and cure services: Above 75% of the consumers and the "referers" showed positive attitude toward the home care and cure services by public health nurses.

3. Attitude toward the service providers for home care and cure services: Both the consumers and the "referers" indicate the "NURSE" are the most appropriate service providers among existing health personnel for care and cure services.

4. Attitude toward 15 selected home care and curative activities by public health nurses: On the average more than 50% of the consumers and the "referers" expressed an affirmative attitude to perform 15 selected home care and cure services by

public health nurses.

5. Attitude toward charging fee for 15 selected home care and curative activities by public health nurses: Above 85% of the consumers thought that they would pay for the services. Only 12.8% of the consumers and 5% of the 'referers" want to have the

services for nothing.

Based upon the findings of this study: That is, the home care and cure services to be given by public health nurse were positively responded by the consumers and the professionals, one can conclude that the services provided by the Department of

Community Health Nursing Service of W Hospital have been well accepted, and that early discharge program of hospital is believed to be a means to lessen the expenses of patients from hospitalization and to promote recovery from illness. It is recommended that hospital centered cure services should be reconsidered and scrutinized to meet community health needs.
