태아 모체간 수혈현상에 관한 연구

임신중에 태아 혈액이 태반을 통하여 모체로 수혈되는 태아 모체간 수혈현상은 전 산모

의 약 50퍼센트에서 발생한다. 이러한 수혈현상은 신생아빈혈의 가장 흔한 원인의 하나이

며, 임신중 만성적으로 태아혈액이 모체로 수혈되면 태아는 이러한 상황에 혈액학적으로

적응하여 출생시에는 빈혈증상이 잘 발견되지 않는 경우가 대부분이다. Rh항원을 가진 태

아적혈구가 Rh음성인 모체로 수혈되면 Rh동종면역을 유발하여 태아적아구증을 일으킨다.

태아모체간 수혈현상은 임신 4주부터 일어나지만 대부분은 태반막이 손상을 받는 진통 및

분만중에 일어난다. 저자는 태아혈색소는 성인 혈색소와 달리 산성배지에서 적혈구로부

터 씻겨나가지 않는 원리를 이용한 Nierhaus 산성용출법을 사용하여 한국인 산모 117명을

대상으로 태아모체간 수혈현상을 조사하여 다음의 결론을 얻었다.

1. 분만후 태아모체간 수혈현상의 빈도는 49퍼센트였으며 수혈양은 0.4㎖미만이 14퍼센

트, 0.4∼4.0㎖가 33퍼센트, 4.0㎖가 33퍼센트, 4.0㎖이상이 3퍼센트였다.

2. 동일한 산모를 대상으로 진통중과 분만후의 태아모체간 수혈현상의 빈도를 비교하였

을 때 통계학적 의의가 없었으며 이 결과로 미루어보아 대부분의 태아모체간 수혈현상은

진통시작과 함께 일어나는 것이라고 추정된다.

3. 임신중 합병증이 태아모체간 수혈현상에 미치는 영향을 조사하였으나 통계학적 의의

는 없었다.

4. 진통기간과 태아모체간 수혈현상과는 연관관계가 없었다.

5. 태아모체간 수혈현상의 빈도는 손으로 태반을 박리한 경우가 태반이 자연박리된 경

우보다 높았다.

6. 태아모체간 수혈현상의 빈도는 자연분만과 자궁수축제를 사용한 유도분만의 사이에

차이가 없었다.

THE STUDY OF FETOMATERNAL TRANSFUSION IN KOREA

Blood samples from mothers during labor and after delivery were tested for the

occurrence of fetomaternal transfusion using Nierhaus acid elution technique.

Fetomaternal trasfusion had occured ½ of the mothers after delivery, ⅔ of the

mothers during labor.

The fetomaternal trasfusions after delivery were most often less than 4.0㎖ and

only few percent had fetomaternal transfusions more than 4.0㎖.

Complicated pregnancy, duration of labor, induction of labor with oxytocics did

not increased the incidence of fetomaternal transfusion. Manual removal of placenta

increased the incidence of fetomaternal transfusion.
