아토피성 피부염에서의 IgE에 관한 연구

1966년 Ishizaka등이 인간면역글로블린 중에서 새로운 γE-globulin의 존재를 발견하고 이것이 reaginic antibody라고 보고하였다. 같은 시기에 Johansson과 Bennich는 atypical myeloma protein에서 IgND라는 면역글로블린을 분리해 내었고(1967) 이것이 γE-globul

in과 동일함이 밝혀져 1968년 WHO World Conference에서 IgE라고 명명되었다.

아토피성 피부염에서 IgE치가 증가되어 나타난다는 보고는 여러 학자들에 의해 이야기되었으며 IgE치의 변화가 임상증상의 경중도와 일치되어 나타난다고 하였다( Rajka, 1961; Gurevitch. 1973 ) 또한 기생충 감염시에도 IgE가 증가되고 (Hogarth-Scoot, Johansson

및 Bennich, 1969)혈청 IgE가 상승할 때는 혈중 호산구의 숫자가 증가한다고 하였다.

본 논문에서는 1978년 10월부터 1980년 3월까지 만 18개월동안 연세외대 부곡 세브란스병원 피부과에서 아토피성피부염으로 확진할 수 있었던 환자 100명을 대상으로 혈청 IgE치, 혈중 호산구의 백분율 및 기생충검사등을 시행하여 조사 관찰한 결과

1. 연령별로 IgE치를 비교하여 볼 때 소아층보다 높게 나타났다 (성인층의 IgE평균치 391.5 unit/㎖, 소아층 251.4 unit/㎖).

2. 남녀의 IgE치를 비교하여 볼 때 남자쪽에서 높게 나타났다(남자의 IgE 평균치 324.5 unit/㎖, 여자 242.7 unit/㎖).

3. 호산구백분율과 IgE의 변화 사이에는 일관성 있는 연관을 맺을 수 없었다.

4.기생충감염이 동반된 환자에서의 혈청 IgE치는 높게 나타날 것으로 예상되었으나 5예에서 구한 평균치가 116.2 unit/㎖로 전체환자의 평균치인 293. unit/㎖보다도 낮게 나타났다. 그러나 이 경우는 그 예수가 적어 정확한 판정이 힘들다 하겠다.

5. 알러지성 천식이 동반된 환자는 2명 있었으며 2명 모두 혈청 IgE치가 1,000 unit/㎖ 이상으로 높게 나타났다.

6. 임상증상의 경중도와 IgE 변화와의 관계를 살펴보면 피부병변이 아주 심한 경우에는( 100예중 4예 )혈청 IgE도 현저하게 증가하는 양상을 보였다( 4예 모두 1,000 unit/㎖).

[영문]

In 1967 Ishizaka et al., Johansson and Bennich independently described the reagins or skin sensitizing antibodies, as belonging to a new class of immunoglobulins, IgE.

Juhlin et al. (1969) found elevated IgE values in patients with atopic dermatitis and Ogawa et al. (1971) reported that serum IgE values were closely correlated with severity of the skin involvement in atopic dermatitis.

Increased serum levels of IgE Have been reported in patients infested with intestinal parasites (Hogarth-Scoot, Johansson, and Bennich, 1969) with the increase in eosinophil count in blood.

Serum IgE values were determined on 100 patients with atopic dermatitis in the Department of Dermatology at Severance Hospital, Yonsei University, College of Medicine, from Oct. 1, 1978 to Mar. 31, 1980.

Mean IgE value of patients with atopic dermatitis were increased to 293.4 unit/㎖ which is significantly higher than reference value.

1. Mean IgE value in adults (391.5 unit/㎖, 3o cases16 rears old) was significantly higher than in children(251.4 unit/㎖, 70 cases 16 7ears odd).

2. Male patients showed higher IgE value (324.5 unit/㎖, 62 cases) than female patients (242.7 unit/㎖, 38 cases).

3. No significant increase was found in % of eosinophil in complete blood count (mean :4.6 %).

4. Patients combined with parasitic infestation showed low IgE value (mean:116.2 unit/㎖) but the size of cases was so small (5 cases) that the data can not be considered reliable for exact evaluation.

5. Number of patients combined with allergic asthma is two and each showed high IgE values exceeding to 1000 unit/㎖.

6. IgE values were roughly correlated with severity of the dermatitis. Four cases marking high IgE values exceeding to 1000 unit/㎖ were included in clinically severe group.
