방어 양상과 정신과적 증상형태에 관한 관련성 연구

자아의 여러 가지 기능 중 방어 기전에 대한 연구는 그리 많지 않은 편이다. Anna Freud에 의해 현재 사용되고 있는 방어 기전의 개념들이 확립되었다고 볼 수 있다. Anna Freud는 방어 기전의 발달은 자아의 발달과 관계가 있으며 자아 발달 수준에 따라 방어기준을 분류할 수 있다고 하였다. 그 이후 여러 연구자들에 의해 방어기전을 자아기능 수준, 혹은 자아의 성숙도에 따른 분류모형들이 제시되었다. 저자는 본 연구에서 방어양상설문지를 적용하여 방어기전들이 1) 자아의 발달 수준에 따라 분류될 수 있는 지를 보았으며, 2) 특정 방어기전들(즉 방어 양상)을 습관적으로 사용하는 집단이 있는지, 만일 사용하고

있다면 그들의 방어 양상 형태가 무엇인지, 3) 특정 방어기전들을 사용하고 있는 서로 다른 군들 사이에 정신과적 증상의 차이가 있는 지를 보았다.

연구 결과를 요약하면 다음과 같다.

1. 방어 양상은 7가지로 분류될 수 있었으며 이러한 방어 양상들은 순위적 개념에 따라 분류될 수 있음을 시사했다.

2. 연구 대상군의 약 반 이상(51.6%)은 특정 방어양상을 사용하지는 않았으며, 나머지는 신경증적 방어 양상과 성숙한 방어 양상이 혼합되어 있는 군, 주로 신경증적 방어 양상이 두드러진 군, 비교적 성숙한 방어 양상을 사용하는 군으로 분류되었다.

3. 각 군 사이에 SCL-90-R을 이용한 정신증상의 차이는 특정 방어 양상을 사용하지 않는 군과 신경증적 방어 양상이 두드러진 군에서 의미있는 차이가 있었으며 전자가 가장 낮은 점수를 후자가 가장 높은 점수를 나타냈다.

This study was based on those studies suggesting that defense mechanisms could be hierarchically classified according to ego functioning level or ego maturity. The author applied Defense Style Questionnaire(DSQ) to 237 subjects, who were consisted

of 147 patients and 90 normal control. DSQ was designed by Bond et al. to measured objectively the habitual defensive behaviors which are considered to appear if ones habitually use the certain defenses.

By the factor analysis 7 defense styles were identified. The result strongly suggested that defense styles could be hierarchically classified according to ego maturity. Second, the author tried to identify the clusters of subjects who might

made used of specific or specific combination of defense styles. By cluster analysis four clusters were extracted. First cluster did not seem to use specific defense style exclusively. Second cluster had the characteristic of simultaneous use of neurotic and mature defense styles. Third cluster seemed to have tendency

for habitual use of neurotic defense style. Fourth cluster had relatively strong tendency for use of mature defense style.

The mean symptom scores obtained by applying SCL-90-R were highest among third cluster and lowest among first cluster. But the author could not conclude that specific symptom patterns were related to the use of specific defense styles.
