Rigiscan을 이용한 야간음경발기와 시청각자극 음경발기의 비교관찰

음경발기는 성적자극발기와 야간음경발기로 구분지을 수 있다. 그러나 정확한 발기기전의 차이는 아직 밝혀져 있지 않다. 야간음경발기를 평가하기 위하여 야간음경발기시 팽창도와 강직도를 동시에 측정할 수 있는 Rigiscan을 사용하였고, 성적자극발기의 측정은 1988년 최형기와 김철수가 고안한 시청각자극 음경발기촬영술과 그의 분류방법을 이용하였다.

정상 성적자극 음경발기를 보인 16례에서는 야간음경발기 검사와의 일치율이 56%였고, 비정상 성적자극 음경발기를 보인 35례에서는 일치율이 51%였다. 이들중 발기불능형 (ⅡA) 및 지연형 (ⅡC)은 일치율이 각각 80% 및 78%로 비교적 높았으나 불안정형(ⅡB)의 경우는 33%로서 일치율이 현저히 낮았다.

이 결과를 종합하여 보면 시청각자극 음경발기촬영술이 일차적 예검법으로 이용될 수 있으나 결정적인 정보제공을 위하여서는 Rigiscan을 사용한 야간음경발기측정이 필수적이다.

Penile erection is either sexual stimulating erection or nocturnal erection. The difference of precise mechanism between the two types is not clarified yet. In order to evaluate nocturnal election, the Rigiscan was used to record nocturnal

penile tumescence and rigidity simultaneously. And sexual stimulating erection was interoduced by Audiovisual Stimulating Penogram which was introduced by Choi and Kim at 1988.

The following results were obtained. The compatibility ratio of 16 normal sexual stimulating erection to nocturnal erection test was 56%, while that of 35 cases of abnormal sexual stimulating erection was 51%. The compatibility ratios of the groupⅡ A and groupⅡ C were 80% and 78%, respectively, revealing high percentage of concordance between the two types of erection. On the other hand, the compatibility ratio of groupⅡ B was low(33%).

From the results presented here, it can be concluded that the AVS-penogram is useful for the screening test and to obtain more useful information, evaluation of nocturnal erection using Rigiscan is necessary.
