HBV DNA내 Pre-S(2)gene에 의하여 바이러스 외막(envelope)의 Pre-S(2)Ag이 만들어지며, 이 항원에 의하여 anti-Pre-S(2)의 형성이 유도된다. 이 Pre-S(2)Ag은 HBV DNA와 HBeAg과 함께 급성간염의 초기의 혈액에 나타나며 HBeAg보다 먼저 소실된다고 알려져 있다. 또
HBV DNA의 노도가 Pre-S(2)Ag의 농도의 변화와 상관관계가 있다는 보고가 있다.
연구자는 1988년 4월부터 9월까지 영동세브란스병원 외래 및 입원 환자중 HBsAg 양성인 급성간염, 만성간염, 간경변증, 무증상 보균자로 내원한 총 73명을 대상으로 Pre-S(2)Ag, anti-Pre-S(2), HBsAg, Anti-HBs, anti-HBe, HBV DNA를 검사하여 다음과 같은 결과를 얻었다.
3. Pre-S(2)Ag은 HBeAg 양성군에서 92.3%, anti-HBe 양성군에서는 60%를 차지했으며 두 군간에는 통계학적으로 유의한 차이가 있었다(P<0.05).
4. Pre-S(2)Ag의 농도는 HBeAg 양성인 군에서 1.047±1.007 O.D.이며, anti-HBe 양성군에서는 0.369±0.463 O.D. 이었으며 두 군간에는 통계학적으로 유의한 차이가 있었다(P<0.01).
5. Pre-S(2)Ag은 HBV DNA 양성군에서 92%를 차지했으며, HBV DNA 음성군에서 75%로 두 군간에는 통계적 유의성이 있었다(P<0.05).
6. 혈청내 HBV DNA 농도와 Pre-S(2)Ag의 농도와는 상관관계가 없었다.
7. HBV DNA 양성군과 음성군의 Pre-S(2)Ag의 평균치는 각각 1.021±0.93 O.D., 0.582±0.786으로 두 군간에는 통계학적인 유의한 차이 없었다.
이상의 결과로 Pre-S(2)Ag은 바이러스의 증식을 반영할수도 있으나 그 정도를 정확하게 나타내 주지는 못하며, 따라서 HBV DNA나 DNA polymerase에 비하여 임상적 가치가 적은 것으로 사료된다. 급성회복기에 Pre-S(2)Ag은 HBV DNA보다는 늦게, HBsAg보다 빨리 소실되며, HBeAg과의 관계는 확실하지 않았으며, 앞으로 Pre-S(2)Ag의 소실과 anti-Pre-S(2)의 출현이 만성간염으로의 이행 여부를 반영하느 예후인자로 유용한지에 관한 연구가 필요할 것으로 사료된다.

Pre-S(2) gene in HBV DNA genome encodes a peptide in HBV envelope which is called Pre-S(2)Ag, and this antigenic deter-minant stimulate the formation of anti-Pre-S(2). It has been reported that Pre-S(2)Ag appear in the early phase of acute viral hepatitis with HBV DNA, HBeAg, and there is a correlation between titer of HBV DNA and that of Pre-S(2)Ag.
We collected total 73 cases with a diagnosis of actue viral hepatitis, chronic hepatitis, liver cirrhosis, and asymptomatic carrier, from April to September, 1988 in order to check Pre-S(2), anti-Pre-S(2), HBsAg, anti-HBs, HbeAg, and anti-HBe.
The following results were obtained.
1. Pre-S(2)Ag was seen in 60 cases(82.1%) of 73 HBsAg positive patients.
2. Anti-Pre-S(2) was not detected in all cases.
3. Pre-S(2)Ag was seen in 92.3% of HBeAg positive patients and in 60% of anti-HBe positive patients. There is a statistic significance between two group(p<0.05).
4. The mean value of Pre-S(2)Ag was 1.047±1.007 O.D. in HBeAg(+) patients and 0.369±0.463 O.D. in anti-HBe(+) patients. There is a statistic significance between two groups(p<0.01).
5. Pre-S(2)Ag was seen in 92% of HBV DNA(+) patients and in HBV DNA(-) patients. There is a statistical significance between two groups(p<0.05).
6. There was no correlationship between titer of HBV DNA and Pre-S(2)AG.
7. The mean value of Pre-S(2)Ag was 1.201±0.93 O.D. in HBV DNA(-) patients. There is no statistical significance.
In conclusion, Pre-S(2)Ag seemed to reflect the replication of virus but not the degree of replication. Pre-S(2)Ag was suggested to have no more significance than HV DNA, but earlier than HBsAg. We need the ;ong-term follow-up to see the prognostic value with anti-Pre-S(2).
