상악골 전방견인 장치의 악정형 효과와 안전성에 관한 연구

상악골 전방견인 장치는 제 Ⅲ급 부정교합의 골격적인 관계, 치아 관계, 그리고 안모를 개선하는 효과가 있다고 하였으나 아직 장기적인 악정형 효과에 대해서는 뚜렷한 결론에 이르지 못하고 있다. 이에 저자는 상악골 전방견인 장치의 상악골에 대한 시상면에서의 단기간 및 장기간의 악정형 효과를 알아보고자 연세대학교 치과병원에 내원하여 골격성 제 Ⅲ급 부정교합으로 진단되어 초경을 시작하기 전에 상악골 전방견인 장치로 치료받았던 환자 20명을 평균 4.4년간 관찰하고, 연세대학교 치과병원에 내원하여 치료받은 환자 중에 골격적 부조화가 없는 25명을 대조군으로 초경 전후 평균 3.7년간 관찰하여 안정골구조물 중첩법(stable structural superimposition method)을 이용하고 자료를 통계적으로 분석한 결과 다음과 같은 결론을 얻었다

1. 상악골 전방견인 장치에 대한 반응은 비강저를 포함하는 상악골 전체가 전하방 이동하는 경우(M point와 A point가 같은 양 전방 이동)와 상악골 몸체는 이동하지 않은 채 비강저와 치조돌기 만이 전방이동(M point는 변화가 없고 A point 만이 전방 이동)하는 두 경우 사이에서 개인마다 다양하게 나타났다.

2. 치료군은 대조군에 비해 상악골의 전방성장 및 nasion에 대한 상대적인 상악골의 전방성장이 통계적으로 유의성있게 크게 일어났으며, 전방견인 장치에 의한 시상면에서의 부가적인 성장량은 소실되지 않고 안정하였다.

It has been studied that the protraction-headgear can improve skeletal relationships, dental relationships, and soft tissue-profile in class Ⅲ malocclusion, but its long-term effects have not been well documented. The purpose of this retrospective study was to analyze the short term and long term orthopedic effects for the maxilla on the sagittal plane. The treated group comprised 24 girls with Class Ⅲ malocclusion who came to the Yonsei University Dental Hospital, Department of Orthodontics and used protraction-headgear before monarchy with a mean total observation period of 3.7 years. And the control group comprised 25 girls with normal sagittal, vertical skeletal relationships who came to the Yonsei University Dental Hospital, Department of Orthodontics before monarchy and received only orthodontic treatment with a mean total observation period of 4.6 years. The stable structural superimposition method was used for cephalometric data analysis, and the obtained data was analyzed statistically. The following results were obtained:

1. The reaction to the protraction-headgear showed individual varieties between two extremes, that is the forward growth of maxillary body including nasal floor and only forward growth of A point excluding the maxillary body.

2. Significantly more forward growth of maxilla and that of maxilla relative to nasion was found with the treatment group compared with the control group, indicating stability of the additional growth by the protraction-headgear on the sagittal plane.
