Abstract

치주질환에 의해 파괴된 골내낭 치료에는 골이식술 및 차단막을 이용하는 수술이 보편화되고 있는데 이중 calcium sulfate는 최근 저렴하고 인체에 유해하지 않으며 골형성을 중진 시킨다는 보고가 있다. 이에 본 연구는 성견에서 실험적으로 하악 소구치 근원심에 폭이 4mm, 깊이가 4mm 되도록 1면 골내낭을 형성한 후, 치은 박리소파술만을 시행한 부위를 대조군으로, DFDB(80%)와 Calcium sulfate(20%)를 혼합사용하여 이식한 것을 실험 1군으로, DFDB(80%)와 Calcium sulfate(20%)를 혼합사용하여 이식한후 Calcium sulfate로 덮

은 것을 실험 2군으로 설정하고 술후 8주후에 실험부위를 적출하여 조직학적으로 비교관찰하여 다음과 같은 결과를 얻었다.

1. 신생골은 대조군에서 70%, 실험1군에서 93%, 실험2군에서 89%의 형성을 보였고, 대조군에 비해 실험군에서 많은 증가 양상을 보였으나 실험군 간의 차이는 없는 것으로 보였다.

2. 신생 백악질의 형성은 대조군에서 96%, 실험1 군 100%, 실험 2군에서 100% 관찰할 수 있었다.

3. 결합조직 유합은 대조군에서 30% 정도 보이며, 결합조직은 치근과 평행하게 배열되어 있었다. 실험 1군에서는 7%, 실험2 군에서는 11% 형성되어 있었으며, 신생골 형성 부위로의 결합조직 유입은 없었다. 대조군, 실험군 모두에서 염증세포의 침윤정도는 미미하였다.

4. 실험 8 주 후, 모든 실험군에서 calcium sulfate는 흡수되어 보이지 않았고,DFDB 이식편들을 관찰할 수 있었다.

이상의 결과로 보아 1 면 골내낭에서 탈회냉동건조골과 calcium sulfate의 흔합사용 및 차단막으로 사용할 때 생체반응이 우수하고 상피나 결합조직의 유입을 효과적으로 막을 수 있으며, 이식재의 유실을 적게하여 신생 치조골의 형성및 신생 백악질 형성에 효과적

이라 할 수 있다.

[영문]

There have been numerous bone graft materials in Priodontics, in attempt to reach the main goal of periodontal therapy, i.e. the regeneration of periodontal tissue lost due to destructive periodontal diseases.

The present study investigates the Effect of composite graft of DFDB and Calcium Sulfate with and without Calcium Sulfate barrier in Periodontal 1 wall intrabony defects in Dogs. Following the initiation of general anesthesia by I.V administration of 40mg/kg of Pentobabital, second premolar was extracted and full thickness flap was elevated. The crown portion of premolars was removed. Exposed root canal were sealed with Caviton and covered completely with flap. After the healing period of 8 weeks, the surgical sites were opened and 1 wall intrabony defects were created, and treated with flap operation alone(control group), with composit graft of 80% DFDB and 20% Calcium sulfate (test 1 group), with composite graft of DFDB and calcium sulfate with calcium sulfate membrane (test 2 group).

Healing response were histologically observed after 8 weeks and the results were as follows :

1, Changes in alveolar bone level was 70 % in the control group, 93 % in the test group Ⅰ, 89 % in the test group Ⅱ. There was a significant difference between control and two test groups. There was a no significant differences between test

group Ⅰ and test Ⅱ. Hut, regard to bone volume, thin bone formation was observed in control group.

2. New cementum formation was 96 % in control group, 100 % in the test group Ⅰ, 100 % in the test group Ⅱ, There was a no significant difference between control and two test groups, There was a no significant difference between test group Ⅰ and test Ⅱ.

3. The lengths of connective tissue adhesion was 30 % in the control, 7 % in the test group Ⅰ, Ⅱ % in the test group Ⅱ. There was no significance between control and two experimental groups.

4. After 8weeks, calcium sulfate was resorbed absolutely and was not obserbed. but DFDB particle was not resorbed.

These results suggest that the use of composite graft of allogenic DFDB and Calcium sulfate with and without Calcium sulfate barrier in periodontal 1 wallintrabony defects has a little effects on connective tissue adhesion ,but has significant effects on new alveolar bone and new cementum formations, and prevent downgrowth of epithelium and connective tissue effectively.
