아토피 피부염 환자의 병변피부에서 집먼지 진드기 알레르겐 특이 T림프구 클론이 분리되었고, 표피 랑게르한스 세포 표면에 IgE-부착 수용체의 존재가 확인되면서, 아토피 피부염 병인에서 피부로 침투된 항원의 전달과정과 IgE 특이성 면역반응의 유발요소로서 피

부 및 점막에 위치한 랑게르한스 세포의 중요성이 대두되고 있다. 아토피 피부염의 병인에 관한 연구로서 집먼지 진드기 항원에 대한 세포매개성 면역반응에 관여하는 랑게르한스 세포의 역할을 규명하고자 본 연구에서는 1997년 10월부터 1998년 2월까지 세브란스병원 알레르기 클리닉에 내원하여 아토피 피부염으로 진단된 18명의 환자를 대상으로 다음과 같은 실험을 시행하였다. 첫째, 아토피 피부염 환자의 정상피부와 병변피부로부터 조직 생검 후 면역조직화학 검사와 면역형광 유량 세포분석을 시행하여 IgE 부착 랑게르한스 세포를 분석하였다. 둘째, 정상피부와 병변피부로부터 분리한 랑게르한스 세포를 환자의 말초혈액 T 림프구에 첨가한 후 T 림프구의 증식반응을 비교하였으며, 혈액 호산구 수, 혈청 총 IgE 및 집먼지 진드기 항원 특이 IgE 등을 측정하여 T 림프구 증식반응과의 연관성 여부를 분석하였다. 셋째, 항원 특이성을 알아보기 위해 집먼

지 진드기 항원과 Candida albicans 항원으로 T 림프구 증식반응실험을 시행하여 정상피부와 병변부위의 랑게르한스 세포의 항원전달능력을 비교하여 다음과 같은 결과를 얻었다.

1. 면역 조직화학 검사상 CD1a 양성 및 IgE 양성세포가 아토피 피부염 환자의 정상표피에 비해 병변부위에서 더 많이 관찰되었으며, 면역형광 유량 세포분석상 CD1a양성 및 IgE 양성 세포들은 병변부위에서 얻어진 랑게르한스 세포 부유액에서 더 많았다.

2. 환자 말초혈액 T 림프구에 병변피부 랑게르한스 세포를 처치한 후의 증식지수는 환자 정상피부의 랑게르한스 세포를 처치한 군의 증식지수보다 높았다.

3. 환자 병변피부의 랑게르한스 세포를 처치한 후 관찰된 T 림프구의 증식지수는 통계학적인 유의성은 없었으나 임상증상의 중중도와는 정비례하고, 혈청 총 IgE치 및 집먼지 진드기 항원 특이 IgE치와는 역비례하는 경향을 보였다.

4. 혈중 호산구가 높은 환자군에서 환자 병변피부 랑게르한스 세포를 처치한 후의 T 림프구 증식지수는 혈중 호산구가 낮은 군의 증식지수에 비해 유의하게 높았다.

5. 집먼지 진드기 항원을 처치한 환자의 병변피부 랑게르한스 세포에 의한 T 림프구 증식지수는 칸디다 항원을 처치한 군의 증식지수에 비해 유의하게 높았다.

이상의 결과로 보아 아토피 피부염 환자의 피부에 알레르겐 노출에 의해 활성화된 랑게르한스 세포가 존재하여 T 림프구에 항원을 전달함으로써 T 림프구의 활성을 유도하여 면역반응 유발에 관여할 것으로 생각된다.

The house dust mite antigen specific T lymphocytes have been isolated from the skin lesion of atopic dermatitis patients, and the existence of Fcε receptor to IgE has been proved on the surface of Langerhans' cells. Thus the role of Langerhans' cells in the IgE-mediated antigen presentation to T lymphocytes in the

pathogenesis of atopic dermatitis has been implicated.

In this study, we tried to verify the role of Langerhans' cells in the pathogenesis of atopic dermatitis. From Oct 1997 to Feb 1998, 18 atopic patients who visited the Allergy Clinic of Severance Hospital were included in this study after taking an informed consent. First, the skin specimens were obtained by shaving biopsy in lesional and non-lesional skin. By immunohistochemical staining, we analyzed the morphological findings of IgE**+ and CD1a**+ cells. From the skin specimens, the Langerhans' cells were enriched. By flow cytometry, the existence of IgE' and COla**+ cells in the Langerhans' cell-enriched suspension was analyzed. Second, patient's peripheral T lymphocytes were co-cultured with Langerhans' cell-enriched suspension for the T lymphocyte proliferation test. Patient's eosinophil count, serum total IgE, serum house dust mite specific IgE were measured, and clinical evaluation grade was evaluated to find out the relationship between the T lymphocyte proliferation test and these parameters. Third, to investigate antigen specificity, we compared the antigen presenting ability of Langerhans' cells in the T lymphocyte proliferation using either house dust mite antigen or Candida albicans antigen.

The results are as follows.

1. The CD1a**+ and IgE**+ cells were more frequently observed in lesional skin than non-lesional skin. The flow cytometry results revealed that CD1a**+ and IgE**+ cells were more abundant from lesional skin than non-lesional skin.

2. The PI(proliferation index) of the proliferation response of co-culture of patient's peripheral T lymphocytes with lesional Langerhans' cells was higher than that of patient's peripheral T lymphocytes with non-lesional Langerhans' cells

3. The PI of the proliferation response of co-culture of T lymphocytes with patient's lesional Langerhans' cells showed a trend in which the PI was proportional to clinical severity and inversely proportional to serum total IgE and house dust mite antigen specific IgE. However, these findings were not statistically significant.

4. The PI of the proliferation response of co-culture of patient's peripheral T lymphocytes and Langerhans' cells of patients with a high blood eosinophil level(%) was significantly higher than that of patients with a low blood eosinophil level.

5. The PI of the proliferation response of co-culture of patient's peripheral T lymphocytes and patient's Langerhans' cells with house dust mite antigen was significantly higher than that of patient's peripheral T lymphocytes and patient's Langerhans' cells with C. albicans antigen.

These results might suggest that the Langerhans' cells activated by a certain allergen existed in the skin of atopic dermatitis patient and they were able to provoke the immune response by activating T lymphocytes.
