최근 심폐소생술(cardiopulmonary resuscitation)의 장비와 방법의 발달로 인하여 심폐소생술 후 순환회복이 가해진 환자의 수가 증가하였지만 그러한 환자들에 있어서 객관적으로 신경학적 예후를 정확하게 측정하는 검사가 없는 실정이다. 뇌척수액내에 존재하는 뇌신경계의 몇가지 효소들을 척수천자를 이용하여 검사하는 방법들이 있으나 인공호흡기와 다른 감시 장비를 부착하고 있는 환자를 대상으로 하는 것은 정확한 측정이 힘들고 위험부담이 따르기 때문에 가능한 실용적이고 간편한 검사방법을 알아보고자 하였다.

1996년 6월부터 1998년 3월까지 21개월동안 연세대학교 의과대학 부속 세브란스병원 응급진료센터에서 심폐소생술을 시행받은 환자 29례를 대상으로 하였다. 심정지 후 혈청 NSE(neuron-specific enolase)를 24시간, 48시간 간격으로 분석하였고 신경학적 예후는 CPC

(cerebral performance category)를 이용하여 분류하였으며 환자별로 연령, 심정지의 목격여부, 초기 심전도의 형태, 심폐소생술 시간 및 CPC에 따른 점수를 조사하였다.

총 29례의 환자중 CPC 분류상 1-3에 해당하는 경우가 9명(31.0%)으로 예후가 좋은 군으로 정의하였고, CPC 분류상 4, 5에 해당하는 경우가 20명(69.0%)으로 예후가 나쁜 군으로 정의하였다. 예후가 좋은 군은 평균 연령이 40세 였고, 나쁜 군은 55세였다. 초기 심전도 소견은 양측 군에서 별다른 차이가 없었으며, 심정지 목격 여부 88.8%와 30.0%로 예후가 좋은 군에서 의미있게 많았다. NSE의 정상치는 10.5±1.8 ng/ml이지만 예후가 좋은 군에서 순환회복 후 24시간,48시간 때의 수치는 33.8±9.3 ng/ml, 34.0±4.73 ng/ml이었

고 예후가 나쁜 군에서 순환회복 후 24시간, 48시간 때의 수치는 99.5±11.7 ng/ml, 114.6±15.8 ng/ml로 예후가 나쁜 군에서 통계학적으로 의미있는 p 값을 보였고, 상대작용특성 곡선상에서 48시간 때의 혈청내 NSE 값이 24시간 때보다 신경학적 예후를 보다 더 민감하게 반영하는 것으로 나타났다. 심폐소생술 시간은 양측 간에 통계학적 차이는 없었다.

혈청 NSE를 이용하여 심폐소생술 후 신경학적 예후를 대략적으로 예측할 수 있고, 순환회복 군에서 신경학적 예후를 보다 더 정확하게 예측할 수 있는 것은 순환회복 후 24시간 보다는 48시간 때의 NSE수치로 뇌척수액 검사를 위한 신경방사선과적 촬영이나 척수천자와 같은 위험부담이 큰 검사를 대체하여 치료방침을 결정하는데 큰 도움을 줄 수 있을 것으로 사료된다.

Patients in cardiac arrest have gradually had good prognosis for development of well-equipped devices and means using in cardiopulmonary resuscitation (CPR), but there are no definitively objective methods determining of their neurologic outcome. The diagnostic procedures such as computed tomography (CT), electroencephalography, or evoked potentials and The examination of cerebrospinal fluid (CSF) may be hazardous and involve many technical difficulties in cardiopulmonary unstable patients.

The purpose of this study is to determine that the assessment of serum NSE level could provide a reliable predictor of early prognostic factors of neurologic outcome after cardiac arrest.

This studied 29 patients (16 were men) with a mean age of 50.8±13.1 years. Prospective, observational study was performed from April 1996 to March 1998 at the Severance hospital. Serum NSE levels were analysed twice at 24 and 48 hours after return of spontaneous circulation (ROSC). 9(31.0%) of 29 patients showed good outcome defined as cerebral performance category (CPC) 1-3, and 20 (69.0%) of them showed bad outcome defined as CPC 4, 5. In the good outcome group they revealed 33.8±9.3 ng/ml at 24 hours, 34.0±4.73 ng/ml at 48 hours (p<0.0003). But in the bad outcome group, levels of NSE in serun were 99.5± 11.7 ng/ml at 24 hours,

114.6± 15.8ng/ml at 48 hours (p<0.0001).

After ROSC, serum NSE level at 48 hours had more important meaning in determination of therapeutic strategies in acute phase and may be replaced invasive methods such as neuroradiologic examinations and spinal tap.

This result shows that NSE value in serum is more precise at 48 hours after ROSC and It may represent a valuable, noninvasive, and useful clinical tool for prediction of neurologic outcome after cardiac arrest.
