진행성 간암환자에서 간동맥내 Adriamycin 주입군과 Cisplatin주입군간의 치료성적의 비교연구

[한글]

원발성 간암은 우리 나라에서 가장 흔한 고형암 중의 하나이며, 발견 당시 종괴가 너무 크거나, 문맥혈전증, 간외 전이, 재한된 간기능 등의 이유로 수술등 완치를 기대할 수 있는 치료가 불가능한 경우가 대부분이어서 예후가 극히 불량한 것으로 보고되고 있다.

특히 진행성 간암인 정우 그 예후가 극히 불량할 뿐만 아니라 효과적인 치료법도 거의 없는 실정이다.

최근에 진행성 간암 환자에서 간동맥내 항암제 주입으로 효과를 보았다는 보고가 있으며 cisplatin의 반복 투여가 효과적이라는 보고가 있었다. 본연구에서는 진행성 간암 환자에서 간동맥 색전술에 많이 사용되는 adriamycin과 cisplatin의 치료효과를 비교해 보고자 수술적 절제가 불가능하며 국소적으로 치료가 어렵다고 판단되는 TNM 병기 IVa기만을 대상으로 두 약제간의 간동맥 주입 치료효과를 비교하고자 하였다.

따라서 본 연구에서는 1992년에서 1996년 사이에 세브란스 병원에 내원했던 환자 중 수술적 절제가 불가능했던 TNM 병기 IVa기인 92예에서 cisplatin 투여군과 adriamycin 투여군으로 나누어 반응률과 생존률의 차이를 비교하였다.

1. 전체 대상 간암환자의 평균연령은 54.5세였으며, 남녀비는 83(90.2%) : 9(9.8%)이었고, 원인은 B형 간염이 62예(67.3%)로 가장 많았고, 종양의 크기는 10cm 이하가 48%, 10cm 이상이 52% 었으며, 종양의 형태는 다결절형이 40.3%으로 가장 많았으며, 문맥이 침범된 경우는 45.7%였다. Child 분류상 A군이 88%, B군이 12%였다. AFP은 400ng/ml이상이 51%였다. 평균 추적기간은 평균이 10개월이었고, 2개월에서 57개월 사이였다. 치료군의 숫자는 adriamycin 투여군이 44예, cisplatin 투여군이 48예였고, 색전술을 시행한 경우는 58.7%, 시행치 않은 경우가 41.3%였으며, 생존은 17예, 사망 67예, 미확인이 8예였다.

2. 총 92명의 치료군 중 6개월, 12개월 및 24개월의 생존률은 adriamycin 투여군이 79%, 42%, 16.5%였고 cisplatin 투여군이 67.4%, 30.2%, 12.7%이었다. 치료방법에 따른 생존률을 산출한 결과 중앙 생존기간은 adriamycin 투여군이 12개월로 cisplatin 투여군의 8개월에 비해 길었으나 통계학적으로 유의하지 못하였으며(p>0.05), 생존률에 영향을 주는 요인으로써는 종양의 크기, 문맥 침범 유무 등이 통계학적으로 유의하였다.

3. 전체 환자에서 부분 반응 이상의 치료 반응을 보인 예가 adriamycin 투여군은 49%, cisplatin 투여군은 28%였고, 1년 누적 생존율은 adriamycin 투여군이 42%, cisplatin 투여군이 27.7%이었으며 통계학적으로 유의한 차이는 없었다(p>0.05).

4. 치료로 인한 합병증으로 오심 및 구토, 복부동통, 고열등이 주 증상으로 양군간에 비슷한 양상을 보였으며, cisplatin 투여군에서 일시적 신기능의 저하가 2예, 간부전으로 사망한 경우가 1예 있었다. 사망의 원인으로는 간암의 파열(22%), 간부전(43%), 상부위장관 출혈(15%)등이 주 원인이었다.

결론적으로 수술적 치료가 불가능하며 국소적 치료가 어렵다고 판단되는 진행성 간암 환자인 TNM 병기 IVa군에서의 adriamycin 투여군과 cisplatin 투여군을 비교하여 보았을 때 두군간에 생존률과 반응률의 통계학적인 유의한 차이는 없었다.

[영문]

Primary hepatocellular carcinoma is one of the most common cancer in Korea. Especially in advanced hepatoma, its prognosis was very poor and it has been found that hardly any curative approaches can be seen, so to this point transarterial chemoembolization was regarded as a most effective therapy in those conditions.

Recently, there were so many trials with adriamycin in transarterial chemoembolization. In Japan, some authors reported that the cisplatin was more favorable response than other chemotherapeutic agents, thereafter there were many doctors who prescribed cisplatin than adriamycin in transarterial chemoembolization today, but there was few studies about the effect of transarterial chemoembolization with cisplatin, furthermore in Korea.

So, We studied about the survival rate and response rate of advanced hepatoma especially on TNM stage IVa and to decide what is more favorable method for the treatment of advanced hepatoma.

From 1992 to 1996 in Severance hospita, 92 cases of patients with advanced HCC were selected for this study treated with transarterial chemotherapy.

1. The average age was 54.5yrs, the male and female ratio was 83:9, HBsAg positive rate was 67.3% overall. The tumor size above 10cm is 52%, the multinodular type was most frequent(40.3%), the portal vein thrombosis rate was 45.7%, the mean survival duration showed 9month(median), and number of lives were 17 patients, and

the dead were 67 patients.

2. We studied about the response rate and survival rate between adriamycin(group Ⅰ) and cisplatin(group Ⅱ) treatment. The results showed that there was no statistical differences in response rate and survival rate(p>0.05). But the median survival time was 12 month and 8 month respectively between group Ⅰ and group Ⅱ And the meaningful prognostic factors of survival rate was tumor size and the extent of portal vein thrombosis.

3. The complications of these patients were assessed and the main complaints were nausea, vomit abdominal pain and fever. In cisplatin group there was transient renal impairment in 3 patients but none in adriamycin group. The causes of death were hepatoma rupture(22%), liver failure(43%) and UGI bleeding(15%).

In conclusion in advanced HCC(TNM IVa) patients between transarterial adriamycin chemtherapy and transarterial cisplatin chemotherapy group, there was no differences in survival rate and response rate.
