Abstract
[한글]

본 연구는 정상아 어머니와 저출생체중아 어머니의 양육 3∼4개월 후의 스트레스를 비교하여 저출생체중아를 양육하는 어머니에게 스트레스를 알아보아 효율적인 간호중재 개발을 하는데 기초자료를 제시하기 위한 서술적 조사 연구이다.

1997년 10월 1일부터 11월 20일에 걸쳐, 저출생체중아의 경우 서울, 경기 소재 4개 병원 신생아 집중치료실에서 퇴원한지 3∼4개월 후에 외래 방문을 한 어머니 중 대상자 선정기준에 맞는 어머니에게 직접 설문지를 배부하여 자료수집 하거나 또는 해당병원에 등록된 영아의 어머니중 대상자 선정기준에 맞는 어머니에게 질문지와 함께 반송봉투를 동봉하여 우편으로 발송하여 총 25명을 대상으로 자료를 수집하였다. 대조군인 정상아의 경우는 출생 후 3∼4개월 후에 예방접종을 위해 Y대학교 부속병원 육아지도실과 Y구 보건소를 방문한 어머니 중 대상자 선정기준에 맞는 어머니에게 직접 질문지를 배부하여 총 31명을 대상으로 자료수집을 하였다.

구도구로는 Abidin(1990)의 PSI(Parenting Stress Index)를 번역하고 수정 보안한 도구로 종속변수인 양육 스트레스를 측정하였으며 양육 스트레스 관련요인중 중요한 변수의 하나인 사회적 지지는 Brant와 Weinert(1981)가 개발한 개인자원질문지(Personal Resource Questionnaire;PQR)를 서미혜와 오가실(1993)이 번역한 도구를 사용하여 측정하였고 자료분석은 SAS window program을 이용하여 통계처리 하였다.

연구 결과는 다음과 같다.

1. 저출생체중아 어머니의 양육 스트레스는 정상아 어머니의 양육 스트레스보다 유의하게 높았으며 양육 스트레스의 세가지 영역인 부모 영역, 부모-자녀 관계 영역, 아동 영역 모두에서 저출생체중아 어머니의 스트레스가 높은 것으로 나타났다.

저출생체중아 어머니의 구체적인 양육 스트레스 내용은 아이의 정상발달 여부에 대한 걱정, 아이가 잘 먹지 않는 것, 질병이환시나 이유없이 울고 보챌 때의 대처방안, 아이와의 의사소통 문제, 수면 부족, 사회생활의 제한, 육체적 피로, 여가시간 부족, 자아정체성 상실, 부모님과의 양육 방법에 대한 의견충돌, 가족의 도움 부족, 경제적 부담등 정상아 어머니와 크게 다르지 않았으나 그 외에 성장발달 지연과 질병이환시 그 원인이 저출생체중아로 태어난 것 때문이라 생각하여 죄책감을 느끼고, 미래에 아이에게 합병증이 나타날지 모르는 것에 대해 불안감을 느끼며 그로 인한 책임감으로 아이와 친밀감을 느끼는데 어려움을 겪기도 하여 저출생체중아 출산에 대한 압박감과 책임감을 양육시 계속하여 스트레스로 경험하고 있었다.

2. 정상아 어머니의 경우 학력이 높을수록, 수입이 높을수록, 사회적 지지가 낮을수록, 양육 스트레스가 높게 나타났다.

3. 저출생체중아 어머니의 경우 어머니가 인지하는 아이의 성장상태와 발달상태가 느릴수륵, 어머니가 아이의 건강상태를 약하다고 인지할 경우, 어머니의 학력이 높고 사회적 지지 점수가 낮을수록 양육 스트레스가 높게 나타났다.

이상의 연구결과를 종합해 볼 때 저출생체중아 어머니는 정상아에 비해 스트레스가 높으므로 양육 스트레스를 감소시키기 위한 간호중재를 실시해야 하는데 효과적이고 실질적인 중재를 위하여 저출생체중아 어머니의 양육 스트레스 영향을 주는 요인으로 밝혀진 아이의 특징에 대한 이해, 아이의 성장과 발달 상태에 대한 바른 정보, 어머니와 아이의 건강관리법에 대한 교육과 죄책감 감소를 위한 교육을 실시하고, 사회적 지지를 높일 수 있는 방안을 마련해야 할 것이다.

[영문]

The main purpose of this study is to compare parenting stress of mothers of low birth weight infants with parenting stress of mothers of normal infants at 3∼4 months after discharge.

The data were collected from October 1 to November 20, 1997 from 25 mothers of low birth weight infants and 31 mothers of normal infants.

The research instruments were the Parenting Stress Index:PSI developed by Abidin(1990) and revised by the researcher and Personal Resource Questionnaire:PRQ developed by Brant & Weinert's(1981) and revised by Seo & Oh(1993).

Descriptive statistics, T-test, correlation and multiple regression were used to analyze data.

The results were as follow:

1. The parenting stress of mothers of low birth weight infants is higher than that of mother of normal infants. Three domains of parenting stress that include parental domain, parent-child domain and child domain all showed the same result. The mothers of low birth weight infants reported to have guilty feelings, anxiety, and the accountability regarding their infants' condition which the mothers of normal infants did not have.

2. Among the mothers of normal infants, higher level of education, higher income, and lower social supports, were significantly related to higher parenting stress.

3. Among the mothers of low birth weight infants, perception of lower growth and development of their infants, poorer health status, higher education, and lower social supports, were significantly related to higher parenting stress. In conclusion, the nurses in NICU and other pediatric clinics should develop teaching plans for the characteristics, growth and development, and the management of common health problems of low birth weight infants. Also the mothers of these infants need support and counseling to decrease guilty and anxiety.
