구강점막의 편평상피세포암종 발생단계에서 세포성장자극 및 억제인자 수용체가 어떻게 분포하는지를 관찰하고 전암병소에서 암발생 위험도를 평가하는 표지자로서 epidermal growth factor receptor(EGFR)과 transforming growth factor(TGF)-β type Ⅱ receptor(TβR Ⅱ)를 사용할 수 있는지 알아보고자, 구강점막의 전암병소, 편평상피세포암종의 암종부위와 암종인접부위에서 EGFR과 TβR Ⅱ의 분포를 비교 분석하였다. 저자는 1990년 1월부터 1995년 8월까지 연세대학교 치과대학 구강병리학 교실에서 구강점막의 백색병소(white lesion)로 가진되어 생검 의뢰된 조직 중 백반증 20예와 편평상피세포암종으로 진단되어 수술 받은 15예를 대상으로 EGFR과 TGF-β type ll 수용체에 대한 항체를 이용한 면역조직 화학적 검색을 통해 다음과 같은 결과를 얻었다.

1. EGFR의 발현은 백반증의 경우, 상피 증식만 있었던 예에서는 기저층에 주로 양성반응을 보였고 국소적으로 유극층 하방 1/3까지 양성 반응을 볼 수 있었다. 이형성이 동반된 경우는 이형성 부위까지 양성 반응이 증가된 소견을 보였다.

2. 편평상피세포암종에서 EGFR은 전부위에 양성 반응을 보였으며, 암종 주위 조직에서는 이형성이 심해질수록 EGFR 분포도 증가되는 경향을 보여 조직학적인 이형성정도와 EGFR의 발현 정도는 비례하는 양상을 보였다.

3. TβR Ⅱ의 발현은 백반증의 경우, 상피 증식에서는 기저층을 제외한 전층에 양성반응을 보였으며, 이형성이 동반된 경우는 이형성 부위에서 음성 반응을 보였고, 그 상층부에서는 양성 반응을 보였다.

4. 편평상피세포암종에서는 기저층을 포함한 전 부위에서 TβR Ⅱ에 양성 반응을 보였다. 특이한 소견으로 암종 주위 조직에서는 백반증과 달리 기저층을 포함하여 이형성이 동반된 부위에서도 양성 반응을 보여 전층에서 양성 반응이 관찰되었다.

이와 같은 결과를 종합해보면 EGFR의 발현은 조직학적으로, 악성 단계로 진행됨에 따라 비례하여 증가되는 양상을 보였으며, TβR Ⅱ는 악성으로 진행됨에 따라 기저층 및 이형성 부위가 음성에서 양성 반응으로 변화되었다. 이와 같은 결과로 미루어보아 구강암의 발생 과정에서 EGFR은 비교적 초기에, TβR Ⅱ는 암종으로 이행되는 시기에 나타나는 소견으로 생각되었으며, 두 성장조절인자 수용체의 발현양상이 암종 발생 과정의 표지자로 사용될 수 있을 것으로 사료되었다.

[영문]

The aims of this study were to observe the expression of epidermal growth factor receptor (EGFR) and transforming growth factor (TGF)-β1 receptor during the development of squamous cell carcinoma of oral mucosa, and to evaluate the usefulness of these receptors as markers for the risk of malignant transformation in oral premalignant lesions. This study used 20 cases of oral leukoplakia and 15 cases of squamous cell carcinoma, which were examined at the Department of Oral Pathology of the Dental Hospital of Yonsei University from January, 1990 to August, 1995. Monoclonal anti-EGFR antibody and polyclonal anti-TGF-β1 type Ⅱ receptor (TβR Ⅱ) antibody were applied for immunohistochemical study.

The results were as follows

1. Expression of EGFR was mainly focused in the basal layer and partly in lower 1/3 of prickle cell layer in the cases of leukoplakia showing hyperplasia, whereas in the dysplastic leukoplakia, the expression was extended up to the dysplastic

lesion.

2. The expression of EGFR were found in the entire of layer squamous cell carcinoma. The expression of EGFR in the adjacent area of carcinoma was increased with the epithelial dysplasia and resection margins showed the same pattern as that of oral leukoplakia. These findings indicated that the expression of EGFR were correlated with the degree of dysplasia.

3. The expression of the TβR ll was examined in the prickle cell layer except for the basal and keratinized layer in the leukoplakia with hyperplasia. Also in the leukoplakia with dysplasia, expression of TβR Ⅱ was demonstratedn either in the dysplastic areas nor ,basal cell layers.

4. In the squamous cell carcinomas, TβR Ⅱ were expressed in the entire layer. Both dysplastic adjacent area and resection margin of squamous cell carcinoma showed positive reaction for the TβR Ⅱ even in the basal cell layer.

From these results, the expression of EGFR were increased with the progression to malignancy. The expression pattern of TβR Ⅱ was altered to be positive in the basal cell layer with the progression to malignancy. These results suggest that expression of EGFR appeared to be early event and TβR Ⅱ may related to malignant transformation during oral carcinogenesis. The expression pattern of EGFR and TβR Ⅱ may contribute to predict the risk of the development to carculorrla in oral premalignant lesions.
