수술제거한 특발성 망막하신생혈관막의 투과전자현미경을 이용한 초미세구조

망막하신생혈관막, 특히 황반하신생혈관막은 황반부 병변 중에서 시력 상실을 초래하는 조건 중 하나이다. 저자는 특발성 망막하신생혈관막의 수술적 제거후 투과전자현미경을 이용한 초미세구조의 연구를 통해 생체 내 연구에서 망막하신생혈관막의 병태 생리를 이해하고자 하였다. 1993년 4월부터 1995년 12월까지 신촌세브란스병원 안과에서 망막하신생혈관막 중 특발성 황반하신생혈관막 진단 하에 수술 제거한 환자 6명의 6안을 재료로 외래 및 입원 기록 조사와 투과전자현미경 검사를 시행하였다. 투과전자현미경 소견 상 내피 세포로 둘러싸인 혈관강이 망막하신생혈관막이 초미세구조의 기본 구조로, 내피세포는 주변세포 혹은 망막색소상피세포로 둘러싸여 있었다. 간질에서의 주된 세포는 망막색소상피세포, 섬유세포, 세포질 내에 멜라닌 색소를 함유한 근섬유모세포 등 이었다. 대식세포, 형질세포와 림프구를 포함한 만성염증세포가 관찰되었으며 정상 망막 구조인 광수용체세포의 외부분절이 관찰되었다.

이상 소견으로 망막하신생혈관막의 생성은 망막색소상피의 이동과 변성을 통해 기능이나 구조 이상을 회복하고자 하는 생체 반응으로 생각되었으며 망막하신생혈관막의 발생은 염증세포 군집, 섬유모세포 출현, collagen과 섬유소 생성 및 신생혈관 발생 등 창상 치유 과정에서 발생하는 일련의 과정과 유사점이 많아 망막하신생혈관막의 병태생리과정은 비정상적 창상치유과정으로 생각된다.

Subretinal neovascular membrane is the condition of visual impairment among the macular lesions. This study is aimed to understand pathophysiology of subretinal neovascular membrane by ultrastructural study. The six cases of surgically excised idiopathic subretinal neovascular membrane were examined ultrastructurally. The vascular channels, which were lined by endothelial cells and surrounded by pericyte or retinal pigment epithelium were the main feature of subretinal neovascular membrane. Other cellular components were retinal pigment epithelium, myofibroblast containing melanosome, chronic inflammatory cells including macrophage, histiocyte, plasma cell and lymphocyte, fibrocyte and outer segment of photoreceptor cell. It is considered that development of subretinal neovascular membrane is similar to wound healing process. It is suggested that pathophysiology of subretinal neovascular membrane may be a process of abnormal wound healing.
