Comparison of the effectiveness of embolic agents for bronchial artery embolization

[영문]Purpose: To compare between the results of embolic agents of bronchial artery embolization for treatment of hemoptysis.Materials/Methods: Bronchial artery embolization was performed in 430 patients with hemoptysis. Patients were divided into three groups. Group 1 included 74 patients treated with gelfoam particle (1x1x1mm), group 2 included 205 patients with polyvinyl alcohol(PVA) (355-500um), and group 3 included 151 patients with PVA (500-710um). Among group 1, the cause of hemoptysis in 44 patients were found to have pulmonary tuberculosis, 18 with bronchiectasis, 5 with lung cancer, and 7 with other diseases. In group 2, 148 patients had pulmonary tuberculosis as a cause, 38 with bronchiectasis, 13 with lung cancer, and 6 with other diseases. In group 3, the source was pulmonary tuberculosis in 94 patients, bronchiectasis in 36, lung cancer in 14, and other diseases in 7. We categorized the results as initial success rate, short-term result, and long-term result. Then, the initial success rate and short-term result were retrospectively compared among 3 groups 1 month after the procedure, and long-term result was compared 12 months after the procedure. Results: Neither the initial success rate (Group 1; 86%, 2; 87%, 3; 90%) nor short-term result (Group 1; 72%, 2; 74%, 3; 71%) showed significant difference among 3 groups. However, the long-term result of group 1 showed lower success rate than other 2 groups (Group 1; 45%, 2; 63%, 3; 62%). No significant difference was discovered in the long-term result between group 2 and 3.Conclusion: PVA is a more optimal choice than gelfoam particle for bronchial artery embolization to improve long-term result, and appropriate size of material needs to be selected to match upon vascular size.
