Abstract
[한글]목적: 항암치료 및 조혈모세포이식 등을 받고 이차적으로 면역결핍 상태에 있는 소아 환자들은 세균감염에 감수성이 높다. 세균의 다약제 내성이 증가하고, 의료의 발달에 따라 암환자의 장기 생존률이 증가되고 있는 시기에 균혈증의 원인균 동정과 내성균의 파악은 중요하다. 이에 본 연구는 단일기관 내 이차 면역결핍증 환아에서 균혈증을 일으키는 균주들과 그들의 항생제 내성에 대한 경향을 분석하고, 이러한 환자들에서 균혈증의 위험 요소를 규명하고자 하였다.

방법: 2001년 1월부터 2008년 1월까지 연세대학교 의과대학 세브란스 어린이 병원에서 균혈증이 확인된 환아 중 이차 면역결핍증에 해당하는 164명을 대상으로 후향적으로 의무기록을 검토하였다.

결과: 164명의 이차 면역결핍 환아들에게서 균혈증을 일으킨 총 180주 중에서 그람양성균, 그람음성균 및 진균의 비율은 각각 52.2%(94), 39.4%(71) 및 8.3%(15)이었다. 그 중 Staphylococcus epidermidis가 가장 흔한 균종이었으며, 그람음성균 중에서는 Pseudomonas spp.가 가장 많은 부분을 차지하였다. 그람양성균의 39.4%(37/94)가 ciprofloxacin에 내성이었고, erythromycin에는 77.7%(73/94), clindamycin에는 55.3%(52/94)의 내성률을 보였다. Vancomycin에 내성을 보인 균주는 Enterococcus faecium이 2주 있었으나 teicoplanin에 내성인 균주는 없었고, methicillin에 내성인 Staphylococcus는 88.3%(53/60)였다. 그람음성균 중 E. coli는 71.4%(10/14)에서 ampicillin에 내성을 보였으며, Pseudomonas spp.는 imipenem에는 33.3%(8/24), ceftazidime에는 16.7%(4/24), cefepime에는 25.0%(6/24)의 내성률을 보였다. 또한, 중심정맥관을 가진 환아들에게서 중심정맥관이 없는 군에서보다 그람양성균과 진균의 분리가 많았으며(p=0.02), 다균성 균혈증의 81.3%가 중심정맥관과 관련 있음을 확인하였다. 중증 호중구 감소증에서는 그람음성균에 의한 균혈증의 빈도가 의미있게 많았고(p=0.003) 그에 따른 생존률의 차이에도 통계학적 유의성을 보였다(p=0.01). 균혈증 발생 당시 혈청 C 반응 단백이 높을수록 균혈증에 대한 생존률이 의미있게 감소하였다(p=0.003).

결론: 중심정맥관의 사용이 균혈증의 중요한 위험인자이므로 면역 결핍 환아에서 장기간 치료로 인해 중심정맥관을 삽입을 하여야 하는 경우 신중히 결정하여야 하며, 중심정맥관을 삽입한 환아에서 발열이 있을 경우 그람양성균에 의한 균혈증을 감안하여 항생제를 선택하고 반드시 항진균제의 투여도 고려하여야 한다. 특히 다균성 및 다약제 내성균에 의한 감염의 가능성도 항상 염두에 두어야 할 것으로 생각된다. 또한 증상 발현 당시 절대 호중구 수와 혈청 C 반응 단백을 예후 인자로 반영하여 균혈증을 미리 진단하고 집중 치료를 하는 것이 생존률을 향상시키는 데에 기여할 것으로 판단된다.



[영문]Purpose: Pediatric patients with secondary immunodeficiency receiving chemotherapy or hematopoietic stem cell transplantation are more sensitive to bacterial infection. The aims of this study are to analyze the current pathogens causing bacteremia and their antimicrobial resistance patterns in pediatric patients with secondary immunodeficiency receiving chemotherapy or hematopoietic stem cell transplantation and to understand the risk factors of bacteremia in these patients.

Method: A retrospective medical records review of 180 episodes of bacteremia in 164 pediatric patients with secondary immunodeficiency between January 2001 and January 2008 was conducted at Yonsei University Severence Children's Hospital.

Results: Among the 180 episodes noted in 164 pediatric patients with secondary immunodeficiency, the incidence rate of gram-positive organisms was 52.2% (94/180), of gram-negative organisms was 39.4% (71/180) and of fungus was 8.3% (15/180). Staphylococcus epidermidis was the most common pathogen and Pseudomonas spp. was the most common among gram-negative organisms. Gram-positive organisms were found to be resistant to ciprofloxacin 39.4% (37/94), erythromycin 77.7% (73/94), and clindamycin 55.3% (52/94). Two organisms were resistant to vancomycin and all gram-positive organisms were susceptible to teicoplanin. Among the antimicrobial patterns of gram-negative organisms, it was found that E. coli was resistant to ampicillin 71.4% (10/14), Pseudomonas spp. was resistant to imipenem 33.3% (8/24), ceftazidime 16.7% (4/24), and cefepime 25.0% (6/24). A higher incidence rate of gram-positive organisms and fungus in patients with central venous catheters was reported than in patients without central venous catheters (p=0.02). The polymicrobial bacteremia was associated with central venous catheters in 81.3%. The incidence of gram-negative bacteremia was significantly increased in severe neutropenic patients (p=0.003), and the survival rate was also distinctly different (p=0.01). The serum C-reactive protein level at initial diagnosis of bacteremia was higher, and the survival rate of bacteremia decreased (p=0.003).

Conclusion: We should decide to carefully insert a central venous catheter because that is an important risk factor of bacteremia in immunodeficiency patients. If the patient with a central venous catheter is febrile, antibiotics should be selected to be efficacious against gram-positive organisms and the addition of an antifungal drug should be considered. Especially, we always reflect on the possibility of polymicrobial sepsis or multi-drug resistance organisms. It will contribute to improving the survival rate that we consider the serum C-reactive protein level and absolute neutrophil count when symptoms are revealed as prognostic factors and give a diagnosis of bacteremia early and intensive treatment.
