Synergy model을 이용한 심혈관 중환자 간호 직무 기술서 개발

본 연구는 Synergy model을 기반으로 심혈관 중환자 간호 직무 기술서를 개발하는 방법론적인 연구이다. Synergy model이란 대상자의 8가지 특성과 간호사의 8가지 특성이 잘 부합하면 최적의 결과를 낳는다는 모델로서, 이를 기반으로 개발된 심혈관 중환자 간호 직무 기술서는 그 동안 직무 기술서 개발 시 도외시 되었던 대상자의 욕구를 반영함으로써 환자 중심의 간호를 지향하는데 기여할 것으로 기대된다.연구의 결과를 요약하면 다음과 같다.1. 심혈관 중환자 간호 직무 기술서는 Synergy model이 제시한 간호사의 8가지 특성을 기반으로 간호사의 직무 기술서의 대분류를 작성하고, 중분류는 기존의 간호 직무 분석 연구를 토대로 작성하였다. 그 결과 8개의 대분류, 20개의 중분류, 130개의 세부직무가 제시된 심혈관 중환자 간호 직무 기술서 초안이 개발되었다.2. 심혈관 중환자 간호 직무 기술서 초안에 대해 2007년 10월 31일부터 11월 5일까지 임상 타당도를 검증하였다. 그 결과 수행 빈도는 거의 매일 한다가 임상적 판단에서 20명(90.5%), 옹호 및 윤리적 도움에서 10명(47.2%), 간호 실무에서 11명(47.8%), 협동에서 7명(30.4%), 조직적 사고에서 3명(11.4%), 다양성에의 반응에서 5명(20.9%), 임상적 연구에서 2명(9.2%), 학습촉진에서 6명(27.1%)으로 나타났다. 필요성 부분에서는 모든 항목에서 22명(96.6%) 이상이 필요하다고 응답하였다.3. 심혈관 중환자 간호 직무 기술서 초안에 대해 2007년 11월 27일부터 12월 5일까지 내용 타당도를 검증한 결과, CVI(Content Validity Index) 0.70 이상인 세부직무와 전문가 의견을 수렴하여 수정, 보완, 삭제, 추가하여 총 1개의 세부직무를 삭제하고, 5개의 세부직무를 추가하였으며, 13개의 세부직무를 수정, 1개의 세부직무를 재분류 하였다. 이를 토대로 대분류 8개, 중분류 20개, 세부직무 134개로 구성된 최종 심혈관 중환자 간호 직무 기술서가 개발되었다.Synergy Model을 기반으로 개발된 심혈관 중환자 간호 직무 기술서는 심혈관 중환자실 간호사의 역할 규명 및 확대에 기여할 것으로 여겨지며, 변화하는 시대의 새로운 간호정책 개발에 기초 자료로 활용될 수 있을 것이다.

[영문]Based on the Synergy Model, this study provides a methodological ground in developing a job description in cardiovascular critical care. The Synergy Model suggests that when eight components of the nurse correspond well to eight characteristics of the patient, the outcome is optimized. The Synergy Model-based job description in cardiovascular critical care will contribute to patient-centered nursing focusing on the characteristics of the patient which have been rather marginalized in developing job descriptions.1. This study employs the eight skills of the nurse in the Synergy Model as the basis of eight major categories of the job description in cardiovascular critical care. Twenty sub-categories and one hundred thirty specified tasks are also developed in this study that are indebted to the previous scholarship.2. The study examines the clinical validity of the drafted job description in cardiovascular critical care. From October 31 to November 5, 2007, I asked twenty-three nurses in cardiovascular critical care at one hospital in Seoul on their clinical experience. The results are as follow. In frequency, they answered 'almost everyday' in clinical decision 20(90.5%), advocacy 10(47.2%), caring practice 11(47.8%), collaboration 7(30.4%), system thinking 7(30.4%), response to diversity 5(20.9%), clinical inquiry 2(9.2%), and facilitation of learning 6(27.1%). In necessity, they answered 'it is needed' at every specified tasks more than 22(96.6%) persons.3. The study also examines the validity of the draft's content and adjusts the draft reflecting the result of CVI and following advice of the experts. The changes involve eliminating one specified task, adding five more, re-classifying one, and modifying thirteen specified tasks. In the end, the study finalizes the job description that consists of eight major categories, twenty sub-categories, and one hundred thirty-four specified tasks.The Synergy Model-based job description in cardiovascular care in this study will enable the nurses in the field to identify their roles more clearly and further their capabilities. It will also serve as fundamental resources in developing new nursing policies in the changing era.
