복잡 심기형을 가진 청소년의 삶

본 연구는 근거이론 방법을 이용하여 한국의 사회 문화적 맥락 안에서 복잡 심기형을 가진 청소년의 삶의 과정을 확인하고 분석하여 실체이론을 개발하기 위해 시도되었다. 이를 위한 연구 질문은 ‘복잡 심기형을 가진 청소년의 삶의 과정은 어떠한가?’이다.본 연구의 수행을 위해 서울 Y대학병원에서 윤리심의 위원회의 승인을 받았으며 참여자는 14세에서 35세의 복잡 심기형을 가진 대상자 중 연구 참여에 동의한 12명이었고 자료수집기간은 2007년 5월부터 11월까지였다. 자료 수집은 연구 참여자와 의 심층면담에 의해 이루어졌고 자료가 포화될 때까지 이론적 표출을 시도 하였으며 자료 분석 방법으로는 지속적인 비교분석방법을 사용하여 개방 코딩, 축 코딩, 선택 코딩의 단계에 따라 분석하였다.연구결과에 따르면, 복잡 심기형을 가진 청소년의 삶은 ‘심장병을 가진 나의 유일성을 찾는 여정’으로 나타났으며 삶의 과정은 위기의 단계와 자아 인식의 단계, 자아 정립의 단계로 나타났다. 위기의 단계에서 참여자들은 ‘남과 다른 나’를 인식하면서 자기존재의 위기감을 가지게 되었으며 이 과정에서 주로 사용되는 전략은 또래와의 동질성 추구이었다. 자아 인식의 단계에서 성취감과 자신감을 느끼게 됨으로서 자신의 역할수행 정도가 충분하다고 생각하는 참여자들은 자기존중을 많이 하였으나 성취감을 느끼지 못해 역할수행을 충분히 하지 못한다고 생각하는 참여자들은 자기존중 보다는 자기비하를 하게 되었다. 자아 정립의 단계에서는 독립성을 성취하고 지식을 습득한 참여자들은 독립적 관리를 주로 하면서 미래를 개척하고자 하였으나 독립성을 성취하지 못하고 지식습득을 하지 못한 참여자들은 의존적 관리를 주로 하면서 현실과 타협하였다.본 연구의 결과는 ‘심장병을 가진 나의 유일성을 찾는 여정’의 과정을 겪고 있는 복잡 심기형을 가진 청소년들을 이해할 수 있는 계기를 마련해 주며 이들을 위해 개별적인 간호중재전략을 개발할 수 있는 기틀을 제공해 준다.

[영문]The present study was aimed at analyzing the process of life of adolescent with Complicated Heart Disease (CHD) on the basis of sociocultural context of Korea and developing the substantive theory by using Grounded Theory. The question for the study was "What is the process of life of adolescent with CHD?".This study was approved by Institutional Review Board at Y University Hospital in Seoul. 12 patients in the age of 14 to 35 participated in the study, and the data was collected from May to November in 2007. The data collection was performed by in-depth interview, and theoretical sampling was tried until the data was saturated. The collected data was analyzed by making continuous theoretical comparisons in the step of open coding, axial coding, and selective coding.The result of the present study showed that the life of adolescent with CHD is the 'journey to finding uniqueness of oneself with CHD', and that the process of life consists of crisis stage, self-recognition stage, and self-establishment stage. In the crisis stage, the participants recognized themselves as different from others, had feeling of crisis on self-existence, and pursued the homogeneity of the same age. While the participants having sufficient role-performance based on feeling of achievement and confidence in self-recognition stage showed lots of self-esteem, those having lack of role-performance showed self-reproof. In self-establishment stage, the participants who got the independence and knowledge prepared for the future by independent management, whereas those in the opposite case conformed to the realities of life by dependent management.The result of present study helps understand the adolescent with CHD in ‘journey to finding uniqueness of oneself with CHD', and provides the basis for developing the strategy of nursing intervention for the individual patients.
