중등도 및 고도 무지외반증에서 근위 역 갈매기형 절골술과 Ludloff 절골술의 결과 분석

근위 역 갈매기형 절골술(1군)과 Ludloff 절골술(2군)의 임상적 결과를 조사하고 방사선학적 결과를 비교하고자 하였다.2001년 10월에서 2006년 10월 사이 연세대학교 세브란스병원 정형외과에서 중등도 및 고도 변형의 무지외반증을 보이고 12개월 이상 추시 가능했던 106명, 134예를 대상으로 하여 후향적 연구를 시행하였고 두 군간의 임상적, 방사선학적 결과를 비교하여 통계 분석 하였다.술 후 6주의 통증 점수는 1군 15.2±5.8점(범위,0-30점), 2군 28.8±4.6점 (범위,20-30점)으로 근위 역 갈매기형 절골술은 술 후 6주째 K-강선의 제거에 따른 불편감과 초기 고정력이 Ludloff 절골술보다 상대적으로 약해 통증점수에 의미 있는 차이가 있었다. 고도 변형의 무지외반증은 제 1, 2 중족골간각의 수술 전후 차이가 1군 13.2도, 2군 18.6도로 Ludloff 절골술이 보다 유리하였고(p<0.05), 반면에 제 1 중족골 길이 단축은 수술 전후의 차이가 각각 0.02, 0.05로 근위 역 갈매기형 절골술의 단축이 보다 적었으며(p<0.05) 각 교정이 클수록 중족골 단축이 많아짐을 알 수 있었다. 또한 Ludloff 절골술시 절골의 방향, 각도 그리고 회전축의 위치가 합병증 예방에 매우 중요할 것으로 사료된다.

The following study was intended to investigate a clinical and radiographic comparative analysis of proximal metatarsal reverse chevron osteotomy(Group I) and Ludloff osteotomy(Group II) for moderate to severe hallux valgus deformity.In this retrospective study, the total of 134 clinical cases in 106 patients who presented with moderate to severe hallux valgus deformity at orthopedic surgery in Yonsei Severance Hospital from October 2001 to October 2006 with a minimum of one year post operative follow-up was evaluated, and the statistical comparative analysis of both clinical and radiographic evaluation between two study groups was conducted.Statistically, there was a significant difference in the pain score at six week after operation between two study groups; 15.2±5.8 (range of 0 to 30) for group I, and 28.8±4.6 (range of 20 to 30) for group II. Proximal metatarsal reverse chevron osteotomy revealed discomfort due to removal of K-wire at postoperative 6 week and weaker initial fixation strength of proximal metatarsal reverse chevron osteotomy than that of Ludloff osteotomy. Ludloff osteotomy is more profitable in terms of severe hallux valgus deformity because the difference of 1st and 2nd intermetatarsal angle (IMA) before and after the operation showed 13.2 degree for group I and 18.6 degree for group II(p<0.05). However, the shortening of 1st metatarsal lengths before and after the operation were separately 0.02 and 0.05, which means the shortening length of proximal metatarsal reverse chevron osteotomy was smaller than that of Ludloff osteotomy(p<0.05). Radiological evaluation reveals a higher shortening level of 1st metatarsal bone with a larger differential of angle correction. Moreover, the direction of osteotomy, angle and position of the rotation axis are more significant in order to prevent complications in Ludloff osteotomy.
