본 연구는 임신과 출산이 우리나라 여성의 구강건강에 미치는 영향을 조사하기 위해 전국적인 대표성이 있는 2000년도에 수행된 출산력조사와 국민구강건강실태조사 자료를 통합하여 분석하였으며, 다음과 같은 결과를 얻었다.1. DMFT index와 DMFS index는 출산횟수가 증가할수록 그 값이 증가했다. 그러나 유산횟수의 증가에 따라 DMFT index와 DMFS index는 증가하는 양상을 보이지는 않았다.2. FS-T index와 T-health index는 출산횟수와 유산횟수가 증가할수록 그 값이 감소하는 경향을 보였다.3. 치주상태를 나타내는 T-CPI는 유산횟수가 증가할수록 그 값이 증가했고, 출산횟수의 경우 0회를 제외하고 비슷한 양상을 보였다.4. 상실치아수의 경우 출산횟수와 유산횟수가 증가할수록 그 값이 증가하는 경향을 보였다.5. 상관분석 결과 출산요인과 가장 관련성이 높은 치아우식증 관련 변수는 DFS index였다.6. DFS를 종속변수로 두고 다중회귀분석을 시행하여 가구소비수준, 치과방문빈도, 치과방문이유, 출산횟수, T-CPI 등이 여성의 치아우식에 유의한 영향을 끼치는 결과를 얻었다(p<0.01).7. T-CPI(6분악 CPI지수 총합)를 종속변수로 두고 다중회귀분석을 시행한 결과, 출산횟수, DFS index, 가구소비, 연령, 교육수준, 치과방문빈도 등이 유의하게 여성의 치주조직에 영향을 끼침을 알 수 있었다(p<0.01).8. 포아송 회귀분석 결과에서 여성의 치아상실에 유의한 영향을 미치는 변수는 출산횟수, 가구소비, 치과방문빈도 및 이유, 외상경험, 부정치과시술 등이었으며, 출산횟수가 증가할수록 여성의 치아상실위험이 0회에 비해서 1회는 1.37배(95% CI 0.96-1.95, p=0.08), 2회는 1.49배(95% CI 1.06-2.10, p=0.02), 3회 이상은 1.71배(95% CI 1.20-2.44, p=0.003) 증가했다.이상의 결과들을 종합해 볼 때 여성의 출산은 여성이 치아를 상실하게 되는데 유의한 영향을 미치는 위험인자로 볼 수 있다. 향후 이러한 연구의 결과를 바탕으로 본 연구의 한계점을 보완한 종적 연구가 필요하다고 사료된다.





[영문]

The purpose of this study is to clarify the influence of childbirth on oral health in Korean Women. For this, we used Korea National Oral Health Survey data in 2000 and Survey on the National fertility, family health and welfare data in 2000: Randomized and representative data of the Korean people. The results are as follows;1. The DMFT index and DMFS index had a positive relationship with increasing number of childbirths. However, there were no positive relationship between that indices and abortions.2. The FS-T index and T-health index had a negative relationship with increasing number of childbirths and abortions.3. T-CPI showed the same increasing trend with increasing number of abortions and childbirths except for non-childbirth women.4. The number of tooth loss increased with increasing number of childbirths and abortions.5. Correlation analysis showed that DFS index had the highest correlation with the number of childbirths and abortions among the caries variables.6. Multiple regression analysis, which placed DFS index with dependent variable, showed that the number of childbirths, T-CPI, house cost level, frequency of dental visiting, and reasons for visiting the dentist had statistical significance with dependent variable(p<0.01). However, significance according to abortions was not shown.7. Multiple regression analysis, which placed T-CPI with dependent variable, showed that the number of childbirths, DFS index, house cost level, education level, frequency of dental visiting, and age had statistical significance with dependent variable(p< 0.01). However, significance according to abortions was not shown.8. The result of Poisson logistic regression analysis, which placed the number of tooth loss with the dependent variable revealed that the number of childbirth, house cost level, frequency of dental visiting, reasons for visiting the dentist, facial trauma experience, history of unlicensed dental treatment were significantly associated with the number of tooth loss. The odds ratios of tooth loss were 1.37 (95% CI 0.96-1.95, p=0.08) between the non-childbirth and one childbirth women, 1.49 (95% CI 1.06-2.10, p=0.02) between the non-childbirth and two childbirths women, 1.71 (95% CI 1.20-2.44, p=0.003) between the non-childbirth and more than three childbirths women. These results suggest a significant relationship between childbirth and oral health, and that childbirth is a risk indicator for tooth loss in Korean women. A systematic study to determine the precise relationship while considering the limitations of this study should be done.
