만성 족관절 외측 불안정성의 Ahlgren-Larsson방법에 의한 재건술

서론 : 만성 족관절 외측 불안정성을 갖는 환자에서 Ahlgren-Larsson 방법을 이용한 외측인대 재건술 후 그 결과를 알아보는데 있다.재료 및 방법 : 만성 족관절 외측 불안정성에 대하여 Ahlgren-Larsson 방법으로 수술한 환자중 평균 24개월 이상 추시 관찰이 가능하였던 24명(25례 족관절)을 대상으로 하였으며 수술시 지지대, 골막, 관절막을 포함한 전거비 인대와 종비인대의 피판을 외과 전하부에 3개의 고정 나사못을 삽입하여 인대를 재건하였으며, 수술시 족관절내의 외측부 동반 병변에 대하여 진단 및 치료를 시행하였다.결과 : 주관적인 평가에서 통증, 부종은 의의있는 호전을 보였 다(p<0.05). 기능적 평가상 활동도와 안정성에서 호전된 결과를 보였다(p<0.05). AOFAS score상 최우수14례(90점이상), 우수 7례(80-90점), 양호 2례(70-79점), 불량 2례(70점미만)의 결과를 보였으며, 2례에서 재발성 안정이 관찰되어 84%에서 만족할만한 결과를 얻었다. 동반 병변으로 관절내 유리체가 12례, 경골 원위부 골극 4례, 거골 외측부 연골 손상이 2례 있었다.결론 : Ahlgren-Larsson 방법은 만성 족관절 외측 불안정성의 교정을 위한 유용한 수술방법이고 수술시 거골 외측면 동반 병변에 대한 동시 평가가 필요할 것으로 사료된다.

[영문]Purpose : To study the result of Ahlgren-Larsson reconstruction for the chronic ankle lateral instability.Materials and Methods : Twenty five ankles in 24 patients who were operated with Ahlgren-Larsson reconstruction for the correction of lateral instability. All patients were followed up more than 24 months. The flaps of anterior talofibular and calcaneofibular ligament including retinaculum, periosteum and capsule were fixed with three suture anchor system in bone trough which was located in the anteroinferior surface of lateral malleolus.Results : In subjective evaluation score of pain and swelling was improved significantly(p<0.05). The level of activity and grade of activity were also improved in functional evaluation(p<0.05). The final AOFAS scale was graded as following : excellent in 14(90 points or more), good in 7(80-89 points), fair in 2(70-79 points), poor in 2(less than 70 points). Ankle instability were recurred in two cases. The overall satisfactory results were obtained in 84%. Intraarticular loose body(12 cases), osteophyte on distal tibia(4 cases), osteochondral lesion of lateral talus(2 cases) were the associated pathologic lesion.Conclusion : The Ahlgren-Larsson reconstruction was effective for the correction of chronic ankle lateral instability. Exploration and treatment for associated pathologic lesion in lateral ankle joint could be done simultaneously.
