Abstract
[한글]본 연구는 호텔업 종사자들을 대상으로 근골격계 증상정도를 조사하고, 증상 특성과 작업특성에 맞는 맞춤 운동프로그램을 설계하여 그 효과를 확인함으로써 실제적인 근골격계질환 관리방안 마련을 위한 중재 개발에 기여하고자 시도된 유사실험연구로서 비동등성 대조군 사전사후 설계이다.연구대상은 일개 호텔에 소속된 종사자 280명을 근접 모집단으로 하여 구조화된 설문지로 근골격계 증상을 확인하고 우선적으로 예방관리가 필요한 부서를 선정하였으며 최종 연구 대상은 실험군 16명과 업무의 성격이 비슷하면서 실험의 확산효과 가능성이 적은 부서의 대조군 16명이었다. 맞춤 운동프로그램을 2006년 10월 23일부터 12월 3일까지 6주간 매일 2회 10분씩 직접 지도하면서 운동프로그램이 지속적으로 할 수 있도록 면담 및 SMS 문자메시지 등을 활용하여 행동변화에 영향을 미치도록 하였다.연구도구는 미국 산업안전보건연구원(NIOSH)의 기준을 근거로 한국산업안전공단에서 작성한 근골격계 부담 작업 유해요인조사 지침의 증상조사표를 바탕으로 김정애(2001)가 근로자 근골격계 통증의 자가 평가에 관한 연구에서 개발되었던 설문지를 본 연구에 맞게 수정, 보완한 도구를 사용하였다.자료 분석은 SPSS 12.0 window 프로그램을 이용하였으며, 동질성 검증은 Fisher's exact test와 t-test로 분석하였고 근골격계 증상완화 운동프로그램 실시에 따른 효과 분석은 McNemar test, paired t-test, t-test로 통계 처리하였다.본 연구의 결과를 요약하면 다음과 같다.1. 전체 대상자의 근골격계 증상 유병률은 80.4%였으며, 신체 부위별 증상 호소는 어깨(20.75%), 허리(19.81%), 다리/무릎(17.75%), 목(14.02%), 발/발목/발가락(13.08%), 손/손목/손가락(11.03%), 팔/팔꿈치(3.55%)순으로 높았다. 대상자 가운데 우선적 예방관리가 필요한 실험부서의 증상 유병률은 89.2%로 NIOSH 진단기준에 의한 자각증상 호소율은 어깨 87.5%, 발/발목/발가락 81.3%, 다리/무릎 68.8%, 허리 62.5%, 손/손목/손가락 50.0%, 목 43.8%, 팔/팔꿈치 25%의 순이었다.2. 대상자의 인구사회학적 특성과 근골격계 관련 특성과의 관계를 분석한 결과 통계적으로 유의한 차이가 없어 동질한 것으로 나타났다. 단, 증상지속기간(p=.003)과 증상빈도(p=.000)는 유의한 차이가 있었다.3. 맞춤 운동프로그램 실시 전, 후 신체 부위별 자각증상 변화를 분석한 결과, 실험군은 발, 발목, 발가락을 제외한 모든 신체 부위에서 자각 증상이 감소하는 경향을 보였으며, 어깨부위(p=.004)와 손, 손목, 손가락(p=.008)은 통계적으로 유의하게 감소하였다. 대조군에서 신체 각 부위별 근골격계 자각증상 호소율은 유의한 변화를 보이지 않았다.4. 맞춤 운동프로그램 실시 전, 후 근골격계 증상 특성을 분석한 결과, 대조군은 유의하게 증가한 반면, 실험군은 증상지속기간이 4.06에서 2.38로, 증상강도는 5.38에서 3.44로 유의하게 감소하였으며 증상빈도는 실험군은 3.75에서 2.94로 감소하였으나 통계적으로 유의하지는 않았다.결론적으로 호텔 종사자들을 대상으로 근골격계 증상완화를 위한 맞춤 운동프로그램의 효과를 확인 하였다. 따라서 대상자의 증상특성과 작업특성에 맞는 맞춤 운동프로그램이 실제적이고 효율적인 근골격계질환 관리방안 마련을 위한 중재로 적극적인 활용이 요구된다.

[영문]This study is to examine the musculoskeletal symptoms of hotel employees and it aims at developing an intervention program to prevent and ease musculoskeletal symptoms by designing and testing a customized exercise program based on symptoms and task characteristics of different individual.The subjects were 280 employees who works at the hotel. A self-administered questionnaire was used in order to identify any musculoskeletal symptoms among those employees. The group with a higher degree of symptoms were categorized by their department. Afterwards, each experiment and control groups, consisting of 16 employees respectively, was assembled. The controlled group was from the department of similar operational characteristics, but with the minimum likelihood of being influenced by the experimental group. The customized 10-minute exercise program was conducted twice a day for 6 weeks between 23 October and 3 December 2006. Counselling and SMS message via mobile phone were provided to the participants for the period of 6 weeks as a means of encouragement.A questionnaire developed by Kim (2001) to self-measure a musculoskeletal symptom was modified as a tool of measurement. Kim’s study was influenced by the surveillance criteria from the National Institute of Occupational Safety and Health (NIOSH) and Korea Occupational Safety & Health Agency (KOSHA).The data was analysed with the method of Fisher’s exact test, t-test, McNemar test, paired t-test by SPSS 12.0 window program.The result of this study are summarized as follows.1. A total of 80.4% workers were discovered suffering from musculoskeletal symptoms among 280 subjects. The prevalence of symptoms was Shoulders (20.75%), Lower back (19.81%), Leg/Knee (17.75%), Neck (14.02%), Foot/Ankle/Toe (13.08%), Hand/Wrist/Finger (11.03%) and Arm/Elbow (3.55%). The 89.2% of the experimental group had musculoskeletal symptoms. The self-awareness of any musculoskeletal symptom was Shoulders (87.5%), Foot/Ankle/Toe (81.3%), Leg/Knee (68.8%), Lower back (62.5%), Hand/Wrist/Finger (50.0%), Neck (43.8%) and Arm/Elbow (25%).2. There was homogeneous characteristics in all of subjects, however a difference was found between symptom duration (p=.003) and symptom frequency (p=.000).3. A symptom on a subject before and after implementing the customized exercise program for the period of 6 weeks, a decline of subjective symptom in most body parts, particularly in Shoulders (p=.004) and Hand/Wrist/Finger (p=.008), was significantly declined. In the control group did not significantly changed.4. The analysis of the musculoskeletal symptom characteristics conducted before and after the customized exercise program shows the increase of symptom in the controlled group. However, the symptom duration decreased significantly from 4.06 to 2.38, the symptom intensity from 5.38 to 3.44. The symptom frequency was decreased from 3.75 to 2.94 showing no significant difference statistically.In conclusion, it has been proved that the Customized Exercise Program helps reducing the musculoskeletal symptom among the hotel employees. Therefore, it has been asserted that the Customized Exercise Program according to the symptom and task characteristics of hotel employees need to be introduced in preventive and proactive manners in order to ease and prevent the musculoskeletal symptom.
