일 산촌 지역의 알코올 관련 8년 사망률

Abstract

[한글]한국은 다른 국가에 비해 알코올 사용이 많고 알코올 사용 장애의 유병률도 높지만 알코올 관련 사망률에 대한 조사는 거의 없다. 이 연구는 한 지역사회 주민을 대상으로 알코올 사용 및 알코올 의존이 8년 동안의 사망률에 미치는 영향을 확인하고자 하였다. 1988년에 일 산촌 지역 주민 1,058명을 대상으로 알코올 사용 및 남용에 관련된 전반적 실태 조사를 한 바 있다. 2006년에 대상지역을 다시 방문하여 8년 전 피조사자들의 사망여부를 조사하였다. 총 989명을 추적하였고 그 중 사망자는 150명(15.2%)이었다. 사회인구학적 자료, 알코올사용 및 알코올 의존 심각도(SADQ), 흡연에 따른 사망률을 분석하였다. 알코올 관련 사망률을 조사하기 위해 여러 가지 변수를 통제하는 로지스틱 회귀분석을 하였다. 알코올 의존 심각도(SADQ) 15점 이상의 알코올 의존군의 사망률(OR)은 1.86 [95%신뢰구간=0.93-3.75]이며 흡연을 통제했을 때는 1.74[95%신뢰구간=0.86-3.51]이었다. 현재 흡연자의 사망률은 1.90[95%신뢰구간=1.13-3.19]이었다. 일일평균음주량이 12g이하의 소량 음주자의 사망률(OR)은 0.82, 24g이하의 보통 음주자는 1.00, 24g을 넘는 과량 음주자는 1.21이었다. 알코올섭취량과 사망률의 관계가 J유형이었으나 통계적 의미는 없었다. 이 연구는 국내 유일의 알코올 관련 사망률에 대한 연구이고, 유럽이나 미국 등의 연구와 유사한 결과를 나타내지만 그들의 연구에 비해 대상군이 적으며 또한 국내 한 지역사회의 결과라는 것이 제한점이다.

[영문]The aim of this study was to demonstrate the effect of alcohol intake and alcohol dependency on mortality over eight years in a rural community in South Korea. Among 1,058 men and women (mean age, 55.0±16.87) in a rural community who reported their alcohol use status in 1998, 150 men and women died during eight years of follow-up. We performed the descriptive analysis of the dead with the basic demographic data, alcohol related data such as daily alcohol amount consumed, Alcohol Dependence Questionnaire (SADQ), drinking behavior and the other health behaviors. In order to detect odds ratio(OR) for alcohol related mortality, we controlled various confounding factors using binary logistic regression. Those who are above 15 of SADQ showed higher mortality rate with an adjusted odds ratio(OR) of 1.86 [95% confidence interval(CI)=0.93-3.75]. Those who had no more than 12g of daily alcohol showed lower mortality than non-drinker or drinker of above 12g per day. The results showed the J-shaped relationship between alcohol intake and intermediate-term mortality as in western countries.
