The effect of calcium aluminate bone cement incorporating LiF-maleic acid on the bone regeneration of 1-wall intrabony defects in beagle dogs

Abstract
[한글]치주 치료의 최종 목적은 질환의 진행을 정지시키는 것뿐만 아니라 이미 파괴된 지지조직들을 기능적으로 재생시키는데 있다. 합성골 이식재 중에서 calcium aluminate는 우수한 조직 적합성, 뛰어난 숙주 적응성, 우수한 광화능력으로 인한 경조직, 연조직의 재건 등을 보인다고 보고되었으며, 경화촉진제인 LiF와 강도증진제인 maleic acid를 첨가하여 빠른 경화시간과 우수한 기계적 특성을 나타내는 calcium aluminate 골시멘트를 개발하였다.이번 연구에서는 6마리 성견에 4×4 mm 1면 골 결손부를 형성하여 치근 활택술 만을 시행한 군을 대조군으로, 새로 개발된 calcium aluminate 골시멘트를 이식한 군을 실험군으로 하여 12주 후에 희생시키고 관찰하여 다음과 같은 조직학적 결과를 얻었다.1. 대조군에서는 notch 상방으로 약간의 신생 백악질이 치근면을 따라 생성되었고, 신생골 형성이 notch 상방으로 관찰되었다. 결합조직 내에 염증세포의 침윤은 미약하게 관찰되었다.2. 실험군에서는 대조군에 비해 신생 백악질 및 신생골의 형성이 관찰되지 않았다.3. 이식된 calcium aluminate cement는 흡수되지 않고 남아 있었으며, 이식된 골시 멘트 입자 주위로 섬유성 결합조직이 피막화 되어 있었다.4. 이식된 골시멘트 입자 주위로 침투된 결합조직 내에는 염증세포가 관찰되었다.이상의 결과를 바탕으로 재료의 특성상 조작성 및 scafold로서 결손부의 형태를 유지하는 능력은 우수하여 골 결손부의 충전재로서 즉, 치조제 증대술 등에는 유용하게 사용될 수 있지만 치주조직의 재생을 위한 골이식재로서는 좀더 많은 연구가 필요하리라 생각된다.

[영문]The ultimate goal of periodontal treatment is not only to deter the progression of the disease, but also to functionally regenerate the already destroyed supporting tissues. One of the synthetic bone graft materials, calcium aluminate has an excellent biocompatibility, host adaptability, and mineralization capacity, which facilitates hard and soft tissue reconstruction. In this study, we use calcium aluminate bony cement incorporating LiF-maleic acid which was developed to have fast setting time in a low exothermic temperature and appropriate mechanical properties for the grafting of bony defects. This cement was grafted into the 1-wall bony defects of beagle dogs. The subject were sacrificed 12 weeks after surgery. In order to evaluate its regeneration capacity of the periodontal tissues and observe the bonding tendencies to the bone and the reaction of the surrounding tissues, we made some tissue slides and gain with the following results.1. In the control group, a slight amount of new cementum was formed along the root surface above the notch and new bone was formed above the notch. Inflammatory cell infiltration in the connective tissue was observed.2. In the experimental group, new cementum and new bone formation was not observed more than in the control group.3. The grafted calcium aluminate cement was not absorbed and fibrous connective tissue was encapsulized around the particles of the grafted bony cement.4. Inflammatory cells were observed in the infiltrated connective tissue around the grafted bony cement.In conclusion, because of the certain favorable properties, such as its easy manipulation and its ability to maintain the morphology of the defect site by acting as a scaffold, the newly developed cement is appropriate suitable for us as a graft material, and it can be used very effectively in alveolar augmentation. However, further study needs to be conducted before it can be used as a bone graft material for periodontal tissue regeneration.
