Abstract
[한글]

자궁내막증은 가임기 여성의 10-15%에서 발생하는 비교적 흔한 질환으로 자궁내막조직이 자궁강 외부에 착상되어 생리통, 성교통 및 만성 골반통 등의 증상을 일으키며 불임의 주요한 원인이 되고 있다. 자궁내막증의 주요 병리 기전으로 생리혈의 역류 현상과 유전적 또는 환경적 요인 등이 중요하게 인식되었으나 최근에 스테로이드 생산 이상 및 면역학적 체계 이상 등이 자궁내막증 병변의 착상 및 전이에 중요한 기전으로 대두되었다. 또한 자궁내막증 병변에서 스테로이드성 급성 조절단백질 및 aromatase 이상 발현이 PGE2에 의해 유도된다는 연구가 보고되었고, cyclooxygenase-2 (이하 Cox-2로 약함)의 발현이 증가되었다는 보고와 함께 자궁내막증의 병리기전에 관한 Cox-2에 대한 연구가 활발히 진행되어 왔다. 이에 자궁내막증에서 발현되는 Cox-2에 대해 조사하여 자궁내막증의 병리기전을 확인하는 것은 의미있는 일이라고 할 수 있겠다. 따라서 본 연구에서는 자궁내막증 환자에서 Cox-2 발현 정도 및 발현 양상에 대해 살펴보고, 자궁내막증의 여러 가지 임상적 요인과의 상관관계에 대해 평가하였다.본 연구는 2005년 4월부터 9월까지 연세대학교 의과대학 영동 세브란스 병원에 내원하여 자궁내막증 또는 기타 양성 부인과 질환으로 수술을 진행한 환자를 자궁내막증 환자군과 자궁내막증이 아닌 대조군으로 분류하여 각각 자궁내막과 자궁내막종 또는 다른 난소 병변의 조직을 채취하여 semiquantitive RT-PCR을 이용하여 Cox-2 발현 정도를 조사하였다. 자궁내막증 환자군의 Cox-2 발현 정도 및 발현 양상을 대조군과 비교하였고, 생리주기, 자궁내막증 병기, 혈중 CA125 수치 및 자궁내막종 크기에 따른 Cox-2 발현에 대해 분석하였다.자궁내막증 환자군의 자궁내막과 자궁내막종에서 Cox-2 발현이 대조군에 비해 통계학적으로 유의하게 증가하였고 (각각 p=0.035, 0.023) 생리주기에 따라 비교한 결과, 자궁내막증 환자의 증식기 자궁내막과 분비기 자궁내막종에서 Cox-2 발현이 통계학적으로 유의한 증가를 보였다 (각각 p=0.046, 0.002). 각각의 병변내에서 생리주기에 따른 Cox-2 발현의 변화는 통계학적으로 유의하지 않았고, 혈중 CA125 및 자궁내막종 크기는 자궁내막과 난소 병변에서 보이는 Cox-2 발현과 유의한 상관관계를 나타냈다.요컨대 자궁내막증 병변에서 증가된 Cox-2는 PGE2 생성을 유도함으로써 자궁내막증의 발생 및 병리 기전에 관여할 것으로 추정되며, 선택적 Cox-2 억제재는 자궁내막증 치료에 이용될 수 있을 것으로 사료된다. 또한 생리주기에 상관없이 자궁내막증 환자의 자궁내막에서 통계적으로 유의하게 증가되어 있는 Cox-2 발현을 볼 때, 자궁내막증 환자에서 자궁내막의 변화와 차이를 예측해 볼 수 있었다. 또한 여러 가지 임상적 요인들과 상관관계를 확인함으로써, 자궁내막증 진단하거나 치료 반응 평가, 재발 유무 확인 및 예후를 평가하는데 이용할 수 있는 가능성을 확인하였고, 자궁내막종의 직경이 커짐에 따라 Cox-2 발현이 증가되는 점으로 보아 Cox-2는 자궁내막증의 전이 및 진행 과정에 작용할 것으로 사료된다.



[영문]Introduction: Endometriosis, a common and complex gynecologic disorder, accounts for infertility, dysmenorrhea, dyspareunia and pelvic pain in 10%?15% of women of reproductive age and is a histologic disorder that is characterized by the presence of endometrial tissue in ectopic sites outside the uterus. Many studies have been reported about the pathophysiology of endometrioisis. Although retrograde menstruation theory and multiple genetic and environmental factors seem to be important, the critical factors that affect the implantation and propagation of endometriotic lesions are still largely unclear. Recent studies demonstrated that aberrant expression of steroidogenic acute regulatory protein and aromatase in endometriotic stromal cells is stimulated by the same molecule (PGE2). The study that the expression of Cox-2 increased in endometriotic lesions was reported recently and many researches were performed to find out the mechanism of Cox-2 involving the pathogenesis of endometriosis.Objectives: we studied to evaluate the expression grade and pattern of Cox-2 in endometriosis and to investigate the correlation between the expression of Cox-2 and several clinical characteristics (the stage of endometriosis, CA125 level, diameter of endometrioma).Materials and Methods: Eutopic endometrium was sampled by endometrial biopsy. Ectopic endometriotic tissues were sampled from endometriotic cysts in patients with endometriosis and ovarian tissue in control group was sampled frome other ovarian benign pathology. We isolated total RNA and synthesized cDNA. cDNA was amplified through semiquantitive RT-PCR and the products were loaded on agarose gel and electrophoresis was performed. The intensity of each band, after normalization with GAPDH mRNA, was quantified by the photographed gels with a densitometer.Results: The expression of Cox-2 in eutopic endometrium and ectopic endometriotic lesions of patients with endometriosis increased significantly (respectively p=0.035, 0.023). According to the menstrual phase, the expression of Cox-2 increased significantly in eutopic endometrium of proliferative phase and endometrioma of secretary phase in patients with endometriosis (respectively p=0.046, 0.002). The changes of Cox-2 expression in endometrium and ovarian lesion according to the menstrual phase were not significant statistically. The expressions of Cox-2 in endometrium and ovarian lesion were correlated significantly with serum CA125 and diameter of endometrioma.Conclusions: Cox-2 seems to be involved in pathogenesis of endometriosis through inducing the synthesis of PGE2 and selective Cox-2 inhibitor will be useful in treatment of endometriosis. We should consider the basal differences of Cox-2 expression in eutopic endometrium between endometriosis patients group and control group. At the point that there were significant correlations between Cox-2 expression and serum CA125 level or diameter of endometrioma, we assumed the possibility of using the Cox-2 expression level in the diagnosis, response evaluation and the prediction of prognosis of endometriosis and considered the involvement of Cox-2 in the pathogenesis of endometriosis by inducing the inflammatory process and angiogenesis within the endometrioma and the propagation of endometriosis.
