한국 성인의 사회경제적 요인 및 구강보건의식행태에 따른 functioning teeth index와 T-Health index의 분석연구

구강건강상태는 많은 사회경제적 요인과 구강보건의식행태들과 관련이 있다. 그동안 구강건강상태를 나타내는 지수로서 우식경험영구치지수(DMFT 지수)를 주로 이용해 왔으나, 그 지수의 많은 한계로 인하여 여러 사회경제적 요인 및 구강보건의식행태들과 구강건강상태와의 연관성을 규명하는데 어려움이 있었다.그 대안으로 Sheiham은 기능치아지수(Functioning teeth index, FS-T 지수)와 건강한 치아조직 지수(T-Health 지수)를 개발하여 발표하였다.본 연구에서는 정부주도로 최초로 시도된 2000년도 한국 구강건강실태조사 자료와 설문자료를 이용하여 FS-T, T-Health 지수, 그리고 가중치에 변화를 준 T-Health modified 지수의 타당도를 검토하기 위하여 기존의 DMFT 지수와 비교하였다.이를 위하여 사회경제적인 요인과 구강건강의식행태에 따른 DMFT 지수, FS-T 지수, T-Health 지수, T-Health modified 지수의 상관계수와 회귀계수 및 수정된 결정계수 등을 비교하여 다음과 같은 결과를 얻었다.1. FS-T 지수(수정된 결정계수=0.5317), T-Health 지수(수정된 결정계수=0.5127), T-Health modified 지수(수정된 결정계수=0.4959) 들을 이용한 모형이 DMFT 지수(수정된 결정계수=0.3244)를 이용한 모형보다 한국성인의 사회경제적인 요인과 구강보건의식행태에 대하여 설명력이 더 높았다.2. FS-T 지수, T-Health 지수, T-Health modified 지수는 한국성인의 사회경제적인 요인과 구강보건의식행태 중에서 나이, 성별, 교육정도, 구강진료기관 최근이용시기, 구강건강의 자가인식상태 등에 대하여 상관성이 있었고, 특히 구강건강의 자가인식상태가 각각의 지수의 변화에 가장 큰 영향력이 있었다.3. T-Health modified 지수의 다양한 형태로 건전치, 충전치, 우식치, 상실치의 가중치에 변화를 주었으나 기존의 T-Health 지수를 이용한 모형과 여러 T-Health modified 지수들을 이용한 모형들은 설명력에 있어서 큰 차이가 없었다.앞으로 구강건강상태와 연관성을 가지는 사회경제적인 요인과 구강건강의식행태를 연구할 때에는, 기존의 DMFT 지수와 함께 FS-T 지수와 T-Health 지수를 함께 고려하는 것이 바람직하다고 여겨진다.

[영문]The status of the oral health has relation with the socio-economic factor and oral health behaviour. But DMFT index has been some difficulties in figuring out the relationship between a socio-economic factor, oral health behaviour and the status of the oral health, due to their limitation. Nevertheless it has been mainly used as a standard index in these kind of researches so far.In the context, Sheiham has made an alternative indices, so called ''Functioning teeth index and T-Health index''.The study which was carried out to evaluate the indices such as FS-T, T-Health index, T-Health modified index by use of analysing the national oral health survey data 2000.The result of analysing can be as followed.1. The model of using FS-T index (Adjusted R-Square=0.5317), T-Health index (Adjusted R-Square=0.4959) is more persuasive into the explanation of the relation between the socio-economic factor and the general behaviour of oral hygiene than that of using DMFT index(Adjusted R-Square=0.3244).2. It can be said that those indexes such as FS-T index, T-Health index and T-Health modified index have relation with the factors that are ages, sex, educational background, the recent times of visiting dental clinic and self- cognition of the oral health. The study shows that it is the self cognition of the oral health that gives the most impact on the variation of the each index.3. The study shows that there is little differences in the various indexes of T-Health and T-Health modified, though it gave some variations in the weighted index.Therefore, FS-T index and T-Health index must be considered in the future studies of the relation between a socio-economic factor and oral health behaviour.
